Typhoid is an infectious bacteria disease transmitted through contaminated food and water. Typhoid
infection first establishes itself in the digestive system from where it spreads further to the rest of the body.
After infecting the intestines, typhoid bacteria destroy the intestinal lining leading to ulceration and
bleeding. It is through the bleeding body vessels that a blood infection is established and then further
spread through the body.
Typhoid is a serious infection and should be attended to promptly whenever it develops. It is a systematic
infection: thus, it can affect any part of the body. However, as mentioned earlier, at onset of infection, the
organ affected is the intestine. This leads to intestinal symptoms being the first to appear. The symptoms
may include passing loose stool, abdominal discomfort and loss of appetite, nausea and passage of bloody
stool. As the disease progresses, blood stream involvement leads to the onset of general malaise, fever,
joint pains, muscular pains, headache and mental clouding. In severe infections, coma and death may
result.
When a patient with typhoid reports to a doctor, the doctor suspecting the disease can carry out some tests
to confirm whether the patient has the infection or not. Among the tests recommended, blood tests are the
most important. However, urine and stool tests can be very useful for the doctor to distinguish it from
other diseases which presents themselves in a similar manner such as parasitic amoeba or the bleeding
vascular disorder of the digestive system.

Once in the blood, typhoid bacteria are filtered and excreted in the urine by the kidneys. Therefore, urine
tests can reveal a typhoid infection. Thus, a qualified doctor or medical personnel should only determine
a diagnosis of typhoid by relevant evidence of the presence of symptoms and signs. The doctor can also
use either direct or indirect evidence of the presence of the responsible bacteria to diagnose the disease.
If you develop any of the symptoms such as nausea, abdominal discomfort, general malaise, joint pains,
fever, dysentery, mental clouding and abdominal swelling, you should seek medical advice immediately.
The doctor will then decide the kind of treatment you require. The treatment given depends on the severity
of the disease, and can involve administration of intravenous medicines or oral medication according to
the doctor’s assessment. The doctor could also have your body fluid and blood replaced die to diarrhea
and dysentery.
Transmission of typhoid can be controlled in various ways. Proper cooking of food and boiling of drinking
water can reduce transmission of typhoid. Washing hands before eating and after visiting the toilet is also
important. Proper disposal of body waste is another good preventive measure.
Early diagnosis and treatment of those who get infected also reduces transmission to others. In addition,
anti-typhoid vaccine provides protection against typhoid for at least three years after which one is due for
another vaccination.

Once it used to be said that ‘the sun never sets on the British Empire.’ This meant that the British
Empire covered all the time zones so that when the sun was setting on one part of the Empire, it
was rising on another.

The same thing can be said about the English language today. English is spoken as the first language
in the United Kingdom, Ireland, the United States of America, Canada, New Zealand and Australia. It
is also spoken in Caribbean countries like Jamaica, Trinidad and Tobago, and Guyana among others.
In addition to this, English is used as a second language in over fifty countries spread across Africa
and Asia. This means that when you have a good command of English you can enjoy the writings of
people from all over the world, and you can communicate your ideas beyond the borders of this
continent.
We in East Africa embraced the English language accidentally through the colonial experience.
English came in as a welcome addition to our already rich language situation of over forty local
languages and Swahili. In fact, the majority of Kenyans today are multilingual. They use at least
three languages; a local language, Swahili and English. A good number of other speak a fourth
language which may be either French, German, Japanese or Arabic among others.
The local or ethnic language bonds a person to the history, culture and religious traditions of his or
her people. If you speak one of the ethnic languages, for instance, you can enjoy the songs, dances,
stories, proverbs and riddles in that language. It also helps you to communicate with the older folks
who may not understand English or Swahili.
Swahili is the national language of Kenya and it is used by people from all ethnic groups. When one
speaks and understands Swahili, one can communicate with people from all over Kenya. Swahili is
a ‘regional language’ because it is spoken in several countries in East and Central Africa. Lately,
Swahili was indeed adopted as one of the working languages at African Union meetings. Swahili,
therefore, opens opportunities beyond the borders of Kenya.
After Kenya attained independence, English was retained as the official language because it was and
still is, a great advantage for many of our operations. Though the original speakers of English were
the people living in the British Isles, English is now spoken as a first language in two other
continents, North America and Australia. It is also the second language for millions of people living
all over the world. Therefore, with English, you can be at home anywhere in the world. English
provides us with a powerful link to the global village.

