MEASURING BODY TEMPERATURE

PURPOSE:
• Maintaining a stable body temperature within normal ranges assists in optimizing metabolic
processes and bodily functions. Therefore, minimizing environmental factors within the
hospital setting which may result in unnecessary body temperature fluctuations is further
important.
Objectives
• recognize that a thermometer is used to measure temperature,
• recognize that temperature can be measured in degrees Celsius,
• recognize that higher temperatures are warmer and lower temperatures are cooler,
• estimate temperatures,
• solve simple problems regarding temperature in degrees Celsius.
EQUIPMENT:
• thermometer
• thermometer sheath or cover
• water- soluble lubricant for a rectal temperature
• disposable gloves
• towel for axillary temperature
• tissues / wipes

1. Check the physician’s order or nursing care plan for


frequency and route.
2. Introduce yourself. Identify patient using two identifiers
(name and birth date or name and check the wristband
according to agency policy.
3. Explain the procedure to the patient.

4. Gather equipment. Make sure the electronic or digital


thermometer is in operating condition.
5. Perform hand hygiene.

6. Ensure privacy by closing door or curtain.

7. Place the client in the appropriate position. (Sitting or


supine position for oral and axillary, Sim's or lateral
position for inserting a rectal thermometer).
8. Remove the digital thermometer from its container.

9. Wipe the thermometer using cotton moistened with


alcohol from the tip to base and discard cotton ball in
appropriate receptacle.

10. Turn on the thermometer.

11. Place the thermometer. A . Oral - Place the bulb on either


side of the frenulum. B. Rectal - apply clean gloves.
Instruct the client to take a slow deep breath during

insertion. Never force the thermometer if resistance is


felt, insert 3.5 cm (1½ in adults). For pediatric client, 1.5-
2.5 cm (1in). C. Axillary- pat the axilla dry with tissue
paper if very moist. The bulb is placed in the center of the
axilla. D. Tympanic - For clients ages 4 years and above,
pull the pinna slightly upward and backward. For 3 years
and below clients, pull the pinna slightly downward
and backward. Point the probe slightly anterior, toward
the eardrum. Insert the probe slowly using a circular
motion until snug. E. Temporal artery - brush hair aside if
covering the temporal area. With the probe flush on the
center of the forehead, depress the red button. Keep
depressed. Slowly slide the probe midline across the
forehead to the hair line, not down the side of the face.
Lift the probe from the forehead and touch on the neck
just behind the earlobe. Release the button.

12. Apply a protective sheath or probe cover if appropriate.


Note: Always lubricate a rectal thermometer unless
contraindicated.

13. Wait for the appropriate amount of time. Electronic and


tympanic thermometers will indicate that reading is
complete through a light or tone.

14. Remove the thermometer. Read the temperature and


record it on your worksheet.

15. Disinfect the thermometer with a cotton pad with


alcohol. Return thermometer to appropriate container.

16. If gloves are worn, remove and dispose in appropriate


receptacle. Perform hand hygiene.

RECORDING AND REPORTING

• Record temperature in nurses’ notes or vital sign flow


sheet or EHR. Document measurement of temperature
after administration of specific therapies in narrative
form in nurses’ notes.
• Report abnormal findings to nurse in charge or health
care provider.
