I, Fidel T. Abel, of legal age, Filipino and a resident of Centro Norte,


Alcala, Cagayan, after having been sworn to and in accordance with law, hereby
depose state and that:

1. I was issued a High School Diploma last 27 th March 2000 from Saint
Philomene Academy, Alcala, Cagayan;

2. On 3 January 2023, while I was preparing the documents needed for the job
I was applying to, I discovered that the said High School Diploma cannot be
located;

3. I exerted earnest efforts to locate the said Diploma but the same could no
longer be found and should now be considered lost and beyond recovery for
all intents and purposes;

4. I am executing this Affidavit to attest to the veracity of my statements and


to support the same in completing my requirements for reacquiring my new
High School Diploma from Saint Philomene Academy, Alcala, Cagayan.
