AFFIDAVIT OF UNDERTAKING

I, Danica Paulino, of legal age, Filipino, single and a resident of


_____________________________, after having been duly sworn to be in
accordance with law do hereby depose and says:

1. That I am the owner of the D Last Bites Food Hub business inside the
campus of St. Jude College located at Don Quijote St., Sampaloc,
Metro Manila;

2. That after careful consideration, I no longer wish to continue the


operation of the said business;

3. That I am transferring the rights/ownership over the said business to Mr.


Cleb Jaudian from this day onwards;

4. That for purposes of the renewal of the Last Bite Food Hub business
permit, and its transfer to Go Manila access, I am requesting that the
transfer be made unto the name of Mr. Cleb Jaudian;

5. That I am executing this Affidavit to attest to the truth of the foregoing


facts, and for whatever legal purposes this may serve best.
