Skill 35-3: Preparing Medications from Vials

1. Performed hand hygiene and observed other appropriate infection
prevention procedures.
2. Prepared medication vial for drug withdrawal:
a. Mixed solution, if necessary, by rotating vial between palms of
hands, not by shaking.
b. Removed protective cap, or cleaned rubber cap of previously
opened vial with an antiseptic wipe by rubbing in circular
motion.
3. Withdrew the medication:
a. Attached a filter needle, if appropriate.
b. Ensured that needle is firmly attached to syringe.
c. Removed the cap from the needle. Drew up into the syringe the
amount of air equal to the volume of the medication to be
withdrawn.
d. Carefully inserted the needle into the upright vial through the
center of the rubber cap.
e. Injected air into the vial and kept the bevel of the needle above
the surface of the medication.
f. Held syringe and vial at eye level to determine that correct
dosage of drug was being drawn into syringe. Ejected air
remaining at top of syringe into vial.
g. Withdrew the prescribed amount of medication.
h. Withdrew the needle from the vial and replaced the cap over the
need using the scoop method.
i. When correct volume of medication plus a little more was
obtained, withdrew needle from vial, and replaced cap over needle
using scoop method.
j. Gently tapped the syringe barrel with a finger or pen while
holding the syringe with the needle end pointing up to dislodge air
bubbles present in the syringe. Expelled air and any ecess medication
from the syringe, maintaining the “needle up” position.
k. Replaced filter needle with a regular or safety needle if giving an
injection.

a. Read manufacturer’s directions.

diluent, unless otherwise indicated by directions.
c. Added amount of sterile water or saline indicated in directions.
d. If multidose vial was reconstituted, labeled vial with the date and
time it was prepared.
e. Once medication was reconstituted, stored it in refrigerator or as
recommended by manufacturer.

Used effective body mechanics throughout procedure.
