FOREWORD
The CLP Power Safety Rules supersede the Power System Safety Rules (Issue No. 1
Revision No. 5, 1 December 2002). The policy and principles of Occupational Safety and
Health as well as Operational Safety are now incorporated with the adoption of a risk
based and total safety philosophy.
The CLP Power Safety Rules, which are mandatory, are issued in four parts:
- Volume 1: Policy and Principles
- Volume 2A: Occupational Safety and Health Rules (OSH Rules)
- Volume 2B: Electrical and Mechanical Rules (E&M Rules)
- Volume 3: Administration Procedure for the Supporting Documents of the CLP
Power Safety Rules.
The CLP Power Safety Rules draw reference to and shall be implemented with
supporting documents including safety instructions, procedures and other guidance
documents issued by CLP Power.
SCOPE
This document, Volume 2B: Electrical and Mechanical Rules, describes standards and
minimum requirements, including electrical and mechanical operational safety, of the
CLP Power Safety Rules covering CLP Power’s business operations. This document is
applicable to all CLP Power employees, contractors, and all other persons while on CLP
Power premises, or, when working on or near the power system, plant or apparatus,
under the control of CLP Power.
AUTHORITY
This document has been issued under the authority of the Director, Power Systems, being
the corporate functional owner of safety and health of CLP Power.
It has been approved by the Director, Generation; Director, Power Systems; and Director,
Marketing and Customer Services.
STATUS AND IMPLEMENTATION
In addition to the CLP Power Safety Rules, all applicable ordinances, regulations, rules
and standards issued by government and regulatory authorities, and as laid down in other
CLP Power documentation shall be complied with at all times.
Any difficulties in implementing this document should be brought to the attention of the
Director, Power Systems.
REVISION
This document shall be reviewed at a frequency of not less than every two years to
confirm that the arrangements are still appropriate.
If there are any significant changes in the arrangements, this document shall be revised
within a period of not more than three months.

1. General Provision

1.1 Application

1.1.1 CLP Power employees, contractors or others working or testing on


or near the CLP Power’s power system shall follow the CLP
Power Safety Rules, hereinafter also referred to as “these Safety
Rules”.

1.1.2 Where the CLP Power employees and contractors are required to
work on or near the power system, plant or apparatus not owned
or controlled by CLP Power, the following shall be determined:

1.1.2.1 CLP Power and the owner or controller of the other power system,
plant and apparatus shall agree on which rules, documents and
procedures to use. The agreement shall be communicated to
employees or contractors concerned.

1.1.2.2 Where there are no rules, documents and procedures applicable for
the owner or controller of the other power system, these Safety
Rules and the associated supporting documents shall be used with
the agreement of the owner or controller of the other power system.

1.1.3 Plant and apparatus shall be added to and removed from the
power system in accordance with approved procedures that will
also determine when these Safety Rules shall apply.

1.2 Issue of This Document

1.2.1 A controlled, current and up-to-date copy of this document shall be


issued to all operational persons appointed by CLP Power.

1.2.2 An updated electronic copy shall be available for reference.

1.2.3 Circulation of this document is restricted to CLP Power


employees, contractors and authorised recipients only.

1.2.4 Unauthorised recipients should not refer to this document for use on
any other systems.

1.3 Treatment of Electric Shock

1.3.1 All persons who may carry out the operation of, work on or testing
of, the power system and associated plant or apparatus, shall be
trained in the treatment of electric shock.

1.3.2 Information regarding such treatment is given in Appendix No. 3.

1.4 Use of Safety Equipment and Wearing of Personal Protective


Equipment

1.4.1 Where any work under these Safety Rules takes place, appropriate
safety equipment and personal protective equipment shall be used
in accordance with section 2.5, OSH Rules.

1.4.2 Insulating gloves of approved types shall be worn under the


following conditions:

1.4.2.1 For high voltage apparatus including:


a) Operation of isolators or isolating devices manually on overhead
lines;

b) Application of portable earthing devices, or

c) Use of voltage testing devices, operating rods, test plugs or test


connections, or measuring sticks.

1.4.2.2 For low voltage apparatus including:


a) Operation of live exposed switches, isolators or fuse gear;

b) Use of voltage testing devices in the vicinity of exposed live low


voltage conductors, or

c) Work on live low voltage conductors.

1.5 Proving Apparatus Dead

1.5.1 To prove apparatus dead, it shall be tested by means of an


approved voltage testing device.

1.5.2 The voltage testing device shall be tested immediately before use
and immediately after use.

1.6 Failure of Supply

During failure of supply, all power systems, apparatus and


conductors shall be regarded as live until they are isolated and
proved dead.

1.7 Interpretations

1.7.1 The term ‘work’, when used in these Safety Rules, may include
testing.

1.7.2 The term ‘testing’, when explicitly used in these Safety Rules in
conjunction with the term ‘work’, draws attention to the additional
requirements for the testing apart from the requirements for the
work.

1.8 Definitions

The following terms have been defined for the sole purposes of this
document. Derived terms are construed accordingly. Where used in
this document, these terms appear in bold.

1.8.1 Additional An earthing device applied to apparatus, in


earth addition to the circuit main earths, after the issue
of a safety document.

1.8.2 Apparatus All equipment in which electrical conductors are


used, or supported, or of which they form a part.

1.8.3 Approved Sanctioned for use, or appointed in writing, by the


Chief Operating Officer or his designated
representative to satisfy the requirements of these
Safety Rules.

1.8.4 Circuit main An earthing device applied to apparatus, before


earth the issue of, and at a position stated on, a safety
document.

1.8.5 CLP Power CLP Power Hong Kong Limited.

1.8.6 Conductor Those normally live conducting parts of


apparatus, including neutral connections of such
apparatus.

1.8.7 Confined Any place in which, by virtue of its enclosed


space nature, there arises a reasonably foreseeable
specified risk.

1.8.8 Danger A risk to health or exposure to bodily injury.

1.8.9 Dead At or about zero voltage and disconnected from


any live power system.

1.8.10 Earth and A dual-purpose device applied to apparatus for


test truck earthing and testing.

1.8.11 Earthed Connected to the conductive mass of the earth,


whose electrical potential at any point is
conventionally taken as zero, by means of an
earthing device.

1.8.12 Earthing An approved means of providing a connection


device between a conductor and earth.

1.8.13 High voltage


live work

1.8.13.1 11kV live line Technique for performing 11kV live line work
glove and where the authorised person (HVLLW) is fully
barrier insulated from the earth and other phases, using
technique approved insulating gloves and sleeves, insulated
platform or insulated elevating work platform and
insulated barriers.

1.8.13.2 11kV live line The minimum distance that shall be maintained
minimum between the unprotected part of the authorised
approach person (HVLLW) and any component at different
distance potential throughout the 11kV live line work.

1.8.13.3 11kV live line Technique for performing 11kV live line work
stick using approved tools and equipment mounted on
technique insulating sticks with the authorised person
(HVLLW) maintaining the 11kV live line
minimum approach distance from energised
components or any components capable of being
energised.

1.8.13.4 11kV live line The minimum tool insulation distance of exposed
tool effective insulating material, which is subject to the voltage
length being contacted.

1.8.13.5 11kV live line A member of the 11kV Live Line Work Team
work safety who assumes the role of safety observer for the
observer 11kV Live Line Work Team during 11kV live
line work.

1.8.13.6 11kV live line A work crew comprising of authorised persons


work team (HVLLW), competent in performing 11kV live
line work in accordance with the approved 11kV
live line work instructions for each task.
The “11kV live line work team” refers to the
work crew of authorised persons (HVLLW)
performing or involved in performing a particular
task requiring 11kV live line work techniques.

1.8.13.7 Helicopter A technique involving a helicopter and a


live line crewmember hovering near energised 132kV and
insulator 400kV overhead lines or structures, for the
washing purpose of washing insulators in accordance with
these Safety Rules, relevant civil aviation
regulations and approved guidelines on the use of
helicopters for live line work.

1.8.13.8 Helicopter A work crew comprising of at least two


live line authorised persons (HCLLW) - a helicopter
insulator pilot and an airborne linesman, competent in
washing team performing helicopter live line insulator washing
in accordance with these Safety Rules, relevant
civil aviation regulations and approved guidelines
on the used of helicopters for live line work.

1.8.13.9 Helicopter A technique involving a helicopter performing


live line work electrical work on or near components or
conductors of high voltage overhead lines with
exposed parts that are energised or capable of
being energised without implementing the full
practice of isolation, proving dead and earthing.
This work shall only be performed by suitably
trained personnel authorised by CLP Power.

1.8.13.10 High voltage Work on or near components or conductors of a


live line work high voltage overhead line with exposed parts that
are energised or capable of being energised
without implementing the full practice of
isolation, proving dead and earthing. This work
shall only be performed by suitably trained
personnel authorised by CLP Power.

1.8.13.11 Minimum An approved insulating tool that is used to


approach measure the 11kV live line minimum approach
distance distance.

1.8.13.12 Minimum The minimum air distance that shall be maintained

approach disc) and all structures, earth wires, vegetation, or

1.8.15 Key safe A device of an approved type for the secure


retention of safety lock keys.

1.8.16 Key safe key A key unique at the location capable of locking
and unlocking the lock on a key safe.

1.8.17 Live Electrically charged.

1.8.18 Long Circuit A circuit that the remote end(s) of it crosses the
border, or a circuit connects to outlying islands
where access from the local end to its remote
end(s) cannot be obtained by land transportation.

1.8.19 Not Unable to be made live from the power system by

1.8.20 Notice

1.8.20.1 Danger live A notice in an approved form, illustrated in


notice Appendix No.2.1, reading “DANGER LIVE”
calling attention to the danger when approaching
or interfering with apparatus or conductors that
are live.

1.8.20.2 Isolation A notice in an approved form, illustrated in


notice Appendix No.2.2, reading “ISOLATION POINT.
CAUTION. DO NOT INTERFERE” conveying a
warning against interference at all times during
the course of work or testing. It is attached at each
point of isolation. The notice shall not be used for
any other purpose.

1.8.20.3 Work in A notice in an approved form, illustrated in


progress Appendix No.2.3, reading “WORK IN
notice PROGRESS. ATTENTION. Permitted to operate
by the responsible personnel only” drawing

1.8.21.2 Authorised A competent person, adequately trained and


person possessing technical knowledge and who has been
duly appointed by CLP Power to carry out duties
specified in writing within the terms of his
authorisation.

1.8.21.3 Authorised A person who is competent and has been duly


person appointed by CLP Power to carry out helicopter
(HCLLW) live line work on overhead line systems with a
voltage level at 132 kV and above and is
authorised to carry out declaration, acceptance,
clearance and cancellation of High Voltage Live
Line Work Certificates.

1.8.21.4 Authorised A competent person who has undergone


person extensive training to achieve the required
(HVLLW) competence, and been duly appointed by CLP
Power to carry out high voltage live line work
and is authorised to carry out declaration,
acceptance, clearance and cancellation of High
Voltage Live Line Work Certificates.

1.8.21.5 Senior An authorised person who has been duly


authorised appointed by CLP Power to carry out duties
person specified in writing within the terms of his
authorisation, and includes the issue and
cancellation of specific E&M safety documents
and E&M status documents.

1.8.21.6 Control The appropriate person nominated by CLP Power


engineer to be responsible for controlling the power system
or a specific part of the power system.

1.8.22 Operational Premises under the control of CLP Power


premises containing plant or apparatus, which includes but
are not limited to control rooms, power generation
stations, electrical substations and switching
stations, plant rooms and underground chambers.

1.8.23 Plant Fixed and moveable items other than apparatus.

1.8.24 Power system Items of plant or apparatus that are used


separately or in combination (electro-mechanical)
in any process associated with the generation,
transmission or distribution of electricity.

1.8.25 Purged The condition of plant or apparatus from which


any dangerous contents have been removed.

1.8.26 Safety The minimum distance away from the nearest


distance exposed live high voltage conductor not earthed,
or from the nearest insulator supporting a high
voltage conductor, which shall be maintained to
avoid danger.

1.8.27 Safety An approved document issued by an approved


document person in accordance with these Safety Rules
before work commences.

1.8.27.1 Electrical and mechanical status document (E&M status


document)
a) Circuit State Certificate (CSC)
A safety document of the form illustrated in Appendix No. 1.9,
issued with the joint agreement of a senior authorised person
and an authorised representative of a customer. This safety
document is issued before the customer issues his safety
document (which is equivalent to a Permit to Work) for work
on the apparatus which may be made live by connection to
both the CLP Power’s power system and the customer’s high
voltage system. It defines the apparatus and the actions taken
to achieve safety from CLP Power’s power system to enable
work to proceed.

b) Customer Switchgear Isolation Certificate (CSIC)


A safety document of the form illustrated in Appendix No.
1.10, issued with the joint agreement of a senior authorised
person and an authorised representative of a customer. This
safety document is issued before the issue of safety
documents by CLP Power for work on or testing of the
apparatus which may be made live by connection to both CLP
Power’s power system and the customer’s high voltage
system. It defines the apparatus and the actions taken to
achieve safety from the customer’s system for the purposes of
work or testing.

c) Inter Control Circuit Isolation Certificate (ICCIC)


A safety document of the form illustrated in Appendix No.
1.11 and No. 1.12, issued with the joint agreement of the
control engineers of the respective power systems. This safety
document specifies the inter-system circuit, which has been
made safe for work and confirms the actions taken to achieve
safety for work from respective power systems, allowing any
one party to issue a Permit(s) to Work on that circuit without
further reference to the other party.

d) Safety Rules Implementation Certificate (SRIC)


A safety document of the form illustrated in Appendix No.
1.13 within which is specified the plant or apparatus which
shall, from the time and date indicated, become subject to these
Safety Rules.

