For the reasons sworn to in the accompanying affidavit, plaintiff humbly


prays that this Hon'ble Court may be pleased to permit the plaintiff to
produce the notarized documents mentioned herein below, by allowing this
application in the interest of justice and equity.

AFFIDAVIT

I, S. GUNASHEKAR S/o Srinivas, aged about 46 years R/at # 894, Yamuna


Nadi Road, Srinagar, Banashankari, Bengaluru south, Bangalore-560050, do
hereby solemnly affirm and state on oath as follows: -

1. I am the plaintiff in the above case. I know the facts of the case and am
swearing to this affidavit.

2. I state that I have filed this suit for declaration and a mandatory injunction
to correct my name and my minor son’s name on the birth certificate.

3. That the above-mentioned case is pending adjudication before this


Hon’ble Court and is fixed for today i.e. 07.01.2025.

4. That the plaintiff would like to place SSLC marks card of the plaintiff No. 2
Sai Kishan G, which were not annexed with the plaint filed by the plaintiff
in the suit for declaration and a mandatory injunction.

5. That the plaintiff is seeking permission of this Hon’ble Court to place the
above said documents on record which were inadvertently and
unintentionally has not been annexed with the plaint.

6. That the present application is filed bonafide and will cause irreparable
loss and grave injury to the plaintiff if the same is not allowed by this
Hon’ble Court.

7. That no prejudice will be caused to the defendant if the present


application is allowed by this Hon’ble Court.

PRAYER
Under the facts and circumstances as explained herein above and in the
interest of justice it is humbly prayed that the Hon’ble Court may be pleased
to allow the present application to place on record the additional documents;
