We hereby declare that we have gone through the modalities ad terms & conditions for establishing
Farm Machinery Bank provided by the department prior to submission of this application and all the
information and statements furnished in/with this application are true and accept that any misinterpretation
contained in it may lead to our disqualification and also declare that we didn’t get subsidy assistance under
SMAM in the year 2020-21. We understand that decision of according approval or rejection of our
application shall be binding on us.

N.B:

1. Rs. 400 (four hundred) will be charged (non-refundable) per form. Application form may be downloaded
from the Department website www.agrimanipur.gov.in
2. Farm machineries and implements proposed for the Farm Machinery Bank may be chosen from the
list enclosed at Annexure-A.

3. Society/Group should purchase machinery/equipment only from the listed Dealers/Farms.


4. All relevant documents should be enclosed.
5. Application without Detail Project Report (DPR) will not be considered.

6. Due to certain changes in the Guide Line of SMAM, DPR should be prepared at the Directorate Office,
Sanjenthog only.

7. In the column of “Details of members of the Organization” minimum of 10 (Ten) members details
need to be fill-up.

8. Society/Group cannot get the benefit of subsidy assistance under SMAM for consecutive years.