1.8.27.2 Electrical and mechanical safety document (E&M safety


document)
a) Limited Work Certificate (LWC)
A safety document of the form illustrated in Appendix No. 1.1,
issued by an authorised person to a competent person. This
safety document defines the limits within which access is
permitted and the nature of the work or testing, which may be
carried out. It is issued when verbal instructions are considered
insufficient for that purpose, and where no other E&M safety

b) Permit to Work (PTW)


A safety document of the form illustrated in Appendix No. 1.2,
issued by a senior authorised person to a competent person.
This safety document specifies the plant or apparatus on
which it is safe to work, the work, which is to be carried out,
and the actions taken to achieve safety.

c) Permit to Work with Restoration of Motive Power


(PTW(ROMP))
A safety document of the form illustrated in Appendix No. 1.3,
issued by a senior authorised person to a competent person.
This safety document differs from the Permit to Work in that
it allows the controlled restoration of certain motive power
supplies required for the testing or functioning of the plant or
apparatus.

d) Sanction for Test (SFT)


A safety document of the form illustrated in Appendix No. 1.4
and No. 1.5, issued by a senior authorised person to an
authorised person. This safety document specifies the high
voltage apparatus that has been made safe for testing. It states
the conditions under which the testing is to be carried out and
confirms the actions taken to achieve safety prior to the testing.

e) High Voltage Live Line Work Certificate (HVLLWC)


A safety document of the form illustrated in Appendix No. 1.6,
which shall have been declared and accepted by the high
voltage live line work team, and the requisite consent of a
control engineer before the commencement of the high
voltage live line work. The safety document shall specify the
work, which is to be carried out, the safety precautions taken
and the limits of the high voltage live line work area.

f) E&M Safety Document Supplementary Sheet


A safety document of the form illustrated in Appendix No. 1.7,
which is used as an attachment for the recording of additional
information should space be insufficient on a Permit to Work,
Permit to Work with Restoration of Motive Power or
Sanction for Test.

g) E&M Safety Document Transfer Record


A safety document of the form illustrated in Appendix No. 1.8,
which is used to record the transfer of a Permit to Work or
Limited Work Certificate.

h) Register of Working Party Members


A safety document of the form illustrated in Appendix No.
1.14, for recording the in and out status of working party
members performing the work or testing as specified on the
associated E&M safety document. It shall be affixed to the
associated E&M safety document and kept in the custody of
the recipient of the associated E&M safety document during
the course of work.

1.8.27.3 Occupational safety and health safety document (OSH safety


document)
Being a Confined Space Entry Certificate (CSEC) or a Hot
Work Permit (HWP) as defined in the OSH Rules.

1.8.28 Safety Lock A lock used for securing an isolating device,


earthing device, vent or drain. Such lock shall
only be operated by a key unique at the location.

1.8.29 Shall A mandatory requirement where neither discretion


nor judgment is permitted.

1.8.30 Shall, where A mandatory requirement, which is possible to


practicable achieve, taking into account current knowledge
and invention.

1.8.31 Shall, where A mandatory requirement where a judgement must


reasonably be made regarding what is reasonable to achieve,
practicable taking into account the magnitude of the risk as
compared with the cost, time, trouble and effort
necessary in order to avoid the risk.

1.8.32 Supervision

1.8.32.1 On-site Supervision by a person having adequate technical


supervision knowledge, experience and competence who is
continuously available at the work site to ensure
the safe performance of the work or testing.

1.8.32.2 Direct Supervision by a person having adequate technical


supervision knowledge, experience and competence who
controls and provides visual verification in the
presence of the person being supervised at every
step of operation, work or testing throughout the
entire process.

1.8.33 Switching The operation of circuit breakers, isolators,


disconnectors, fuses or other devices for the
making or breaking of an electrical circuit, and the
application and removal of earths.

1.8.34 Testing Operation of any apparatus within the isolated


operation zone under the terms of a Sanction for Test,
including:
a) operation of switching devices, except isolating
devices at the points of isolation;
b) removal and application of earths, and
c) connection and disconnection of test plugs or
test leads.

1.8.35 Vented Having an outlet open to the atmosphere so that


pressure has equalised to atmospheric pressure.

2. Safety Distance And Safety Precautions Governing Access

2.1 Safety Distance

2.1.1 A person shall maintain a safety distance, as shown in Table 1,


between any part of his body or any objects under his control and
the nearest exposed live high voltage conductor.

2.1.2 In addition to the safety distance from the exposed live high
voltage conductors, a distance of 300 millimetres, as shown in
Diagram 1, shall also be maintained from that portion of insulators
supporting live high voltage conductors which is outside the
appropriate safety distance from the conductors.

2.1.3 When the safety distance cannot be maintained, the person(s)


approaching exposed high voltage conductors or the supporting
insulators thereof shall either:

2.1.3.1 be the recipient or under the on-site supervision of the recipient of


an appropriate safety document which is issued after the
conductors are made safe for access in accordance with section
4.1.1.1 c) i), or

2.1.3.2 work with the conductors live in accordance with approved live
work procedures.

2.1.4 In assessing safety distances, due consideration shall be taken of


the possibility of conductors and insulators movement due to wind
conditions.

2.1.5 Objects to Approach Exposed High Voltage Conductors

2.1.5.1 When exposed high voltage conductors are not isolated, the only
objects that are allowed to approach the conductors, or the
insulators supporting the conductors, within the safety distances,
shall be approved insulated devices.

2.1.5.2 When exposed high voltage conductors are isolated, the only
objects that are allowed to approach the conductors, or the
insulators supporting the conductors, within the safety distances,
shall be approved insulated devices or earthing devices.

2.2 Access to Operational Premises

2.2.1 A person shall only enter or gain access to operational premises


with the permission of a responsible officer.

2.2.2 Access to operational premises shall be secured against


unauthorised entry.

2.2.3 Guards preventing access to fixed ladders shall be secured in


position.

2.2.4 Any person who enters or gains access to any operational


premises not containing exposed live high voltage conductors,
which may give rise to danger, shall be a competent person or be
under the on-site supervision of a competent person.

2.3 Access to Operational Premises Containing Exposed Live High


Voltage Conductors

Only an authorised person, or persons under the on-site


supervision of an authorised person, shall gain access to
operational premises containing exposed live high voltage
conductors where there is a risk of infringing safety distance.

2.4 Access to High Voltage Switchgear Spouts

All spout shutters not required for immediate work or operation


shall, if accessible:

2.4.1 be locked shut with safety locks and the keys shall only be
available to an authorised person, or

2.4.2 be secured with other approved locks in accordance with


approved procedures.

2.5 Access to and Work on Poles, Towers and High Structures

2.5.1 Reference shall be made to section 3.5.8, OSH Rules for safety
requirements for working above a height of two metres.

2.5.2 A person shall be accompanied by at least one person who may


remain at ground level when:

2.5.2.1 climbing above a height of two metres on a pole, tower or high


structure, of overhead lines, or

2.5.2.2 working above a height of two metres on any pole, tower or high
structure.

2.5.3 Work shall not be carried out on any pole, tower or high structure
when other work in the vicinity may affect the stability of the pole,
tower or high structure.

2.5.4 Before climbing a pole, a competent person shall check the


condition or stability of the pole in accordance with approved
procedures to ensure it is safe to climb.

2.5.5 If the condition and stability of the pole is doubtful, it shall be


reported to an authorised person for further action. Such pole shall
not be climbed until it has been securely guyed and if necessary,
splinted. It shall then be climbed by only one competent person
under the direct supervision of an authorised person.

2.5.6 Gates and devices to prevent climbing of towers, high structures


and gantries supporting high voltage conductors shall be secured
against unauthorised access by using lock or bolt and nut. Access
shall be controlled by an authorised person or competent person
in receipt of an appropriate safety document.

2.6 Access to Substations, Switch Rooms, Enclosures and


Compounds Containing Exposed Live Low Voltage Conductors

2.6.1 Barriers, doors or gates preventing access into enclosures or


compounds containing exposed live low voltage conductors shall,
where practicable, be secured against unauthorised entry.

2.6.2 Unless the exposed live low voltage conductors have been
screened or guarded to avoid danger, then access to the enclosures
of such conductors shall be restricted to competent person(s), or
person(s) acting under the on-site supervision of a competent
person.

2.7 Access to and Work in Underground Chambers and Confined


Spaces

For these requirements, reference shall be made to section 3.5.2,


OSH Rules.

2.8 Access to Vessels Containing Oil, Flammable or Toxic


Substances

For these requirements, reference shall be made to section 3.5.1 and


3.5.2, OSH Rules.

3. Requirements For Operations On Apparatus

3.1 Requirements for High Voltage Switching

3.1.1 General Requirements

3.1.1.1 Switching shall be carried out according to instructions given by a


control engineer, except for the following conditions:
a) emergency switching as stated in section 3.1.8;

b) under the terms of an E&M safety document on isolated


apparatus as stated in sections 3.1.10 and 9.4.4.4, or

c) agreed routine switching in accordance with approved


procedures.

3.1.1.2 Switching according to instructions given by a control engineer


shall be carried out by the authorised person who receives the
instructions or a competent person under the direct supervision of
the authorised person.

3.1.1.3 Switching devices used for the isolation of apparatus shall, where
practicable, be secured with safety locks.

3.1.1.4 Where fuses or links are removed for the isolation and safety locks
cannot be applied, the fuses or links shall be retained in a secure
location.

3.1.1.5 Isolation notices shall be attached to the points of isolation.

3.1.1.6 The sequence of switching instructions shall be such that all the
switching for isolating the apparatus from all points of supply is
confirmed as having been completed before a control engineer
authorises the earthing instructions.

3.1.1.7 Switching devices recorded on a safety document as points of


isolation shall not be operated when that safety document is in
force.

3.1.1.8 Switching instructions shall be logged clearly in approved format,


in accordance with approved procedures before and after the
switching is carried out, by both the control engineer giving and
the authorised person carrying out the instructions.

3.1.2 Preparation for High Voltage Switching

3.1.2.1 Before any switching instruction is given, a control engineer and


an authorised person shall reach an agreement on the location and
identification of the apparatus and the sequence of the proposed
switching.

3.1.2.2 For inter-system circuits, a control engineer or senior authorised


person shall co-ordinate all switching with relevant parties in
accordance with section 7.

3.1.2.3 Switching instructions shall be given directly by a control


engineer to the authorised person who is about to carry out the
operations, except for section 4.10.2.3. The authorised person
shall then repeat the instructions to the control engineer to confirm
that the correct switching instructions have been received and
understood.

3.1.2.4 When switching is to be carried out by a person under training, the


control engineer shall issue instructions to the authorised person
and to the person under training. The person under training shall
carry out switching under the direct supervision of the authorised
person.

3.1.3 Execution of High Voltage Switching

3.1.3.1 When carrying out the switching, the authorised person shall:
a) be meticulous, neither rushing nor causing undue delay, and
should take nothing for granted.

b) follow the switching instructions and ensure he is working on


the correct apparatus before taking any action.

c) log the time of operations in approved format in accordance


with approved procedures and check by any appropriate means
that the operations are correctly completed.

3.1.3.2 When operating a pole mounted switching device, the operator


shall use safety equipment and wear personal protective equipment
in accordance with section 1.4.

3.1.4 Confirmation of High Voltage Switching

3.1.4.1 The authorised person shall confirm the completion of switching


instructions, including the time of completion to a control engineer
without undue delay.

3.1.4.2 Where the switching is carried out by a person under training, in


addition to the confirmation of the authorised person, the person
under training shall also confirm the completion of instructions
directly to a control engineer.

3.1.5 Abnormal Situation

If a circuit breaker, or any other switching device, shows signs of


distress:

3.1.5.1 all operations on such apparatus shall cease immediately;

3.1.5.2 steps shall be taken to prevent such apparatus from being


operated, if it is safe to do so;

3.1.5.3 all persons shall keep clear of such apparatus, and

3.1.5.4 such apparatus shall be de-energised from remote ends as soon as


possible by switching devices with fault breaking capacity.

3.1.6 High Voltage Switching at System Control Centre

Switching operations at the control centre shall be carried out by a


control engineer in accordance with approved procedures.

3.1.7 High Voltage Switching at Generating Station

Switching, including agreed routine switching on specific plant or


apparatus, which is under the control of generating station
personnel, shall be carried out in accordance with approved
generating station procedures.

3.1.8 High Voltage Switching – Emergency Conditions

3.1.8.1 Emergency switching, in the circumstances where there is an


immediate danger to human life, may be carried out without
instruction from a control engineer.

3.1.8.2 Emergency switching is limited to de-energising operation only


using switching device(s) with fault breaking capacity.

3.1.8.3 The person who carries out the emergency switching shall be
competent to recognise the duty and the operation of the switching
device(s).

3.1.8.4 Such switching shall be reported to a control engineer by the


person who has carried it out as soon as possible.

3.1.9 High Voltage Switching – Functional Check

3.1.9.1 Functional checks involving the operation of circuit breakers,


disconnectors, isolators or earth switches that are not carried out
under the terms of a Permit to Work, shall only be carried out with
the consent of a control engineer, except for withdrawable circuit
breakers that have been withdrawn.

3.1.9.2 For functional checks on withdrawable circuit breakers that have


been withdrawn, reference shall be made to section 4.4.6.

3.1.9.3 The authorised person shall confirm the final status of the
apparatus to a control engineer after completion of functional
check on the apparatus.

3.1.10 High Voltage Switching Under the Terms of a Permit to Work


or a Permit to Work with Restoration of Motive Power

3.1.10.1 The recipient of a Permit to Work or Permit to Work with


Restoration of Motive Power or a person under the on-site
supervision of the recipient may carry out switching, without
reference to a control engineer, on the high voltage apparatus
stated on the E&M safety document.

3.1.10.2 The switching stated in section 3.1.10.1 shall not include the points
of isolation or circuit main earths of any E&M safety document
in force.

3.1.10.3 The final status of the apparatus shall be confirmed to a control


engineer by the senior authorised person who cancels the E&M
safety document.

3.1.11 Restoration of Withdrawable Apparatus

Before restoring withdrawable apparatus to service, an authorised


person shall check the apparatus, which is cleared of connections,
tools and foreign objects.

3.2 Requirements for Earthing of High Voltage Apparatus

3.2.1 General Requirements

3.2.1.1 Before earths are applied, the apparatus shall be isolated, and
where practicable, be proved dead in accordance with section 1.5
at the point where the earths are to be applied.

3.2.1.2 Earths shall be applied to or removed from the power system by


an authorised person under the instructions of a control engineer
in accordance with section 3.1, except for the conditions as
specified in sections 3.2.2.3, 3.2.2.4, 3.2.3.1, 3.2.4 and 9.4.4.4 a).

3.2.1.3 A control engineer in the system control centre shall log the
location and the time of application and removal of each earth
instructed by him in approved written format. This does not
include earths applied or removed as specified in sections 3.2.2.4
and 3.2.3.1.

3.2.1.4 The first earth connection on the apparatus shall, where


practicable, be made by a device with an adequate fault making
duty.

3.2.1.5 Where section 3.2.1.4 is not practicable, the apparatus shall,


where practicable, be earthed by an integral earthing device, or
portable earth leads applied by means of an approved earthing
pole.

3.2.1.6 When a circuit breaker is used for applying earths, trip features that
will operate the circuit breaker in the earth position shall, where
practicable, be rendered inoperative before closing.

3.2.1.7 Earths on overhead lines shall, be applied as near as practicable to


the points of work.

