AFFIDAVIT OF DISCREPANCY
I, _______________, of legal age, Filipino, married, and a resident of
____________________________, Concepcion, Tarlac, after having been duly sworn to in
accordance with law, do hereby depose and state:
1. That I was born to ______________________ on December 9, 1979;
2. That the said date of birth is reflected on my government issued
Identification cards, to wit:
a. PhilSys/National ID with No. ______________
b. Social Security System (SSS) ID No. _______________
c. Postal Identity Card with PRN ______________ P issued by
Philippine Postal Corporation
d. Solo Parent Identification Card with No. __________ issued by
MSWDO Concepcion, Tarlac.

3. That this date of birth, December 9, 1979, is likewise reflected in the


following official records/documents:
a. Certificate of Marriage with Registry No. 2007-1091, issued
by the Philippine Statistics Authority.
b. Baptismal Certificate from the Baptismal Register of San
Nicolas De Tolentino Parish Shrine, Capas, Tarlac.

4. That, however, upon obtaining a copy of my PSA Birth Certificate, I


discovered that my birth date is erroneously recorded as December 31,
1981, which, in truth and in fact, is the date of birth of my sister;
5. That the discrepancy in the record of my birth date has caused confusion and
inconvenience, particularly in the processing of the pension and other
benefits of my late husband with the GSIS and other government agencies;
6. That I hereby affirm that my correct date of birth is December 9, 1979, as
consistently reflected in my valid identification documents, marriage
certificate, and baptismal certificate.
7. That I am executing this affidavit to attest to the discrepancy in the records,
to confirm the correctness of my date of birth as December 9, 1979, and for
whatever legal purpose this may serve.