Recent developments in modern technology have enhanced our ability to speak, read, write, and
understand English well. Many of the computer programmes available, for example, are made to
respond to instructions given in English.
English is also important for participation at the international level. Many reports on recent
developments, science and other special research are in English. There is much more written
literature in English than there is in any other language. Therefore, a student who can read and
write in English well is at an advantage.
For those who want to be part of what is happening in the world, English is a very important tool.
What does this mean for those of us in Africa and Asia, where English is learned mainly in school?
There is a need to put a lot of effort into the teaching and learning of this language so that we can be
part of the vast changes taking place in the world today.

One day Hare, popularly known as Ogila Nyakarondo, met Elephant on his way to the forest. As Hare
is a cunning and clever animal, he wanted to fool Elephant on some issues. He started talking to
Elephant; ‘Elephant my brother, Good Morning. What is the matter? Why are you walking alone like
that my friend? Why don’t you ask me to walk you through the forest?
After they had walked together for some time Hare said, ‘Elephant my brother, I want to go on a
rope-pulling contest with you. I know that I can defeat you miserably, for I am stronger than you.’
The elephant replied, iii! Ogila Nyakarondo, you are too small to contest with me. You are playing
about, my brother. Think twice.
They then planned to go on a contest the next day in the forest, but Elephant went laughing, thinking
that Ogila could do nothing to him. The same day, Ogila met with Hippo and started talking to him
saying, ‘Hippo my brother, I want you to contest with me one of these days. We shall pull a rope
across the forest and see who will be the winner. ‘Hippo said, ‘Don’t play with me, Hare my brother,
you are just too small to contest with me.’ He made the same plan, just as he had planned with the
Elephant to pull the rope in the contest across the forest.
The next morning, Hippo and Elephant went to each side of the forest thinking they were going to
contest with Hare. They started pulling the rope. Meanwhile, Hare was sitting on a high hill watching
what was happening below. He sang a song to taunt Elephant and Hippo. As he sang, Hippo and
Elephant pulled even harder. They pulled and got very tired. At dusk, they almost passed out and
stopped pulling.
Hare immediately ran to the elephant and asked, ‘am I not great?’ Elephant told Hare, ‘Oh, Hare my
brother, I have agreed that you are great. I have had it tough the whole day.’
Hare then ran to Hippo and told him the same thing he had told Elephant. Hippo wondered at the
strength of the Hare and said, ‘Hare my brother, I have realized that you are very strong. In fact, you
should be made the King of the jungle.’ Hare went away merrily playing his guitar.

Mrs. Maneno’s twin sons, Kiprotich and Jowi had passed well in their KCPE examinations. She decided
to share her experiences as a Form One student with them. She hoped this would help them know how to
conduct themselves in the new schools.
“When I joined Marsabit Secondary School,” Mrs.Maneno began, “I was very excited and settled down
quickly. The only thing that I did not like was taking a cold bath early in the morning. Many of us were
not used to this lifestyle and we dreaded it. Some girls would go to the shower rooms, undress, and pretend
to be bathing. However, what they actually did was to scoop water with their hands and throw it over their
shoulder to the wall behind them. They would then wet their towels a bit for them to appear like they had
been used. Such girls thought they were fooling the whole school but in no time the secret was unveiled.
During the second week, our teacher complained about the odor in our classroom. They informed the
matron. When the matron asked us whether we had been bathing, everybody said yes. Of course, she was
not convinced and she promised to investigate further.
One afternoon after classes, we were all assembled in our dormitory. The matron threatened to have all of
us undressed and washed by the other girls while she watched. This was not a welcome idea. After a brief
tense moment, she ordered those who had not been bathing to step forward. At first, no one moved, then
one girl stepped forward timidly and several others followed. They were ordered to go and scrub