3.2.2 Circuit Main Earths

3.2.2.1 Circuit main earths shall, where practicable, be secured with


safety locks.

3.2.2.2 Circuit main earths that are recorded on a safety document shall
only be removed after that safety document is cancelled, except for
conditions as specified in sections 3.2.2.3, 3.2.2.4 and 9.4.4.2 e).

3.2.2.3 Circuit main earths stated on a Sanction for Test may be


removed and re-applied without reference to a control engineer.

3.2.2.4 For work on feeder spouts or voltage transformer spouts, circuit


main earths stated on a Permit to Work may be removed and re-
applied as specified in section 4.4.4.6 by the Permit to Work
recipient.

3.2.3 Additional Earths

3.2.3.1 The application or removal of additional earths issued with an


E&M safety document shall be carried out, without reference to a
control engineer, by the E&M safety document recipient or under
the direct supervision of the recipient while the E&M safety
document is in force.

3.2.3.2 If the circuit main earths through which the apparatus is earthed
are not close to and/or not visible from the point of work, the
apparatus shall, where practicable, be earthed by means of
additional earths at those points of work.

3.2.3.3 Additional earths applied to overhead lines shall remain in


position when work is in progress under the terms of a Permit to
Work.

3.2.4 Other Earths

Earths other than circuit main earths and additional earths, may
be applied or removed under the terms of a Permit to Work in
accordance with approved procedures and the following
conditions:

3.2.4.1 The application or removal of such earths is an item of work to be


done under the terms of a Permit to Work;

3.2.4.2 Such earths applied may be quoted as the circuit main earths in
the subsequent E&M safety document(s);

3.2.4.3 The application or removal of these earths may be carried out


without reference to a control engineer;

3.2.4.4 The application or removal of these earths shall be carried out by


the recipient of the Permit to Work or under the direct
supervision of the recipient of the Permit to Work, and

3.2.4.5 The status and location of such earths so applied or removed shall
be confirmed to a control engineer by the senior authorised
person who cancels the Permit to Work.

3.2.5 Requirements for the Use of Portable Earth Leads

3.2.5.1 When portable earth leads are used, they shall be applied to the
earth before being connected to conductors. When portable earth
leads are to be removed, they shall first be disconnected from all
phase conductors.

3.2.5.2 Where portable earth leads are to be used as circuit main earths,
care shall be taken to ensure that the point at which these circuit
main earths are to be applied, is of adequate capacity to safely
discharge any resultant fault current if the circuit is energised by
mistake.

3.2.5.3 The application of portable earth leads to, or removal from high
voltage conductors, shall be carried out by:
a) an authorised person accompanied by at least one competent
person when the portable earth leads are used as circuit main
earths. Such portable earth leads may be applied or removed by
the competent person(s) under the direct supervision of the
authorised person, or

b) at least two competent persons when the portable earth leads


are used as additional earths.

3.2.5.4 Approved devices for applying and removing portable earth leads
shall, where practicable, be used.

3.2.5.5 Conductors of all phases shall be earthed, even if work is to be


carried out on one phase only.

3.2.5.6 Portable earth leads shall only be applied or removed where safety
distance can be maintained as specified in section 2.1 from any
exposed live conductor.

3.2.5.7 Where bundled conductors are being earthed, each conductor


shall be earthed unless they are solidly bonded electrically at or
near the point of earthing.

3.3 Requirements for Low Voltage Switching

3.3.1 All switching with low voltage apparatus live shall be carried out
by a competent person or under his direct supervision.

3.3.2 When operating a switching device, where there are exposed live
low voltage conductors, the operator shall use safety equipment
and wear personal protective equipment in accordance with section
1.4.

3.3.2.1 Isolation notices shall be attached to the points of isolation.

3.3.2.2 Where fuses or links are removed for the isolation and safety locks
cannot be applied, the fuses or links shall be retained in a secure
location.

3.3.3 Abnormal Situation

If any low voltage apparatus shows signs of distress and is unsafe


to operate, all operations on such apparatus shall cease
immediately. All persons shall keep clear of such apparatus, and it
shall be de-energised from a remote end as soon as possible.

3.4 Requirements for Commissioning and Restoration of


Apparatus

3.4.1 General Requirements

Before the commissioning or restoration of apparatus, the


responsible operational person shall:

3.4.1.1 carry out appropriate checks and tests to ensure the apparatus is
safe for commissioning or restoration;

3.4.1.2 confirm that the apparatus is serviceable;

3.4.1.3 ensure all testing equipment, tools and materials that do not form
part of the apparatus are removed;

3.4.1.4 ensure all persons are clear of the apparatus and have been warned
not to work on the apparatus;

3.4.1.5 cancel all relevant E&M safety documents;

3.4.1.6 ensure all earths are removed;

3.4.1.7 verify the identification of the apparatus when it is ready for


commissioning or is restored after system rearrangement, and

3.4.1.8 where applicable, issue a Safety Rules Implementation


Certificate in accordance with section 9.5.4.

4. Work On Or Testing Of High Voltage Apparatus

4.1 Requirements for Work or Testing on or Near High Voltage


Apparatus

4.1.1 General Requirements

4.1.1.1 Work or testing on or near any high voltage apparatus shall only
be carried out under one of the following situations:
a) Work on or testing of external parts of any apparatus, where
there are no exposed live conductors and the nature of work or
testing does not give rise to danger from the power system,
shall be carried out under the following conditions:

i) Clear instructions limiting the work or testing, and work area


shall be given to a competent person by an operational
person, and
ii) When verbal instructions are considered insufficient, a
Limited Work Certificate shall be issued.
b) Work or testing near any exposed live high voltage conductor,
where the safety distance can be maintained, shall be carried
out under the following conditions:

i) Clear instructions limiting the work or testing, and work area


shall be given to a competent person by an authorised
person as stated in section 4.2.2.4 or to an authorised
person by a senior authorised person as stated in section
4.9.2.4 respectively, and
ii) When verbal instructions are considered insufficient, a
Limited Work Certificate shall be issued.

c) Work or testing on or near the conductors of any apparatus,


where the safety distance cannot be maintained or the nature of
work or testing may give rise to danger from the power system,
shall be carried out under the following conditions:

i) The work or testing shall be carried out under the terms of a


Permit to Work, Permit to Work with Restoration of
Motive Power or Sanction for Test in accordance with
sections 4.1.2 and 4.1.3, or
ii) Work or testing may be carried out with the conductors live
in accordance with sections 6.2 and 6.3.
d) Work on or testing of apparatus, which is not electrically
connected, may be carried out without an E&M safety
document but precautions shall be taken to avoid danger.

4.1.1.2 Work or testing shall only commence on apparatus, which is


readily identifiable or has fixed to it a means of identification,
which will remain effective throughout the course of work except in
section 4.1.1.1 d).

4.1.2 Requirements for Preparing the Issue of Permit to Work,


Permit to Work with Restoration of Motive Power and Sanction
for Test

These E&M safety documents shall only be issued after the


following conditions have been completed:

4.1.2.1 The apparatus has been taken out of service;

4.1.2.2 The apparatus has been isolated from all points of supply;

4.1.2.3 If applicable, common neutral earthing equipment, voltage


transformers and auxiliary transformers have been isolated;

4.1.2.4 Where practicable, all points of isolation have been secured with
safety locks;

4.1.2.5 For withdrawable switchgear with the associated circuit breaker not
in service position, shutters of spouts containing live conductors
have been secured with safety locks and danger live notices
attached;

4.1.2.6 Isolation notices have been attached to the points of isolation;

4.1.2.7 Circuit main earths have been applied in accordance with section
3.2.2 on the apparatus between the point of work or testing and
every point of isolation within the isolated zone, except for the
condition in sections 4.1.2.8 and 4.1.2.9;

4.1.2.8 For work or testing on an isolated section of a busbar or on the


busbar spouts of a multi-panel switchboard, at least one circuit
main earth has been applied to the busbar at any panel in the
isolated section;

4.1.2.9 For work or testing within the isolated transformer bank zone of a
generating unit, at least one circuit main earth has been applied on
each side of the generator transformer in accordance with approved
procedures, and

4.1.2.10 Additional precautions for the apparatus and associated plant


have, where applicable, been taken. These shall include draining,
venting, purging and containment or dissipation of stored energy.

4.1.3 Requirements for Commencement of Work or Testing under


the terms of a Permit to Work, Permit to Work with
Restoration of Motive Power or Sanction for Test

Work or testing shall only commence after the following are


completed:

4.1.3.1 The apparatus has been released for work or testing by the issue of
an appropriate E&M safety document in accordance with sections
4.1.2 and 9.4;

4.1.3.2 The safe working area and the apparatus to be worked or tested on
have been clearly identified to the recipient of the E&M safety
document at the work site, and

4.1.3.3 Where applicable, additional earths have been applied on the


apparatus in accordance with section 3.2.3.

4.1.4 Additional Requirements for Testing of Apparatus

4.1.4.1 Precautions shall be taken to ensure persons are prevented from


contacting or approaching the apparatus during the test, which
may give rise to danger.

4.1.4.2 The apparatus shall be fully discharged and left in a safe condition
after the apparatus has been electrically charged during the test.

4.1.5 Additional Requirements for Restoration of Motive Power

4.1.5.1 Additional safety precautions shall be taken to ensure that safe


working conditions are maintained before and after restoration of
motive power.

4.1.5.2 When motive power supply is restored, it shall be carried out by, or
under the direct supervision of, the recipient of the Permit to
Work with Restoration of Motive Power.

4.1.5.3 When the restoration of motive power involves high voltage


switching, the operation shall be carried out by an authorised
person at the request of the recipient of the Permit to Work with
Restoration of Motive Power.

4.1.6 Requirement for Replacement of 11kV Fuse on Ring Main


Units or Fuse Switches

The replacement of high voltage fuses in ring main units and fuse
switches shall be carried out by an authorised person under the
instruction of a control engineer.

4.2 Work on High Voltage Overhead Lines

4.2.1 General Requirements

4.2.1.1 For safety requirements for access to or work on poles, towers and
high structures, reference shall be made to section 2.5.

4.2.1.2 Climbing on 132 kV and 400 kV overhead line towers shall be


carried out in accordance with approved procedures.

4.2.1.3 Auto-reclose facilities for the circuits concerned shall be switched


out of service before work commences within the defined limits of
safe access above the anti-climbing device of the poles or towers
supporting live conductors, except for low voltage fuse
replacement and work on an overhead line remote terminal unit.

4.2.1.4 In the event of the near approach of a lightning storm to an area


where overhead line work is in progress, all work shall cease
immediately and the responsible senior authorised person shall be
informed. Work may be resumed at his discretion and under his on-
site supervision.

4.2.1.5 A person shall not patrol alone an overhead line when visibility is
dangerously impaired or under adverse weather conditions.

4.2.1.6 If it is necessary to patrol an overhead line during darkness, group


patrols equipped with suitable lighting may be undertaken.

4.2.2 Identification at the Point of Work

4.2.2.1 Circuit identification facilities available at all poles, towers or high


structures supporting an overhead line shall be used for the
identification of the overhead line to be worked on at the point of
work.

4.2.2.2 For double circuit overhead line towers, additional identification


facilities including circuit colour plate, wristlet and circuit colour
flag with non-interchangeable socket or bracket shall be used. The
overhead line to be worked on shall be identified by means of such
facilities in accordance with approved procedures.

4.2.2.3 If the overhead lines supported by poles or towers have no circuit


identification facilities, they shall be identified with the aid of
records or diagrams, and visual tracing. The points of work shall be
clearly identified by the attachment of green flags or markers. The
identification shall be carried out by an authorised person.

4.2.2.4 The zone of work and the limits of safe access on poles or towers
carrying high voltage live conductors shall be specified by an
authorised person before work commences.

4.2.2.5 To prevent infringement of the safety distance during work, the


operational person in charge shall ensure clear markers are fixed
at the limits of safe access before work commences at positions
above anti-climbing device of a pole or tower supporting the live
overhead lines, except for low voltage fuse replacement and work
on an overhead line remote terminal unit.

4.2.2.6 The competent person fixing the markers shall be under the
observation of another competent person at ground level.

4.2.3 Requirements for Work Where Safety Distance Can be


Maintained

4.2.3.1 Where safety distance can be maintained, any work on the pole,
tower or high structure above anti-climbing device, other than low
voltage fuse replacement and work on an overhead line remote
terminal unit, shall be carried out under the terms of a Limited
Work Certificate.

4.2.3.2 Even if safety distance can be maintained, work on aerial earth


wires, other than inspection on aerial earth wires, of an overhead
line tower shall be carried out with the respective overhead line
circuit isolated and under the terms of a Permit to Work in
accordance with sections 4.1.1.1 c) i).

4.2.4 Requirements for Work Where Safety Distance Cannot be


Maintained

4.2.4.1 Where safety distance cannot be maintained, work on overhead


line conductors, or other work shall be carried out in accordance
with sections 4.1.1.1 c) i), except for work carried out in accordance
with sections 6.2 and 6.3.

4.2.4.2 At the work site, the senior authorised person issuing the Permit
to Work shall identify to the recipient of the Permit to Work the
circuit upon which it is safe to work on.

4.2.4.3 When issuing a Permit to Work for work on double circuit


overhead line tower with circuit colour plate, the circuit
identification flags and wristlets shall be issued to the Permit to
Work recipient. The items issued and the colour identification of
the circuit shall be recorded on the Permit to Work.

4.2.4.4 Before disconnecting any conductors between distribution


overhead lines and transformers, either additional earths shall be
applied on the transformer side of the overhead line to be
disconnected, or the low voltage fuses of all the transformers on the
section to be disconnected shall be removed.

4.3 Work on High Voltage Cables and Associated Pilot Cables

4.3.1 General Requirements for Work on High Voltage Cables

4.3.1.1 Work on external parts of a cable, where the nature of the work
does not give rise to danger from the power system, shall be
carried out in accordance with section 4.1.1.1 a).

4.3.1.2 Work on conductors or work, which may give rise to danger from
power system, shall be carried out in accordance with section
4.1.1.1 c) i).

4.3.1.3 Positive identification of cable shall be carried out:


a) by making reference to cable records, where necessary
supported by tests in accordance with approved procedures, or

b) by visual tracing of the cable from the point of work along the
cable to the apparatus which has already been identified.

4.3.1.4 Before issuing a Permit to Work, the senior authorised person


shall carry out or witness the cable identification unless the cable
has been spiked or cut.

4.3.1.5 The identified cable shall be marked clearly at the point of work
either by or in the presence of the senior authorised person who
has carried out or witnessed the cable identification.

4.3.1.6 All cables shall, where reasonably practicable, be spiked with an


approved spiking device before cutting unless the cable is
identified in accordance with section 4.3.1.3 b).

