Dear Madam:

I am writing your office in behalf of my daughter Princess Maicah


S. Mangudadatu, who is 15years old and presently enrolled as grade 9
student, this school.

My daughter wish to change her surname from “Mangudadatu” to


“Sezian”. The legal basis to change her surname is in accordance with
Article 364 of the Civil Code provides: “Legitimate and legitimated
children shall principally use the surname of the father. The provision
states that legitimate children shall "principally" use the surname of the
father, but "principally" does not mean "exclusively." This gives ample
room to incorporate into Article 364 the State policy of ensuring the
fundamental equality of women and men before the law, and no
discernible reason to ignore it. The Supreme Court has explicitly
recognized such interpretation in the case of Alanis III versus Court of
Appeals citing Alfon v. Republic 1. As a matter of fact, my children are
about to file the petition in court for the change of surname and I will be
glad to furnish your office of the filed petition.

Withal, I respectfully request that my daughter be enrolled in your


school using her preferred surname Sezian instead of Mangudadatu.
Anticipating for your favorable consideration on my request, the same
favored request extended to my 2 other children by Infant Jesus
Academy.

Attached herewith are the following:

1) Affidavit of my daughter Princess Maicah S. Mangudadatu.

2) Affidavits of her 2 siblings and their issued school ID's.