themselves clean and then report to the matron. Later in the afternoon, the matron talked to us about
cleanliness.
I remember her emphasizing the need to take a bath at least once a day, twice if possible. She said that our
bodies perspired a lot especially when it was hot, and there was a need which, if not washed off, resulted
in body odour. She also reminded us about brushing our teeth every day to avoid dental problems and bad
breath. She said toothpaste gives one fresh breath and kills germs. At this point, one girl raised her hand
and asked, “Is it wrong to share toothbrushes with friends or family members?” Everybody turned to stare
at her in disbelief. The matron told her it was wrong and also unhygienic. She also added that clean teeth
gave one the confidence to talk or smile without fearing that there would be food particles on the teeth. In
addition to that, she also mentioned that some students had a bad habit of spitting all over the compound.
“Mum, do boys need to bathe daily like girls?” Kiprotich interrupted. “Yes, of course, they do,” Mrs.
Maneno replied. All the things I’m telling you apply to both boys and girls, and even adults.” She then
went on. “The matron informed us that the sharing of personal items like towels, basins, and other clothes
was unhygienic and could lead to skin infections. She reminded us to hang our towels and underwear out
in the sun to air them and kill germs. After our meeting in the dormitory, we went about our school routine,
and were no more incidents of that nature. The good thing about that incident was that from that day
onwards, Maritha, one of the girls who had not been bathing, became the cleanest girl in the school. At
the end of the year, she was awarded the coveted award for the cleanest and neatest girl in the school.”
At this point, Jowi interrupted and asked,” Mum, you mean you didn’t get the award?” Mrs. Maneno
smiled and said, “I had my problems combing my hair and I hardly washed it. On several occasions, I had
been sent out of class to comb my hair and polish my shoe.” The twins looked at her, shock written all
over their faces. They had always thought their mother had been the cleanest student because of the many
lectures she gave them about cleanliness.
“Many girls,” she continued,” got into trouble over their long nails.”
“Well, there is nothing wrong with long nails,” Mrs. Maneno said.

“The only problem is that many people don’t clean the surface under the nails. The nails, therefore,
become dirty and harbor germs. The best thing is to cut them short. Short nails are easy to manage, just

Once upon a time, there lived an old man. One day he paid a visit to his in-laws. On entering
the house of his mother-in-law, he found that she had been roasting some meat, among which was a
delicious-looking sausage and she was not in the house. He immediately took the sausage and quickly put
it into his quiver. And it so happened that a piece of live coal had got stuck on the sausage but the old man
didn’t know. He quickly shut the quiver.
No sooner had he sat down than the owner of the house came in. They sat down to talk about the children’s
health. When they had finished, it was time for the old man to return to his home. Just then, the woman
noticed smoke issuing from the quiver and asked the old man; “Paker, how come the quiver is smoking?”
The man answered, “Oh. It’s some naughty fire stick with a soft head that smokes whenever it comes into
contact with soft wood and the arrows.” The woman kept quiet and got up to escort her guest.
When they had walked only a short distance, the fire made a hole in the quiver and the arrows fell out,
tiak! Together with the stolen sausage. The lady, who was walking closely behind, exclaimed: ‘See, had
I not known it! Then the old man fears that his sausage might be eaten.