4.3.1.7 When it is not desirable to spike a cable and it is not practicable to


trace visually in accordance with section 4.3.1.3 b), identification of
the cable shall be carried out by an authorised person and repeated
by a senior authorised person.

4.3.1.8 At the work site, the senior authorised person shall personally
identify to the recipient the cable to be worked on when issuing the
Permit to Work.

4.3.1.9 Cable identification, spiking, cutting, core identification and work


of minor nature associated with a test, if necessary, may be carried
out under the terms of a Sanction for Test in accordance with
approved procedures.

4.3.1.10 When work is carried out on conductors of a cable directly


connected to an overhead line, earths shall be connected to the
cable sealing end or uptake connections to the overhead line.

4.3.1.11 When work is carried out on cable circuits that are in close
proximity to other live circuits, and on cables with fully insulated
metallic sheaths, special precautions shall be taken in accordance
with approved procedures to avoid danger arising from induced
voltage.

4.3.2 General Requirements for Work on Pilot Cable

4.3.2.1 Before work involving cutting or disconnecting of pilot cable is


carried out, the pilot cable shall be identified and clearly marked by
an authorised person in charge of the work.

4.3.2.2 Work on the pilot cable shall be carried out using an effective
combination of approved insulating mat, insulating boots,
insulating gloves and insulated tools in accordance with approved
procedures.

4.4 Work on High Voltage Switchgear

4.4.1 General Requirements

4.4.1.1 When work is to be carried out on or near the conductors of high


voltage switchgear, it shall be done in accordance with section
4.1.1.1 c) i), except for the work stated in section 4.4.6.

4.4.1.2 Before work commences, the senior authorised person shall


identify to the Permit to Work recipient at the work site the
apparatus to be worked on.

4.4.1.3 Opening a cover of a multi-panel switchboard, which is not totally


dead, in order to gain access to isolated high voltage conductors:
a) shall be carried out under the direct supervision of a senior
authorised person if the cover is for the access to a busbar or
the cover which has no clear identification label, or

b) may be carried out without the direct supervision of a senior


authorised person if the cover is not for the access to a busbar
and the cover has a clear identification label which has been
positively identified by a senior authorised person at the point
of work.

4.4.2 Requirements for Work on a Totally Dead Switchboard and


Single-Panel Switchboard up to 33kV

4.4.2.1 All circuits connected to the switchboard to be worked on shall be


made dead, isolated and earthed at the ends remote from the
switchboard.

4.4.2.2 All secondary fuses, links or miniature circuit breakers of the


associated voltage transformers shall be removed or switched off as
an additional precaution.

4.4.2.3 Where a circuit is connected to an overhead line, circuit main


earths shall be applied at the nearest location(s) to the point of
work where access to the conductors can safely be obtained.

4.4.2.4 Additional earths shall, where practicable, be applied at all


feeder circuits of the switchboard to be worked on before work
commences.

4.4.2.5 Work on a totally dead switchboard shall be carried out in


accordance with approved procedures.

4.4.3 Requirements for Work on Busbar Spouts, Busbars and Busbar


Connections of a Multi-Panel Switchboard Which is Not Totally
Dead

4.4.3.1 For single busbar switchboard:


a) The section of busbar to be worked on shall be isolated from all
sources of supply.

b) All spouts shutters with live conductors shall be locked shut


and attached with danger live notices.

4.4.3.2 For double busbar switchboard:


a) The section of busbar to be worked on shall be isolated from all
sources of supply.

b) For withdrawable switchgear with the associated circuit breaker


not in service position, shutters of spouts containing live
conductors shall be locked shut and attached with danger live
notices.

c) The busbar spout shutters of the dead busbar section to be


worked on shall be locked shut to prevent circuit breakers from
being racked in to the dead busbar section, except under the
condition as stated in section 4.4.3.2 d). Isolation notices shall
be attached to the spout shutters if they are points of isolation.

d) When it is not desirable to rack out a circuit breaker to achieve


the requirement as stated in section 4.4.3.2 c) in order to
maintain supply to customers, the circuit breaker shall be
prevented from being racked out in accordance with approved
procedures until the Permit to Work or Sanction for Test on
the section of the busbar is cancelled. Isolation notices shall
also be attached.

4.4.3.3 Before work on the isolated busbar or busbar connections


commences, the senior authorised person shall, where
practicable, prove the busbar or busbar connections are dead in
accordance with section 1.5.

4.4.3.4 For work on busbar spouts, a Permit to Work shall be issued to a


senior authorised person. The work shall be carried out under his
direct supervision and in accordance with approved procedures.

4.4.3.5 Before work on busbar spouts commences, the senior authorised


person shall personally identify the busbar spouts to be worked on
and prove that each busbar spout is dead in accordance with section
1.5.

4.4.4 Requirements for Work on Feeder Spouts and Voltage


Transformer Spouts of a Multi-Panel Switchboard Which is
Not Totally Dead

4.4.4.1 Where the feeder spouts are connected to an overhead line circuit,
earths shall be applied at the nearest location to the point of work
where access to the conductors can safely be obtained.

4.4.4.2 For work on feeder spouts or voltage transformer spouts, a Permit


to Work shall be issued to a senior authorised person. The work
shall be carried out under his direct supervision and in accordance
with approved procedures.

4.4.4.3 Before work on feeder spouts commences, the senior authorised


person shall personally identify the spouts to be worked on and
prove that each spout is dead in accordance with section 1.5.

4.4.4.4 Before work on voltage transformer spouts commences, the senior


authorised person shall personally identify the spouts to be
worked on and shall, where practicable, prove that each spout is
dead in accordance with section 1.5.

4.4.4.5 If the only earthing devices that can be applied to the circuit are
those applied in the spouts and these are circuit main earths, then,
while this work is in progress, no other work shall be carried out on
the circuit connected to these spouts.

4.4.4.6 To facilitate the work, the senior authorised person may, with the
consent of a control engineer, remove the circuit main earths at
the point of work. On completion of the work, the circuit main
earths shall be re-applied.

4.4.5 Requirements for Work on Gas Insulated Switchgear

4.4.5.1 Work not involving access to a gas zone and the nature of work
does not give rise to danger from power system, shall be carried
out in accordance with section 4.1.1.1 a).

4.4.5.2 For work involving access to an SF6 gas zone, the following shall
also be observed in addition to the requirements stated in section
4.1.1.1 c) i):
a) Where the integrity of the isolation is dependent on the presence
of SF6 gas, the gas pressure shall be monitored while the Permit
to Work is in force;

b) The senior authorised person shall positively identify the


point(s) of access to the gas zone to the recipient of the Permit
to Work;

c) Before work commences within an SF6 gas zone under the terms
of a Permit to Work:

i) The senior authorised person issuing the Permit to Work


shall identify the SF6 gas supplying valves, if any, and the
drain valves to the Permit to Work recipient;
ii) All SF6 gas supplying valves to the gas zone shall, where
practicable, be locked in the closed position with work in
progress notices attached by the Permit to Work recipient
or under his direct supervision;
iii) The Permit to Work recipient shall remove the SF6 gas from
the gas zone in accordance with approved procedures, and
iv) Drain valves of the gas zones shall, where practicable, be
locked in the open position with work in progress notices
attached by the Permit to Work recipient or under his direct
supervision.

d) If more than one working party is working under the terms of


Permit to Work within the same gas zone:

i) none of the working parties shall be allowed to refill SF6


gas;
ii) before refilling SF6 gas, all the E&M safety document shall
be cancelled;
iii) only one Permit to Work shall be issued within the gas
zone, for refilling SF6 gas to the gas zones, and
iv) the recipient of the Permit to Work shall ensure all
personnel are withdrawn from the gas zone.

4.4.5.3 Gas sampling and filling in live apparatus shall be carried out in
accordance with approved procedures and precautions shall be
taken to avoid leakage of SF6 gas as it will affect the safe operation
of the apparatus.

4.4.6 Requirements for Work on Withdrawable Apparatus

4.4.6.1 When withdrawable apparatus is isolated and removed from its


service position, its conductors shall be discharged to earth.

4.4.6.2 All spout shutters not required to be opened for immediate work,
testing or operation for the withdrawable apparatus, shall be
locked shut with safety locks. Danger live notices shall be
attached to shutters of spouts that are live.

4.4.6.3 Where such apparatus remains on site, the work shall be carried
out under the terms of a Limited Work Certificate, except for
work stated in section 4.4.2.

4.4.6.4 Where work inside the housing of the withdrawable apparatus is


required, the work shall be carried out under the terms of a Limited
Work Certificate, except for work stated in section 4.4.2.

4.4.6.5 When under the terms of a Limited Work Certificate, functional


check on withdrawable circuit breakers that have been withdrawn
may be carried out without reference to a control engineer.

4.5 Work on High Voltage Transformers and Reactors

4.5.1 General Requirements

4.5.1.1 When work is carried out on external parts and auxiliary equipment
of a transformer or reactor, and where the nature of work does not
give rise to danger from the power system, it shall be done in
accordance with section 4.1.1.1 a).

4.5.1.2 When work is carried out near any exposed live high voltage
conductor, where the safety distance can be maintained, it shall be
carried out in accordance with section 4.1.1.1 b).

4.5.1.3 When work is carried out on or near the conductors of high


voltage transformer or reactor, it shall be done in accordance with
section 4.1.1.1 c) i).

4.5.1.4 Where applicable, the transformer or reactor shall be isolated from


all common neutral earthing equipment. This does not require
disconnection of solidly earthed neutral equipment.

4.5.1.5 Circuit main earths shall, where applicable, be connected between


the point of isolation from the common neutral earthing equipment
and the point of work in accordance with approved procedures.

4.5.1.6 If circuit main earths are not visible from the point of work,
additional earth shall, where practicable, be applied to the
exposed conductors of the transformer or reactor before work
commences.

4.5.1.7 Where the nature of work is such that danger may arise from the
inadvertent operation of auxiliary apparatus, such apparatus shall
be isolated and, where practicable, be secured with safety locks.

4.5.1.8 Before work commences, the senior authorised person shall


identify to the Permit to Work recipient at the work site the
transformer or reactor to be worked on.

4.5.2 Requirement for Work on Motor Driven Tap Changers

Low voltage supply to tap changer driving mechanisms shall be


isolated, and where practicable, be secured with safety locks and
work in progress notices attached before an E&M safety
document is issued.

4.6 Work on High Voltage Apparatus Operated by Compressed


Air, Gas or Hydraulic Power

4.6.1 General Requirements

Other than operating adjustments, when any work is carried out on


high voltage apparatus or associated plant operated by
compressed air, or on the hydraulic system or hydraulically
operated mechanism, the following precautions shall be taken:

4.6.1.1 The plant or apparatus including the prime mover of plant shall
be isolated from all sources of energy supply. The isolating devices
shall, where practicable, be secured with safety locks and work in
progress notices attached.

4.6.1.2 Stored energy shall be released, the associated system drained or


de-pressurized and the valves shall, where applicable, be secured in
the open position with safety locks and work in progress notices
attached.

4.6.1.3 Charged springs shall, where applicable, be released and the


charging mechanism shall, where practicable, be secured in the
de-energised position with safety locks and work in progress
notices attached.

4.6.2 Requirement for Operating Adjustment

All operating adjustments shall be carried out under the direct


supervision of the recipient of a Permit to Work with
Restoration of Motive Power.

4.7 Work on High Voltage Connections to Generator Windings,


and the Windings of Unit and Generator Transformers

4.7.1 General Requirements

4.7.1.1 Work on or testing of the connections to the windings of a


generator, or the windings of the associated generator transformer
or unit transformer, shall be carried out in accordance with section
4.1.1.1 c) i).

4.7.1.2 All associated voltage transformers shall be isolated, and where


practicable, be secured with safety locks and isolation notices
attached to prevent the possibility of the windings being made live
by back-feed.

4.7.1.3 The excitation supplies shall be isolated, and where practicable,


be secured with safety locks and isolation notices attached.

4.7.1.4 Energy supplies to the prime mover shall be isolated.

4.7.1.5 Circuit main earths shall be applied at all points of isolation from
high voltage supplies.

4.7.1.6 Circuit main earths shall also be applied at a point:


a) common to the generator, unit transformer, and generator
transformer windings if a direct electrical connection exists, or

b) on the generator side of circuit breaker, if a circuit breaker exists


and if work is carried out on the high voltage connection to
generator windings and/or on the windings themselves.

4.7.2 Requirements During Work

4.7.2.1 Before any work commences, which involves opening up of any


part of a generator casing normally containing hydrogen, reference
shall be made to section 8.11 for safety requirements for work on
hydrogen filled plant.

4.7.2.2 The machine shall be at rest during work, unless permission has
been given by a senior authorised person issuing the E&M safety
document, for the work to be carried out while the machine is
turning on barring gear.

4.8 Work on High Voltage Static Capacitors

4.8.1 General Requirements

4.8.1.1 Work on capacitors shall be carried out in accordance with section


4.1.1.1 c) i).

4.8.1.2 Before applying earths, adequate time delay shall be allowed for
discharge of the capacitor units.

4.8.1.3 An integral earth switch shall, where practicable, be used to


initially discharge the capacitor units.

4.8.1.4 Where integral earth switches are not provided, the initial
discharge of the capacitor units shall only be carried out through a
circuit breaker or other switching device.

4.8.1.5 Circuit main earths shall be applied under the direct supervision
of a senior authorised person, on capacitor units by applying
portable earth leads to:
a) the phase conductors;

b) the capacitor frames, and

c) the neutral connections of each group of capacitor units, if the


neutral point is not directly connected to earth.

4.8.1.6 Before any disconnection is carried out on capacitor units, an


additional earth shall, where practicable, be applied on both
sides of, and in close proximity to, the point of disconnection.

4.8.1.7 Capacitor units shall be short-circuited and remain short-circuited


on removal from the capacitor group.

4.9 Work In Enclosure Containing Exposed Live High Voltage


Conductors

4.9.1 General Requirement

4.9.1.1 Safety distances shall be maintained at all times from the nearest
exposed high voltage conductors that are, or may become, live.

4.9.1.2 For work on overhead line poles, towers and high structures
carrying exposed live high voltage conductors, reference shall be
made to section 4.2.

