AFFIDAVIT IN SUPPORT OF FRESH ADDRESS OF THE ACCUSED


I, Sweta Kumari, Aged about 26 years, Authorized Representative/ Attorney of
the Complainant Company having her office at:- 85-A 1st & 2nd Floor
Rishyamook Building Panchkuian Road, Near RK Ashram Marg Metro Station
New Delhi-110001, do hereby solemnly affirm and declare as under:

1. I state that I am the Authorized Representative of the Complainant


Company and am well conversant with the facts and am competent and
authorized to swear this Affidavit.
2. That the Accused has been evading the service of process in this case, and
despite my best efforts to locate the accused at the known addresses
previously available, I have been unable to effect service.
3. That, recently, the complainant has come to know that the Accused has
moved to a new address, which is as follows:
Anasuya Moharana
W/O Santosh Kumar Ojha,
Kamarasahi, Jharapada, Cuttack, Near Alekha Tungi,
P.O Govindapur (Ck), Cuttack Orissa 754003;

Office Address: Executive, Employee Code - ZW150068, Ziqitza Health


Care Ltd, 7th Floor, IDCO Tower, Unit-9, Shaheed Nagar, Bhubaneswar,
Odisha - 751007.
4. That the above-mentioned address is the last known address available to
Complainant Company, and the same is being provided before this
Hon’ble Court.

5. That I request this Hon’ble Court to issue the necessary directions for
service of summons and/or notices to the Accused at the above-
mentioned address.

6. I state that the contents of this Affidavit have been filed and drafted by
my counsel under my instructions and the same are correct and true and
nothing stated therein is false or concealed there from.