Almost half of all sick children in public hospitals do not have their vital signs recorded at admission,
as required, a study has said. The largest ever study on nursing care of children, which was conducted
over two years, also show one in 10 children die mainly due to lack of basic care.
The survey conducted in 13 referral county facilities and covering 54 000 admission was carried out
by the clinical information network (CIN) a collaboration of the Ministry of Health, Kenya Medical
Research Institute, University of Nairobi and Kenya Pediatric Association.
The survey found out that the vital signs in 43 per cent of the children had not been taken at admission.
Body temperature, pulse and respiratory rates ae supposed to be recorded and monitored throughout
the hospital stay. However, this was only consistently done in 13 hospitals. In some facilities, less than
two per cent of children had their vital signs recorded. More than eight per cent of children did not
have any of their vital signs recorded while 0.2 per cent had their pulse rate taken.
Once admitted, the researchers said it was a requirement that patients’ vital signs be monitored and
recorded at least three times in every 24 hours. Again, this was only consistently done in three of the
13 hospitals.

Even when monitoring was done, the quality of recording was poor; with evidence showing most of
the nurses had recorded estimates. For example, most of the recorded data were round-up figures or
most ended in even numbers. This, the authors said, was an indication that the figures were estimates.
“Our data raise concern that in practice, inaccurate respiratory rate measures are likely to result in
misclassification of pneumonia, poor targeting of treatments and inability to detect deterioration or
improvement,” says the study. The CIN group said it was disturbing that pneumonia and respiratory
illness remained major problems in almost all hospitals despite availability of protective vaccines.
“The high proportion of children admitted with pneumonia is worthy of note despite more than 10
years use of flu (HiB) vaccine and use of PCV10 Pneumonia vaccine.”
Two recent studies at Gertrude’s Children Hospital and Kenyatta National Hospital have questioned
the effectiveness of pneumonia and rotavirus vaccines. The researchers further investigated the cause
of children’s neglect in top-tier public hospitals, which they identified as acute shortage of nursing
staff.
“In these hospitals each qualified nurse is typically responsible for between 10 and 40 beds. There can
be two or more children to a bed,” says the report.
In five of the 13 hospitals, typically one qualified nurse was present on a shift whereas in the other
eight, there were only two nurses. Data from the Health Ministry indicate there are 20,981 nurses in
public hospitals against a requirement of about 52,000.

Discipline is something that we have all experienced personally in different forms, seen used on others,
and is also something that many of us will go on to use later in life, both in the form of self-discipline
and as something to keep children and even employees in check. It is essential to life as we know it,
and we need it in its many different forms in many different situations.
The first reason that discipline is so important is that we all need to exercise self-discipline to be
successful in life. Self-discipline can mean very different things to different people; for students, for
example, self-discipline is often about motivating yourself and making yourself concentrate on your
studies and get your assignments in on time. For working people, it can be as simple as getting up on
time every morning, however tired you may be and how much you may hate your job, getting to work
on time and doing your job. Without this kind of self-discipline, people would not be able to enjoy
academic success, or be successful in their careers either.
Self-discipline is also required for dieters and anyone doing regular exercise, because given the chance,
most of us would prefer to be lazy rather than get up and exercise, and eat burgers and fries rather
than healthy food. Without it, even more people would be fat and unhealthy, and a lack of self-
discipline in some people certainly contributes to the obesity crisis.
Discipline is also something that needs to be used on others where necessary. If parents didn’t
discipline their children when they were naughty, children wouldn’t grow up knowing right from
wrong, or be able to become productive members of society who contribute to the system. Equally,
schoolteachers need to be able to dish out punishments to children who don’t behave themselves.
Without discipline in the classroom, there would be a great deal of disruption and nobody would ever
learn anything. Indeed, teachers who struggle to command the respect of students and who fail to use
discipline effectively will often have trouble even making themselves heard in a classroom.
In the workplace, discipline is also essential to maintaining a hierarchy and dealing with employees
who do not follow company policies and procedures, regularly arrive late or not at all, or treat their co-
workers unfairly. Then, you have to consider that without discipline, there would be no law
enforcement. Murderers would be roaming the streets and everybody would be stealing from each
other, because there would be no consequences for their actions.