4.9.2 Requirements for Maintaining Safety Distance During Work

When work is to be carried out in an enclosure, where there are


exposed live high voltage conductors and there is a risk of
infringing the safety distance, the following precautions shall be
taken:

4.9.2.1 Identification of zone of work


a) The zone of work and its access shall be identified and defined
by a senior authorised person.

b) The zone of work shall, where practicable, be demarcated and


made clearly distinguishable by suitable means in accordance
with approved procedures, so that the safety distance can be
maintained at all times.

c) Danger live notices and other items used for demarcation of the
zone of work shall be fixed or moved only by, or under the
direct supervision of, an authorised person.

d) Danger live notices shall be attached to adjacent apparatus,


which is, or may become, live.

e) Items used for demarcation of the zone of work shall be clearly


visible, and shall not be attached to any structure supporting
apparatus, which is, or may become, live.

f) For work which requires leaving ground level or a platform or


access way, the access shall only be made by means of an
approved ladder or other approved means in accordance with
sections 4.9.2.2 and 4.9.2.3. Climbing on structures to gain such
access is forbidden.

4.9.2.2 Use of portable ladders and long objects


a) Portable ladders shall be of an approved type and of no greater
length than that is required for the work to be carried out.

b) The use of portable ladders or long objects shall only be


permitted by a senior authorised person who shall define the
conditions of use to the competent person in charge of the
work.

c) The movement and erection of the portable ladders and long


objects shall be defined on site by a senior authorised person,
and shall be carried out under the direct supervision of an
authorised person.

d) When portable ladders or long objects are being moved at


ground level, they shall be carried in a horizontal position, and
as near to the ground as is practicable.

e) Portable ladders that give access to fixed ladders that terminate


above ground level shall be secured in position with locks by an
authorised person. They shall remain locked during the period
that the ladders are in use.

f) When no longer required, all portable ladders shall be removed


from enclosures containing exposed live high voltage
conductors.

4.9.2.3 Use of cranes, scaffolds and other mobile access equipment


a) The route of movement of cranes, scaffolds and other mobile
access equipment within an enclosure shall be agreed, and
defined on site by a senior authorised person.

b) The limits of operation of cranes and other mobile access


equipment or the limits of erection of scaffolds shall be defined
by a senior authorised person to an authorised person in
charge of the work.

c) Thereafter, such equipment shall only be erected, dismantled,


moved or operated within these limits under the direct
supervision of the authorised person.

d) Cranes, metal scaffolds or other mobile access equipment shall


be connected to the substation earth system on reaching the work
area.

4.9.2.4 When work is carried out in an enclosure containing exposed live


high voltage conductors, where safety distance can be
maintained, a senior authorised person shall determine the work
to be carried out under:
a) the terms of an E&M safety document, or

b) the on-site supervision of an authorised person at the point of


work.

4.9.3 Adverse Weather Conditions

In the event of the near approach of a lightning storm to an area


where work on exposed conductors in outdoor substations or
apparatus connected to exposed conductors is in progress, all
work shall cease immediately and the responsible senior
authorised person shall be informed. Work may be resumed at his
discretion and under his on-site supervision.

4.10 Work on Plant or Apparatus with Local, Remote or Automatic


Control

4.10.1 General Requirements for Work on or Testing of Plant or


Apparatus

4.10.1.1 Before work or testing is carried out on plant or apparatus, in


addition to the requirements specified in section 4.1.1.1 c) i), the
associated local, remote and automatic control devices shall be
rendered inoperative, and where practicable, be secured with
safety locks and work in progress notices attached to each of
them.

4.10.1.2 No work shall be carried out on the controlling equipment and the
associated wiring and relays that have been rendered inoperative in
section 4.10.1.1, if such work may restore the operational status of
the control functions.

4.10.2 General Requirements for Work on or Testing of Controlling


and Protection Equipment

4.10.2.1 Work on, testing of or making adjustments to controlling and the


protection equipment of plant or high voltage apparatus in service
shall be carried out with the consent of a control engineer by a
competent person, who has adequate knowledge on the equipment.

4.10.2.2 Work on or testing of controlling and protection equipment in order


to trigger the operation of switching device of a high voltage
apparatus which is in service, shall be carried out by an
authorised person who shall receive instructions directly from a
control engineer.

4.10.2.3 For work on protection equipment, which involves the operation of


switching devices, that are in service, at more than one location, the
authorised person shall coordinate the whole process including the
removal and replacement of trip links and fuses. In this case, the
authorised person may instruct another authorised person at the
remote end substation to remove and replace trip links and fuses.

5. Work On Or Testing Of Low Voltage Apparatus

Work on or testing of low voltage apparatus with the conductors


live shall be carried out in accordance with section 6.1. Work on or
testing of low voltage apparatus with the conductors dead shall
be carried out in accordance with the following requirements.

5.1 General Requirements for Work on or Testing of Low Voltage


Apparatus

5.1.1 Work on or testing of a low voltage apparatus shall be carried out


under the following conditions:

5.1.1.1 The apparatus shall be taken out of service;

5.1.1.2 The apparatus shall be isolated and the points of isolation of the
apparatus shall, where practicable, be secured with safety locks;

5.1.1.3 Where fuses or links are removed for the isolation and safety locks
cannot be applied, the fuses or links shall be retained in a secure
location;

5.1.1.4 Isolation notices shall have been attached to the points of


isolation, and

5.1.1.5 Work on or testing of a low voltage apparatus shall be carried out


by a competent person or under his on-site supervision.

5.1.2 Before work or testing commences, a competent person of the


work team shall, where practicable:

5.1.2.1 screen off any adjacent exposed conductor, which is, or may
become, live and from which danger may arise, and

5.1.2.2 prove the apparatus to be worked on is dead in accordance with


section 1.5.

5.1.3 Before work or testing resumes after suspension, a competent


person of the work team shall, where practicable, prove the
apparatus to be worked on is dead in accordance with section 1.5,
unless the apparatus has been isolated and secured with safety
locks and the work or testing is carried out under the terms of an
E&M safety document.

5.2 Requirements for Work on Low Voltage Overhead Lines

5.2.1 For safety requirements for access to or work on poles and high
structures, reference shall be made to section 2.5.

5.2.2 Before work or testing commences, the following requirements


shall be observed:

5.2.2.1 Conductors to be worked on shall be isolated in accordance with


section 5.1.1.2;

5.2.2.2 Conductors to be worked on shall be proved dead in accordance


with section 1.5 at the point of work;

5.2.2.3 Exposed conductors including the neutral to be worked on shall be


short-circuited by portable earth leads and where practicable, be
earthed with approved equipment.

5.2.3 When work or testing on low voltage apparatus mounted on a pole


supporting high voltage conductors is required and where safety
distance cannot be maintained, the work shall be carried out under
the terms of a Permit to Work in accordance with Section 4.1.1.1
c) i).

5.3 Requirements for Work on Low Voltage Cables

5.3.1 Positive identification of cable shall be carried out:

5.3.1.1 by making reference to cable records, where necessary supported by


tests in accordance with approved procedures, or

5.3.1.2 by visual tracing of the cable from the point of work along the cable
to the apparatus which has already been identified.

5.3.2 Spiking a low voltage cable to prove it dead is not essential. If not
spiked, each core of the cable shall be exposed in turn by stripping
the cable to prove it dead in accordance with section 1.5 before
work commences.

5.4 Requirements for Work on Low Voltage Terminals of Ground


Mounted Distribution Transformers

5.4.1 The high voltage side of the transformer shall be isolated and
secured with safety locks.

5.4.2 Portable earth leads shall, where reasonably practicable, be


applied to the conductors on the low voltage side of the
transformer.

5.4.3 Portable earth leads shall be applied as near as practicable to the


points of work.

5.4.4 A Limited Work Certificate shall be issued for the work unless
the work is covered under the terms of a Permit to Work.

5.5 Requirements for Work on Low Voltage Capacitor Bank

After the capacitor bank has been isolated, the cover for the access
to the conductor shall only be opened after an adequate time delay
to allow for discharge of stored electrical energy from the capacitor
units.

6. Requirements For Live Work

6.1 Low Voltage Live Work

6.1.1 General Requirements

6.1.1.1 Work on live apparatus shall only be carried out by a competent


person who has received approved training and has been duly
authorised to carry out such work.

6.1.1.2 Work on live apparatus shall be carried out in accordance with


approved procedures and statutory requirements.

6.1.1.3 On-site risk assessment shall be conducted by a competent person


possessing the knowledge and experience to identify potential
hazards present on site and to take safety precautions required prior
to the commencement of any live work.

6.1.2 Requirements for Work on Live Low Voltage Apparatus

6.1.2.1 All adjacent metal which is electrically connected to earth, or low


voltage conductors that are at a different potential to that on which
work is to be carried out, shall be screened or guarded with suitable
insulating material to avoid danger.

6.1.2.2 Appropriate safety equipment and personal protective equipment


shall be used in accordance with section 1.4 and approved
procedures.

6.1.2.3 Access to the work area shall be controlled by suitable means.

6.1.2.4 Adequate working space and adequate lighting shall be provided.

6.1.2.5 Weather conditions shall be taken into consideration.

6.1.2.6 The neutral conductor shall be connected before phase conductors


when connecting the apparatus to live apparatus.

6.1.2.7 The phase conductors shall be disconnected before neutral


conductor when disconnecting the apparatus from live
apparatus.

6.1.3 Requirements for Work on Live Low Voltage Overhead Line

6.1.3.1 For safety requirements for climbing or working on poles and high
structures, reference shall be made to section 2.5.

6.1.3.2 Live work on steel poles is not allowed.

6.1.4 Testing and Running Adjustments on Live Low Voltage


Apparatus

6.1.4.1 Testing or running adjustments, including functional testing, may


be carried out with the low voltage apparatus live, provided that
approved insulated tools are used to avoid danger.

6.1.4.2 If the testing or running adjustments require the covers of the


apparatus to be removed so that the conductors that are live or
may become live, are exposed, precautions shall be taken to prevent
any unauthorised access to, or interference with, the apparatus.

6.2 High Voltage Live Line Work on 11kV Overhead Lines

6.2.1 General Requirements

6.2.1.1 Personnel

11kV live line work shall only be carried out by an 11kV live line
work team, the members of which have been appointed as the
authorised persons (HVLLW).

6.2.1.2 Techniques, procedures, tools and equipment for the work


a) There are three approved techniques for 11kV live line work:

i) 11kV live line stick technique;


ii) 11kV live line glove and barrier technique, or
iii) the combination of the above two techniques.
b) The work shall be carried out in accordance with approved
procedures using approved tools and approved access
equipment.

c) Approved tools and equipment shall be inspected before use to


ensure that they are clean and dry and in sound condition for the
intended purpose.

d) Approved insulated elevating work platform shall be used to


gain access to the working position and in accordance with
approved procedures.

e) 11kV live line minimum approach distance for carrying out


high voltage live line work on 11kV overhead conductors and
apparatus is 350mm which shall be measured with an
approved measuring device and in accordance with an
approved procedure.

f) The 11kV live line tool effective length of approved insulating


stick for 11kV live line work shall not be less than the 11kV live
line minimum approach distance.

g) Where the 11kV live line minimum approach distance cannot


be maintained, either the conductors and apparatus shall be
covered or the authorised person (HVLLW) carrying out the
work shall be protected by approved insulated equipment in
accordance with approved procedures.

h) Only approved tools and equipment shall be allowed to


approach the exposed live 11kV overhead line conductors and
apparatus within the 11kV live line minimum approach
distance.

i) Live line work on steel poles shall not be carried out.

6.2.1.3 Weather conditions

11kV live line work shall:


a) only be carried out during visible daylight hours, and

b) not commence in adverse weather conditions including strong


winds, lightning storm, wet and foggy weather.

6.2.2 Public Safety

Before any 11kV live line work commences, adequate precautions


shall be taken to avoid creating a hazardous situation to the public.

6.2.3 Safety Precautions

Before any live line work is permitted on live 11kV overhead line
conductors and apparatus, the following safety precautions shall
be taken:

6.2.3.1 Power system safety precautions


a) The protection system of the overhead line shall be capable of
clearing the fault at the 11kV live line work location.

b) All auto-reclose facilities for the overhead line where 11kV live
line work is carried out shall be switched out of service and
tagged prior to work commences.

c) Open points that are operable by remote control on the overhead


line where live line work is carried out shall be tagged and
rendered inoperative.

d) 11kV live line work shall not take place on any section of an
11kV overhead line where the failure of any apparatus,
conductor or tool, could cause the associated or adjacent
overhead line which has been isolated and earthed for work
under the terms of a different E&M safety document, to
become live.

6.2.3.2 Site safety precautions


a) For safety requirements for access to or work on poles, towers
and high structures, reference shall be made to section 2.5.

b) Before work commences, the live line work team on the work
site shall:

i) identify the presence of hazards in accordance with


approved risk assessment procedures;
ii) determine the safety precautions to reduce risk;
iii) establish the method and work sequence, and
iv) have agreements from all members of a 11kV live line work
team, who will be collectively responsible for the safety, the
safety precautions and work procedures that are the safest
and best for the work to be carried out.
c) One member of the 11kV live line work team shall be assigned
to perform the role of 11kV live line work safety observer
during the 11kV live line work is being carried out. The 11kV
live line work safety observer shall be the contact person
between the control engineer and the 11kV live line work
team.

6.2.4 Safety Document

6.2.4.1 A High Voltage Live Line Work Certificate shall be issued for
the 11kV live line work.

6.2.4.2 The 11kV live line work team shall collectively be responsible for
the safety of the 11kV live line work and shall be responsible for
the declaration and acceptance of the High Voltage Live Line
Work Certificate. The 11kV live line work team receives the
High Voltage Live Line Work Certificate shall only commence
the 11kV live line work after the consent of a control engineer is
obtained.

6.2.4.3 The High Voltage Live Line Work Certificate shall be cleared
and cancelled by all members of the 11kV live line work team
under any of the following conditions:
a) on completion of the 11kV live line work;

b) if any of the power system safety precautions cannot be


maintained or any of the site safety precautions cannot be taken,
or

c) at the end of that work shift for which the certificate was issued.

6.2.4.4 The 11kV live line work safety observer shall notify a control
engineer on the cancellation of the HV live line work certificate.

6.2.5 Suspension and resumption of work

6.2.5.1 11kV live line work shall be suspended in the event of the:
a) approach of adverse weather to an area where the 11kV live line
work is in progress; or

b) presence of any person or occurrence of any incident in the


vicinity of the work site which is considered as a distraction to
the HV live line work team.

6.2.5.2 If it is necessary to suspend an 11kV live line work for any reason,
the overhead line and equipment shall be left in a safe condition. A
control engineer shall be notified of the suspension and the state of
the work, and the estimated resumption time or conditions that will
allow work to be resumed.

6.2.5.3 11kV live line work may be resumed at the discretion of all
members of the 11kV live line work team only when they are fully
satisfied that the site condition is safe, and a control engineer is
notified of the resumption of work.

6.2.6 Apparatus Become Dead During the Course of Work

6.2.6.1 If the 11kV overhead line on which work is being carried out
becomes dead for any reason, a control engineer shall
immediately communicate with the 11kV live line work safety
observer to determine the necessary actions to be taken.

6.2.6.2 Apparatus that becomes dead for any reason shall still be regarded
as live. If work on the apparatus continues, it shall be undertaken
in accordance with approved 11kV live line work procedures.

6.2.6.3 The overhead line shall not be re-energised unless it is clarified


with the 11kV live line work safety observer that the re-
energasation of the overhead line will not affect the safety of the
worksite.

6.3 Helicopter Live Line Insulator Washing

6.3.1 General Requirements

6.3.1.1 Personnel

Helicopter live line insulator washing shall only be carried out by


a helicopter live line insulator washing team. All members of this
work team shall be authorised as the authorised persons
(HCLLW) in accordance with approved procedures.

6.3.1.2 Reference documents

The helicopter live line insulator washing requirements shall


make reference to the following documents:
a) Relevant civil aviation regulations;

b) Guidelines on Helicopters for Live Line work, Electricity


Supply Association of Australia Limited (ESAA);

c) IEEE Guide for Maintenance Methods on Energised Power


Lines IEEE Standard 516, and

d) IEEE Guide for Cleaning Insulators IEEE Standard 957.

6.3.2 Approach to Exposed 132kV or 400kV Overhead Structures,


Conductors, Insulators and Apparatus

6.3.2.1 The minimum helicopter approach distances shown in Table 2


below shall be maintained between any part of the helicopter or any
equipment associated with the helicopter, and the nearest exposed
conductors, structures, insulators or apparatus, on the same or
adjacent structures.

6.3.2.3 These air gaps in Table 2 have been derived on the basis of the
switching impulse withstand strengths and for application to an
altitude of 900 metres. The maximum transient overvoltages
considered were:
a) Up to and including132kV a.c. = 3.0 per unit

b) From 132kV upto and including 400kV a.c. = 2.4 per unit

6.3.2.4 The minimum helicopter approach distances shown in Table 2


include an allowance of 0.9 metre for differential conductor
movement, line construction tolerances and the possible degrading
effect on the electrical strength of the air gap due to the helicopter
acting as a floating electrode, and corresponds to the voltage
whichever is the highest under any circumstances (refer to Diagram
2).
a) Any part of the helicopter including its rotor shall maintain a
minimum air distance of 0.9 metre from overhead lines or
adjacent structure to prevent physical contact with overhead
lines or structures.

b) Authorised person (HCLLW) shall not allow any part of his


body or any conductive tool to approach exposed overhead line
conductors and apparatus, or any apparatus at different
potentials, within the minimum helicopter approach distance.

6.3.3 Safety Precautions

Before any helicopter live line insulator washing is permitted on


live overhead line, the following safety precautions shall be taken:

6.3.3.1 Power system safety precautions


a) A control engineer shall take the following power system
safety precautions and then confirm with the authorised person
(HCLLW) who will carry out the work:

i) Check the protection systems of the overhead line are


operational for the circuit where helicopter live line
insulator washing is performed, and
ii) Where applicable, switch out of service the auto-reclose
facilities for the overhead line where helicopter live line
insulator washing is performed.
b) The authorised person (HCLLW) shall record the safety
precautions that are taken by the control engineer on the High
Voltage Live Line Work Certificate.

c) The authorised person (HCLLW) shall only commence work


on a circuit with High Voltage Live Line Work Certificate
after the consent of a control engineer is obtained.

6.3.3.2 Site conditions, site safety precautions and risk assessment


a) The helicopter live line insulator washing work team shall
check that the site conditions are suitable for work.

b) The team shall also carry out site-specific risk assessments to


identify the presence of site-specific hazards and determine the
site safety precautions to avoid danger and reduce risks.

6.3.4 Safety Document

6.3.4.1 Declaration and acceptance


a) The helicopter live line insulator washing work team shall
declare that they:

i) have obtained consent from a control engineer to commence


work;
ii) will take the site safety precautions during work, and
iii) accept responsibility for the work specified on the High
Voltage Live Line Work Certificate.
b) The authorised person (HCLLW) shall then notify a control
engineer of the completion of declaration and acceptance.

6.3.4.2 Clearance and cancellation


a) The High Voltage Live Line Work Certificate shall be cleared
and cancelled by all members of the helicopter live line
insulator washing work team:

i) on completion of the helicopter live line insulator washing;


ii) if any of the power system safety precautions cannot be
maintained or any of the site safety precautions cannot be
taken, or
iii) at the end of that work shift which the certificate was issued.
b) The authorised person (HCLLW) shall then notify a control
engineer of the cancellation of the High Voltage Live Line
Work Certificate.

6.3.5 Suspension and Resumption of Helicopter Live Line Insulator


Washing

6.3.5.1 The helicopter live line insulator washing shall be suspended at


the request of:
a) a control engineer: or

b) the helicopter live line insulator washing work team.

6.3.5.2 The authorised person (HCLLW) of the helicopter live line


insulator washing work team shall notify a control engineer of
the suspension and the state of the work.

6.3.5.3 Helicopter live line insulator washing may be resumed at the


discretion of all members of the helicopter live line insulator
washing team only when they are fully satisfied that the site
conditions are safe for helicopter live line insulator washing, and
consent from a control engineer is obtained for the resumption of
work.

7. Inter-system Safety

When work or testing is carried out on inter-system plant or high


voltage apparatus which connects to CLP Power’s power system
and has one or more point(s) of high voltage supply outside the
control of the system control centre of CLP Power, the following
requirements shall be observed.

7.1 General Requirements

7.1.1 Work or testing carried out on any inter-system plant or apparatus


and the safety precautions to be taken shall be agreed by all parties
concerned before the work or testing commences.

7.1.2 Switching and earthing required for making safe the inter-system
plant or apparatus for work or testing shall be agreed by:

7.1.2.1 a senior authorised person and the authorised representative of the


customer for plant or apparatus connecting to customer’s high
voltage equipment, or

7.1.2.2 the control centres concerned for plant or apparatus connecting


between CLP Power’s power system and the neighbouring
systems.

7.1.3 The inter-system plant or apparatus to be worked on or tested


shall be isolated and earthed by the respective control centres or
customer’s authorised representatives.

7.1.4 Work or testing on or near inter-system plant or apparatus shall


be carried out in accordance with section 8 or section 4.1.1.1 c) i)
respectively.

7.1.5 Declaration shall be made by appropriate parties controlling the


remote ends of the inter-system plant or apparatus.

7.1.6 An appropriate E&M safety document shall be issued for work or


testing on the inter-system plant or apparatus when work is
carried out by CLP Power personnel.

7.2 Work on Inter-system Plant or Apparatus

7.2.1 For the requirements for testing of inter-system apparatus, which


requires the issue of a Sanction for Test, reference shall be made
to section 7.3.

7.2.2 Declaration of the status of isolation and earthing on the plant and
apparatus shall be made by the issue of an appropriate E&M
status document or other appropriate documents.

7.2.3 For work on inter-system circuit connected to CLP Power


generation station or neighbouring power utility, declaration shall
be made between the relevant control centres by issuing an
appropriate E&M status document.

7.2.4 For CLP Power’s work on inter-system circuit connected to


customer high voltage equipment, the senior authorised person
on site shall request the customer’s authorised representative to
declare the status of isolation and earthing on the customer side by
issuing an appropriate E&M status document or an appropriate
document. The senior authorised person shall then notify a
control engineer of the issue of the document.

7.2.5 For customer’s work on inter-system circuit connected to customer


high voltage equipment, the senior authorised person on site shall
issue an E&M status document declaring the status of isolation
and earthing on the CLP Power side to customer’s authorised
representative.

7.2.6 The status of the isolation and earthing devices stated on the
E&M status document or the appropriate document shall not be
altered when the document is in force.

7.2.7 Permit to Work shall then be issued, without reference to all other
relevant control centres or the customer’s authorised representative,
for work on the inter-system plant or apparatus by CLP Power
personnel.

7.2.8 The E&M status document or the appropriate document shall not
be cancelled unless all relevant control centres or customer’s
authorised representative are clear of any work and all Permit(s) to
Work issued on the plant or apparatus making reference to the
E&M status document have been cancelled.

7.3 Testing of Inter-system Apparatus

7.3.1 Before a Sanction for Test is issued on an inter-system apparatus,


the respective apparatus shall be clear of any work or testing.

7.3.2 Only one party shall conduct testing on the inter-system apparatus
at any one time.

7.3.3 For testing of inter-system circuit connected to CLP Power


generation station or neighbouring power system(s), the following
shall be observed:

7.3.3.1 Any E&M status document in force shall be cancelled;

7.3.3.2 Only one Sanction for Test for testing of the circuit shall be issued
by any control centre at any one time;

7.3.3.3 The control centre, which has consented to the Sanction for Test,
shall reach agreement for carrying out the testing with all other
relevant control centres;

7.3.3.4 The authorised person in charge of the testing may, without


reference to a control engineer, remove the circuit main earths on
CLP Power side to facilitate the testing, and

7.3.3.5 The authorised person in charge of the testing may also request a
control engineer to request any other relevant control centre to
instruct their authorised person to remove the circuit main earths
on their side to facilitate the testing.

7.3.3.6 Before the Sanction for Test is cancelled, all circuit main earths
listed on the document shall be re-applied.

7.3.4 For testing of inter-system circuit connected to customer high


voltage equipment, the following shall be observed:

7.3.4.1 All E&M safety document and E&M status document on the
isolated section of the apparatus to be tested shall be cancelled;

7.3.4.2 The senior authorised person on site shall request the customer’s
authorised representative to issue him an appropriate E&M status
document or an appropriate document declaring the status of
isolation and earthing on the customer side, and

7.3.4.3 The application or removal of circuit main earths at customer side


to facilitate the testing shall be carried out by the customer’s
authorised representative only upon the request of the recipient of
the Sanction for Test.

8. Work On Or Testing Of Plant

8.1 General Requirements

8.1.1 Repair, maintenance, testing, cleaning or alteration on plant shall


only commence after the plant has been rendered safe.

8.1.2 Work on plant shall only commence after:

8.1.2.1 the plant has been isolated from all sources of danger;

8.1.2.2 where applicable, the stored energy has been properly released;

8.1.2.3 where applicable, the hazardous substances have been removed, and

8.1.2.4 a Permit to Work has been issued.

8.1.3 Isolating devices recorded on a safety document as points of


isolation shall not be operated when that safety document is in
force.

8.1.4 Where operation of plant by restoration of motive power is


required for testing or making adjustments, a Permit to Work with
Restoration of Motive Power shall be issued.

8.1.5 In cases where Permit to Work or Permit to Work with


Restoration of Motive Power are not applicable and verbal
instructions are considered insufficient, a Limited Work
Certificate shall be issued.

8.1.6 For access to and work in plant area, which is protected by fixed
fire fighting equipment, reference shall be made to approved
procedures and section 2.2.4, OSH Rules.

8.2 Preparation for Work on Plant

8.2.1 Isolation

8.2.1.1 The plant shall be readily identifiable or have fixed to it a means of


identification, which will remain effective throughout the work.

8.2.1.2 The plant shall be isolated and all practicable steps be taken to
lock off from all sources of danger. Isolation notices shall be fixed
at all points of isolation.

8.2.1.3 The plant shall be immobilised by isolation from all sources of


energy and, where necessary, by taking precautions to prevent
rotation or movement.

8.2.1.4 The plant shall, where necessary, be screened to avoid danger.


Appropriate warning notices shall be attached to plant from which
danger may arise.

8.2.1.5 A control valve and a non-return valve shall not be employed as an


isolating device, except for screw down type non-return valve.

8.2.1.6 A double shut-off valve shall, where reasonably practicable, be


used to achieve isolation.

8.2.1.7 Where fuses or links are removed for the isolation of apparatus
associated with isolation of plant and safety locks cannot be
applied, the fuses or links shall be retained in a secure location.

8.2.2 Dissipation of Stored Energy

The plant shall, where applicable, be:

8.2.2.1 made safe from danger which could arise from the release of stored
energy, by taking action to contain or dissipate this energy safely;

8.2.2.2 vented, where danger could arise from pressurisation. Where vents
could give rise to danger, they shall be secured with safety locks
in the appropriate positions;

8.2.2.3 drained, or the contents reduced to a level which avoids danger.


Where the drains could give rise to danger, they shall be secured
with safety locks in the appropriate positions, and

8.2.2.4 cooled to a temperature which does not give rise to danger during
work or internal access.

8.2.3 Removal of Hazardous Substances

The plant shall be purged and cleared of hazardous substances


within the limits of isolation, in accordance with approved
procedures.

8.3 Testing of Plant

8.3.1 All necessary safety precaution to avoid danger shall be specified


on a Permit to Work with Restoration of Motive Power.

8.3.2 Safe working conditions shall be maintained before and after


restoration of motive power.

8.3.3 The safety document recipient shall keep the keys for the
restoration of the motive power for the test.

8.3.4 Floating of boiler safety valves shall be carried out in accordance


with approved procedures.

8.4 Work on System Containing Hazardous Substances

In addition to the requirements in sections 8.1, 8.2 and 8.3,


precautions shall also be taken to avoid danger from handling
flammable, explosive, toxic, harmful, corrosive, irritating,
oxidizing, or carcinogenic substances, when the system is being
isolated, vented, drained and/or rinsed. These precautions are as
follows:

8.4.1 Hazardous properties of the substances shall be observed and


appropriate safety precautions shall be taken;

8.4.2 Incompatible substances shall not be mixed;

8.4.3 Water shall not be added to pure or concentrated acids;

8.4.4 Appropriate personal protective equipment shall be used to avoid


exposure to or contact with hazardous substances;

8.4.5 Chemical systems shall be isolated in accordance with approved


procedures;

8.4.6 Depressurizing, venting, draining or rinsing of chemical systems


shall be carried out in accordance with approved procedures;

8.4.7 All spillage and waste substances shall be handled in accordance


with approved procedures;

8.4.8 Chemical cleaning for boiler and heat recovery steam generator
shall be carried out in accordance with approved procedures, and

8.4.9 Reference shall also be made to section 3.5.1, OSH Rules for the
requirements for hazardous substances control.

8.5 Work on Pulverised Fuel System

In addition to the requirements in sections 8.1, 8.2 and 8.3, the


following precautions shall also be taken before carrying out work
on coal pulverising plant.

8.5.1 Requirements for access inside pulverised fuel mills

8.5.1.1 All coal shall, where practicable, be run off.

8.5.1.2 The mill shall, where practicable, be purged.

8.5.1.3 All electrical supplies to the mill group shall be isolated and,
where practicable, be secured with safety locks.

8.5.1.4 The mill shall be isolated from the furnace, hot air, fire fighting
system, steam or gas inerting system, and coal supplies.

8.5.1.5 The mill group shall be cooled and checked for the presence of fire
within the mill.

8.5.1.6 Coal and pulverised fuel surrounding the mill shall be removed.

8.5.1.7 A Permit to Work shall be issued for the removal of covers to


permit testing of the mill atmosphere.

8.5.1.8 Before access to the mill, reference shall be made to section 3.5.2,
OSH Rules for the safety requirements for entry to or work in a
confined space.

8.5.2 Requirements for work on pulverised fuel pipework

8.5.2.1 For external work on pulverised fuel pipework, which may give rise
to danger, the associated mill group shall be isolated, locked off,
and a Permit to Work issued.

8.5.2.2 For work that involves the dismantling of pulverised fuel pipework,
the section to be dismantled shall, where practicable, be isolated
from the furnace by means of a blank.

8.5.2.3 Non-return flap valves shall not be used as a point of isolation.

8.5.3 Work in coal feeders or chutes

8.5.3.1 The feeder shall be isolated from the bunker and, where
practicable, the coal in the feeder, chute and mill run off.

8.5.3.2 The associated mill group shall be isolated.

8.5.3.3 A Permit to Work shall be issued.

8.5.3.4 If internal access to feeder or chute is required, reference shall be


made to section 3.5.2, OSH Rules for the safety requirements for
entry to or work in a confined space.

8.5.4 Work in bunkers or silos

8.5.4.1 Entry into bunkers or silos for the purpose of trimming of coal or
ash is forbidden.

8.5.4.2 Entry into bunkers or silos for the purpose of maintenance or final
cleaning shall be carried out under the following conditions:
a) Arrangements shall, where practicable, be made to ensure that
material cannot be fed into the bunkers or silos during the period
of work;

b) The bunker or silo shall be ventilated and all associated plant


isolated;

c) A Permit to Work shall be issued, and

d) Reference shall be made to section 3.5.2, OSH Rules for the


safety requirements for entry to or work in a confined space.

8.5.4.3 The final cleaning out of material from the bunker or silo shall be
carried out by persons either:
a) working from outside of the bunker, or silo, or

b) working inside the bunker or silo using approved safety


harnesses with life lines. The life lines shall be properly secured
and kept taut during the entire period while the person is in the
bunker or silo, by two persons remaining outside the bunker or
silo. These persons shall be able to warn the person inside of
danger and render assistance if required.

8.6 Work on Flammable Gas Installations

8.6.1 Flammable gas installations refer to fixed plant, which hold natural
gas, liquefied petroleum gas or hydrogen in bulk quantities.

8.6.1.1 Natural gas control area is a designated area in which natural gas
may be present and within which special precautions shall be
adopted.

8.6.1.2 Hazardous gas area is a designated area in which liquefied


petroleum gas or hydrogen may be present and within which special
precautions shall be adopted.

8.6.2 Hazardous areas classification

Hazardous areas (zones 0 to 2) for flammable gas installations shall


be classified in accordance with internationally recognised
standards.

8.6.3 General requirements for work on flammable gas installations

In addition to the requirements in sections 8.1, 8.2 and 8.3, the


following precautions shall also be taken:

8.6.3.1 Smoking is forbidden;

8.6.3.2 Ignition sources shall be controlled;

8.6.3.3 Approved types of non-spark tools shall be used;

8.6.3.4 Vehicle entry into zone 0 or zone 1 is forbidden;

8.6.3.5 A vehicle entry into zone 2 area shall be fitted with a flame arrestor
at the exhaust pipe and the area of access shall be monitored free of
flammable gas;

8.6.3.6 Access inside natural gas control area or hazardous gas area shall
be controlled by approved procedures, and

8.6.3.7 Any excavation work taking place in the vicinity of or over buried
pipelines shall be carried out in accordance with approved
procedures and a Limited Work Certificate shall, where
necessary, be issued.

8.6.4 Specific requirements for work on flammable gas installations

8.6.4.1 Work or testing which involves the opening up of any part of the
plant filled with flammable gas, shall be carried out in accordance
with approved procedures.

8.6.4.2 All flammable gas sources shall be isolated, where applicable,


using the double block and bleed method.

8.6.4.3 The plant to be worked on shall be cleared of flammable gas by


purging with nitrogen or carbon dioxide in accordance with
approved procedures.

8.6.4.4 Venting and purging of flammable gas to the atmosphere shall be


carried out in accordance with the following:
a) Fixed venting system, specially built and designed for venting
purpose, shall, where practicable, be used;

b) Temporary vent pipes shall stand vertically at or above 3 metres


from the working level;

c) Prior to venting and purging, the ignition sources in the vicinity


of anticipated flammable space shall be removed. If any of the
ignition sources cannot be removed, precautions shall be taken
to prevent ignition from these sources;

d) Venting shall be discontinued during thunderstorms, and

e) Due consideration shall be taken of prevailing wind direction


prior to venting gas to the atmosphere.

8.6.4.5 Control of ignition sources


a) Ignition sources shall be controlled when work or activity is
carried out inside the natural gas control area or hazardous gas
area, unless the plant containing flammable gas is isolated and
purged.

b) Only intrinsically safe or explosion protected equipment


appropriate to the classification of hazardous areas, shall be used
within zone 0 or zone 1.

c) Explosion protected equipment appropriate to the classification


of hazardous areas shall be used.

d) General tools shall only be used in open spaces with good


ventilation. The area of work shall be closely monitored to
detect that it is free from flammable gas during the course of the
work.

e) Hot work or any potentially spark producing activity shall only


be allowed under special circumstances after implementation of
additional precautions and in accordance with approved
procedures. An E&M safety document and a Hot Work Permit
shall be issued to control work of this nature. Reference shall be
made to section 3.5.3, OSH Rules for the safety requirements for
issuing a Hot Work Permit.

8.6.4.6 Prior to charging up with flammable gas:


a) appropriate tests shall be carried out to verify the integrity and
tightness of the flammable gas system, and

b) the plant shall be cleared of air by purging with nitrogen or


carbon dioxide.

8.6.5 Authorisation for work on natural gas

In addition to the requirements specified in Section 9, E&M safety


document for work on or testing of natural gas installations shall
be issued by a senior authorised person (natural gas) and received
by a competent person (natural gas), appointed in accordance with
approved procedures.

8.7 Work on Pressure Vessels

In addition to the requirements in sections 8.1, 8.2 and 8.3,


depending on the scope, work on pressure vessels and associated
pipework may be carried out on plant at different operation modes
under one of the following conditions:

8.7.1 Work with pressure vessels that are isolated and non-
operational

8.7.1.1 All external sources of energy for pressure and/or temperature


raising shall be isolated and secured with safety locks.

8.7.1.2 The content of the pressure vessel shall be de-pressurised, vented,


drained and purged as required for the nature of the work.

8.7.1.3 Vent and drain valves, where they could give rise to danger, shall
be secured with safety locks in the appropriate positions to prevent
back-feed or pressurisation from plant in operation.

8.7.1.4 For work inside pressure vessels and associated pipework, reference
shall be made to section 3.5.2, OSH Rules for the safety
requirements for entry to or work in a confined space.

8.7.1.5 Actuators of control valves shall, where practicable, be locked off


in the appropriate position.

8.7.2 Work with pressure vessels in an operational or partly


operational mode

8.7.2.1 Repair of leaks on external part of the plant, which does not give
rise to danger, shall be carried out in accordance with approved
procedures and under the terms of a Limited Work Certificate.

8.7.2.2 Work on associated fittings of pressure vessels which can be


isolated and depressurised, shall be carried out under the terms of a
Permit to Work.

8.7.2.3 Tightening of external header caps and manhole doors of internally


seal type may be carried out with the consent of a control engineer.

8.7.2.4 Floating of safety valves may be carried out without the issue of an
E&M safety document but the work shall be agreed with a
control engineer. The work shall be carried out in accordance with
approved procedures under the direct supervision of a competent
person who has been appointed to perform such duty.

8.7.3 Inspection, pressure testing and hydraulic testing on steam


generator

The following precautions shall be taken:

8.7.3.1 For access to gas and air passes, furnace and dead spaces of a steam
generator, reference shall be made to section 8.8;

8.7.3.2 For pressure testing and/or hydraulic testing of steam generators, all
persons shall be clear of the gas pass, furnace and dead space
during the pressure raising process;

8.7.3.3 Only those persons directly involved in the inspection or test shall
be allowed to enter the gas pass, furnace or dead spaces whilst any
steam generator is undergoing pressure testing and/or hydraulic test,
and

8.7.3.4 Compressed air shall not be used as a media for pressure testing on
pressure vessels. Pressure testing and/or hydraulic testing shall be
carried out in accordance with approved procedures.

8.8 Work on Boiler or Heat Recovery Steam Generator,


Combustion Chambers, Gas or Air Passes, and Dead Spaces

In addition to the requirements in sections 8.1, 8.2 and 8.3, the


following precautions shall also be taken.

8.8.1 Isolation

Before any person is allowed to gain access to the combustion


chamber, gas or air pass or dead space of a steam generator, a
Permit to Work shall be issued and the following precautions
shall be taken as appropriate:

8.8.1.1 For Boiler


a) All means of firing the boiler shall be isolated and secured with
safety locks.

b) All supplies, which enable the operation of sootblowers,


thermoprobes, and the movement of oil burners, and plant
associated with ash or dust removal shall be isolated and
secured with safety locks.

c) The steam or compressed air supplies to the sootblowers shall be


isolated and secured with safety locks.

d) All sources of draught to the boiler shall be isolated and secured


with safety locks, except with the permission of the senior
authorised person, such fans, if necessary, to assist in cooling
or ventilation may be kept in service.

e) All supplies which enable the movement of dampers and vanes,


with the exception of those required to control cooling and
ventilation, shall be isolated and the dampers fixed in an
appropriate position.

f) Where necessary to avoid danger, fan impellers shall be


physically prevented from rotating.

8.8.1.2 For heat recovery steam generator


a) All sources of fuel supply and ignition equipment for the
associated gas turbine shall be isolated and secured with safety
locks.

b) After the steam generator has been sufficiently cooled, all


sources of energy input to the prime mover shall be isolated and
secured with safety locks.

c) The steam generator shall be depressurised. All sources of


external steam supply to the unit shall be isolated and secured
with safety locks.

d) All supplies that enable the movement of dampers, with the


exception of those required to control cooling and ventilation of
the gas pass, shall be isolated and the dampers fixed in an
appropriate position.

8.8.2 Initial Access

8.8.2.1 The gas pass shall be regarded as a confined space. Reference


shall be made to section 3.5.2, OSH Rules for the safety
requirements for entry to or work in a confined space.

8.8.2.2 Precautions shall be taken to prevent the falling of clinkers.

8.8.3 Precautions on completion of work

8.8.3.1 Following the completion of work, the competent person in charge


of the work shall ensure that:
a) no person or equipment remains inside;

b) all persons are warned not to enter the gas and air passes, or
dead space after they have been withdrawn;

c) if it is necessary to make a final check, all access points are


locked closed except one. A guard is posted at the remaining
access point to ensure that no person re-enters during the check;

d) when re-entering for the final check, reference is made to section


3.5.2, OSH Rules for the safety requirements for entry to or
work in a confined space, and

e) after the final check, the competent person shall personally


supervise the closure of the final access.

8.8.3.2 Before the senior authorised person cancels the Permit to Work,
he shall ensure that all access points are securely closed. If any
access point is found open, or not securely closed, the senior
authorised person shall refer the matter to the competent person
in charge of the work who shall repeat the action as stated in
8.8.3.1.

8.9 Work on Electrostatic Precipitators

In addition to the requirements in sections 8.1, 8.2 and 8.3, the


following precautions shall also be taken.

8.9.1 Before access to the gas pass of a precipitator, the following


requirements shall be observed:

8.9.1.1 The electrical supply to all zones of electrodes shall be isolated and
secured with safety locks;

8.9.1.2 Circuit main earths shall be applied to the high voltage


apparatus of each zone at the point of isolation from the supply;

8.9.1.3 All means of firing the boiler shall be isolated and secured with
safety locks;

8.9.1.4 All sources of draught to the boiler shall be isolated and secured
with safety locks, except with the permission of the senior
authorised person, such fans, if necessary, to assist in cooling or
ventilation may be kept in service;

8.9.1.5 All sources of flue gas treatment shall be isolated and secured with
safety locks;

8.9.1.6 The power supply for the rapping gear and the air supply of the
sonic horn system shall be isolated and secured with safety locks;

8.9.1.7 Where work is to be done on or near a rapping gear of the 'tumbling


hammer' type, the competent person in charge of the work shall
ensure that all hammers are in a stable condition;

8.9.1.8 Portable earth leads shall be applied to the high voltage wire
electrodes of each zone at the points of entry into the precipitator
gas pass. These earths shall, where practicable, be applied from a
position outside the gas pass by an authorised person accompanied
by a competent person, in accordance with section 3.2.5;

8.9.1.9 Where access is required to the lower walkways, the dust hoppers
shall, where practicable, be cleared of dust by the normal
operational means, and

8.9.1.10 Before allowing access to and work in a gas pass, a Permit to


Work shall be issued and Register of Working Party Member
shall be completed in accordance with approved procedures.
Reference shall be made to section 3.5.2, OSH Rules for the safety
requirements for entry to or work in a confined space.

8.9.2 Precautions on completion of work in the gas pass of a


precipitator

8.9.2.1 The competent person in charge of the work shall warn the
working party not to enter the precipitator gas passes, or high
voltage enclosures and request them to sign out on the Register of
Working Party Members

8.9.2.2 Before the Permit to Work is cleared, the competent person in


charge of the work and of the closing of the access doors shall
ensure that no person or equipment remains inside. He shall also
ensure that:
a) if it is necessary to make a check, all access points are closed
and secured except one. A guard is posted at the remaining
access point to ensure that no person re-enters during the check,
and

b) when re-entering for the check, reference is made to section


3.5.2, OSH Rules for the safety requirements for entry to or
work in a confined space.

8.9.2.3 The senior authorised person shall make a final check that no
person remains inside. He shall then ensure that all access points
are closed and secured before he cancels the Permit to Work.

8.9.3 Work in the high voltage roof housings external to the gas pass

8.9.3.1 When work is to be carried out on the external parts of the gas pass
in an enclosure containing exposed isolated high voltage
conductors, or on other isolated apparatus any of which may
become live due to faults in their gas pass, then circuit main earths
shall be applied.

8.9.3.2 The circuit main earths shall be applied between the point of work
and the isolated zone to which the conductor or other isolated
apparatus is connected.

8.9.3.3 A Permit to Work shall be issued.

8.10 Work on Rotating Machinery

In addition to the requirements in sections 8.1, 8.2 and 8.3, the


following precautions shall also be taken:

8.10.1 All energy sources to the rotating machinery shall be isolated and
secured with safety locks;

8.10.2 The auxiliary system for facilitating rotation of the machine shall
be isolated and secured with safety locks;

8.10.3 The rotating machinery shall be secured at standstill position with


an appropriate braking device, where applicable. The braking
device, shall, where practicable, be secured with safety locks;

8.10.4 Relevant working parties shall be informed prior to manual


barring, and

8.10.5 All protective guards to the rotating parts shall be reinstated upon
completion of work.

8.11 Work on Hydrogen Filled Plant or Apparatus

In addition to the requirements in sections 8.1, 8.2 and 8.3, persons


shall not use any naked flame or welding equipment in or near
hydrogen filled plant or apparatus, or associated equipment,
which may contain hydrogen, unless the precautions stated in
sections 8.11.1, 8.11.2 and 8.11.3 have been taken.

8.11.1 Work on internal parts of hydrogen filled generators

8.11.1.1 Before work or testing commences which involves opening up of


any part of a generator casing the provisions of section 4.7 shall be
satisfied.

8.11.1.2 Work or testing which involves access to parts normally containing


hydrogen, the following shall also be observed:
a) The hydrogen supply shall be isolated by physical
disconnection from the hydrogen system;

b) The generator casing shall be cleared of hydrogen and


completely scavenged with carbon dioxide in accordance with
approved procedures;

c) The carbon dioxide in the casing shall then be replaced by air,


and

d) A Permit to Work shall be issued and reference shall be made


to section 3.5.2, OSH Rules for the safety requirements for entry
to or work in a confined space even after the removal of
generator rotor from the stator.

8.11.2 Work on external part of hydrogen filled generators


Before work or testing commences on the external parts of a
hydrogen filled generator and a risk of fire or explosion exists, the
hydrogen in the casing shall be replaced by carbon dioxide.

8.11.3 Work on other hydrogen filled plant or apparatus

Before work or testing commences on any other plant or


apparatus which may contain hydrogen, the following shall also
be observed:

8.11.3.1 The plant or apparatus shall be isolated from all sources of


hydrogen and the valves secured with safety locks;

8.11.3.2 Plant or apparatus which may contain hydrogen shall, where


practicable, be scavenged with carbon dioxide or nitrogen;

8.11.3.3 A Permit to Work shall be issued stating the precautions taken to


clear the plant or apparatus of hydrogen, and

8.11.3.4 When internal access is required, reference shall be made to section


3.5.2, OSH Rules for the safety requirements for entry to or work in
a confined space.

8.12 Work on Conveyors and Associated Plant and Apparatus

In addition to the requirements in sections 8.1, 8.2 and 8.3, the


following precautions shall also be taken.

8.12.1 General requirements

8.12.1.1 Maintenance and clearing of spillage or scraping of belts, involving


the removal of protective guards, shall be carried out while the
associated conveyor is not in motion except for tracking of belts as
stated in section 8.12.1.2.

8.12.1.2 Tracking of belts, which requires the restoration of power supplies


shall be carried out under the terms of a Permit to Work with
Restoration of Motive Power.

8.12.1.3 Riding on any conveyor belt is forbidden.

8.12.1.4 Stepping onto, or crossing a conveyor belt except by walkways, is


forbidden, unless the conveyor is isolated and a Permit to Work
has been issued.

8.12.1.5 Conveyors shall not be operated until:


a) all guards are in place;

b) all safety trip features are operable, and

c) all audible warning has been sounded throughout the conveyor


system before it is started.

8.12.2 Requirement for work on conveyor

When work is required on conveyor plant and apparatus, the


following requirements shall be observed:

8.12.2.1 The conveyor belt(s) and associated plant and apparatus on which
work is carried out shall be isolated, and, where practicable, be
secured with safety locks;

8.12.2.2 Other conveyor belts and/or equipment which normally feed into
the area of work shall either be isolated or prevented from feeding
into the area;

8.12.2.3 A Permit to Work shall be issued;

8.12.2.4 The work area shall be cleared of excessive spillage before work
commences;

8.12.2.5 The competent person in charge of the work shall ensure that
danger cannot arise from the release of stored energy. This may
require the belt to be clamped, and special arrangements made to
secure any gravity take-up pulleys and weights, and

8.12.2.6 The chutes shall be regarded as a confined space and reference


shall be made to section 3.5.2, OSH Rules for the safety
requirements for entry to or work in a confined space.

8.12.3 Clearing of spillage and removal of coal, dust or ash from idlers or
pulleys shall only be carried out with the conveyor belt not in
motion:

8.12.3.1 Cleaning work, where removal of protective guards is required,


shall be carried out under the terms of a Permit to Work.

8.12.3.2 Cleaning work, where removal of protective guards is not required,


shall be carried out as follows:
a) The trip wire switch shall be in the tripped position with work
in progress notices attached, and

b) The trip switches shall, where practicable, be secured with


safety locks.

8.13 Work on Overhead Travelling Cranes or Similar Equipment

In addition to the requirements in sections 8.1, 8.2 and 8.3, the


following precautions shall also be taken:

8.13.1 Getting on or off overhead travelling cranes

8.13.1.1 Persons shall obtain permission from the driver prior to getting on
or off operational overhead travelling cranes and similar machines.

8.13.1.2 The driver shall only operate or move the crane after the person has
got on or off the crane and has confirmed it with the driver.

8.13.1.3 Persons gaining access to a crane shall make sure the driver is fully
aware of their proposed activities and establish effective
communication with the driver so that both parties are alerted of
any danger.

8.13.2 Work on or in the immediate vicinity of exposed low voltage


overhead conductors

8.13.2.1 Where work is to be carried out on or in the vicinity of exposed low


voltage overhead conductors, which may give rise to danger, the
following shall be observed:
a) The electrical supply to the conductors shall be isolated and,
where practicable, be secured with safety locks;

b) A Limited Work Certificate shall be issued;

c) Before work commences on exposed overhead conductors, they


shall be proved dead in accordance with section 1.5 at the point
of work, and

d) The exposed conductors, including the neutral, shall be short-


circuited and, where reasonably practicable, be earthed with
approved equipment.

8.13.2.2 All persons who may have to work in the vicinity of exposed live
low voltage overhead conductors shall be aware of the danger of
handling long conducting objects.

8.13.3 Work on or near the wheeltracks of overhead travelling cranes


or similar machines

When persons are to work on or near the wheeltracks of overhead


travelling cranes or similar machines, the following precautions
shall be taken:

8.13.3.1 Where reasonably practicable, the machines shall be isolated and


a Permit to Work issued;

8.13.3.2 Where isolation of the machines is not reasonably practicable for


operational reasons, a Limited Work Certificate shall be issued by
an authorised person after:
a) the driver and the competent person have been warned that the
overhead conductors are live and the machine is operational,
and the warning is recorded on the Limited Work Certificate;

b) the driver at the time of issue has been warned that persons are
to work in a position where they could be struck by the machine
or its load and that the driver shall not let his machine approach
within 6 metres of such persons, and

c) in addition to the warning given to the driver, where practicable,


the 6 metres approach limit has been indicated by a warning
notice or other effective means.

8.13.3.3 The authorised person shall ensure that the conditions in sections
8.13.3.2 b) and 8.13.3.2 c) continue to be met, whilst the Limited
Work Certificate is in force, and

8.13.3.4 A copy of the Limited Work Certificate shall be displayed in the


cab of the machine.

8.13.4 Work in the vicinity of operational overhead travelling cranes


or similar machines

Persons working near operating overhead travelling cranes or


similar machines, are exposed to the danger of being struck by the
machines or their loads. This could be particularly harmful if the
persons are above floor level, and fall as a result of being struck.
Supervisors of all persons who work in such situations shall warn
their staff of this danger.

8.14 Work on Culverts

In addition to the requirements in sections 8.1, 8.2 and 8.3,


precautions shall be taken in accordance with approved procedures
when work is required in culverts. Reference shall also be made to
section 3.5.2, OSH Rules for entry to or work in a confined space.

8.14.1 Isolation

8.14.1.1 Plant associated with the culvert which could cause excessive
water flow shall be isolated and secured with safety locks;

8.14.1.2 The isolation of the culvert shall include any chemical injection
points into the system.

8.14.2 Draining and venting

8.14.2.1 Culverts shall, where reasonably practicable, be drained and


vented.

8.14.2.2 When draining, vent valves shall, where practicable, be opened in


order to draw clean air into the culvert.

8.14.2.3 Barriers and suitable notices shall be applied at all points of access
to prevent access during draining.

8.14.3 Precaution for diving work

If diving operations are necessary, the following requirements shall


be observed:

8.14.3.1 Diving operations shall be carried out under the terms of a Limited
Work Certificate;

8.14.3.2 Diving operations shall comply with the relevant statutory


regulations and any approved procedures, and

8.14.3.3 Prior to commencement of work:


a) method statement appropriate to the task shall be available;

b) assessment of risk shall be conducted with all mitigating

c) a second set of safety equipment used in parallel for the diving


work shall, where applicable, be available.

9.1.2 E&M safety document defines the plant or apparatus on which it


is safe to work or test. It also states the safety precautions to permit
such work or testing.

9.1.3 The E&M supplementary document is used in conjunction with an


appropriate E&M safety document.

9.1.4 E&M status document specifies the status of certain plant or


apparatus. It does not grant permission to carry out any work or
testing.

9.1.5 OSH safety document defines the safety precautions to be taken to


protect persons from general danger (non-power system danger)
as stated in the OSH Rules.

9.2 Safety Lock Keys and Key Safes

9.2.1 Appropriate safety lock keys shall be secured in key safe(s).

9.2.2 A lock having a key unique at the location shall be used to lock the
key safe(s). This key safe key shall be issued to the recipient of a
safety document, except where the key safe is at a remote location.

9.2.3 An operational person issuing a safety document shall at the


same time apply an additional lock to the key safe, except where
the key safe is at a remote location. The key to the additional lock
shall be in the possession of an authorised person.

9.2.4 Where the key safe is at a remote location, an authorised person


shall lock the key safe(s) and attach a tag to the key safe in
accordance with approved procedures. The key safe key(s) shall
be retained in the custody of an authorised person.

9.2.5 The key safe number(s) shall be recorded on the safety document.

9.2.6 The operational person, who receives the key safe key together
with the safety document, shall retain the key in his safe custody
until he returns the safety document.

9.3 Language and Terminology

9.3.1 All safety documents shall be written in Chinese and/or English


that being the language the recipient understands.

9.3.2 When required in safety documents, only approved abbreviations


shall be used.

9.4 E&M Safety Document

9.4.1 General Requirements

9.4.1.1 The issue and receipt of the E&M safety document shall be
carried out as follows:
a) The safety document shall be issued at the work site;

b) The authorised person or senior authorised person issuing the


safety document shall:

i) satisfy himself that the safety precautions stated on the safety


document to achieve safety from the power system are
adequate, and
ii) explain the contents of the safety document to the recipient.
c) When an authorised person or a senior authorised person
who signed the declaration of the safety document is not the
person to hand the safety document to the recipient, the safety
document shall be issued by another authorised person or
senior authorised person in accordance with approved
procedures;

d) The recipient shall:

i) read back the contents to the authorised person or senior


authorised person;
ii) confirm that he understands and is fully conversant with the
nature and extent of the work or testing to be done, and
iii) sign the receipt.
e) An authorised person or senior authorised person shall issue
the safety document together with, if applicable, the key safe
key(s), additional earths and other items as stated on the safety
document to the recipient.

f) The recipient shall ensure that all working party members


under his charge understand the content, nature and extent of the
work or testing, and the requirements imposed by the safety
document before allowing work to commence.

g) The recipient shall, where applicable, request all the working


party members, including new members joining the working
party in the course of work, to sign in on the Register of
Working Party Members in accordance with approved
procedures.

h) The recipient shall retain the original safety document in his


safe custody together with, if applicable, the key safe key(s),
additional earths and other items issued with, and listed on the
safety document during the course of work or testing.

i) The issuer shall retain a copy of the safety document when he


is not the recipient.

9.4.1.2 A Permit to Work, or a Limited Work Certificate which is not


for low voltage live work, is only allowed to be transferred. The
transfer shall be carried out as follows:
a) The recipient returning the safety document shall warn the
working party not to continue work on or testing of the plant or
apparatus and where applicable, request them to sign out on the
Register of Working Party Members.

b) The recipient returning the safety document shall sign the


Safety Document Transfer Record and return the safety
document, together with, if applicable, the key safe key(s),
additional earths and other items issued with, and listed on the
safety document to an operational person responsible for the
transfer.

c) The operational person who receives the returned safety


document shall check the items returned are the same as stated
on the safety document; he shall then sign the Safety
Document Transfer Record.

d) An operational person, who is responsible for the transfer of


the safety document to a new recipient, shall satisfy himself
that the safety precautions stated on the safety document to
achieve safety from the power system are adequate. If it is
adequate, he shall then sign the Safety Document Transfer
Record.

e) During transfer, the operational person responsible for the


transfer shall hand, if applicable, the key safe key(s), additional
earths and other items listed on the safety document to the new
recipient. The issue and receipt of the safety document shall be
carried out in accordance with section 9.4.1.1.

f) The operational person responsible for the transfer shall


transfer the same responsibilities to the new recipient of the
safety document as if the safety document was being issued
initially.

g) The new recipient shall sign the Safety Document Transfer


Record in the presence of the operational person responsible
for the transfer.

9.4.1.3 Recipient of the safety document unfit or unavailable to confirm


clearance

When a recipient of a safety document is unfit or unavailable for


confirming the clearance of the safety document, the safety
document shall be cleared by two senior authorised persons
assigned by a business unit manager of the operational unit and a
work team member who is involved in the work or test.

9.4.2 Limited Work Certificate

9.4.2.1 Preparation

An authorised person who issues a Limited Work Certificate


shall ensure that:
a) the safety precautions necessary for achieving safety for the
work or test have been taken and recorded on the Limited Work
Certificate;

b) where applicable, all keys to safety lock(s) used to secure points


of isolation have been placed and locked in the key safe(s);

c) where applicable, the key safe key(s) are handled in accordance


with section 9.2, and

d) if the work or testing under the terms of a Limited Work


Certificate is in the vicinity of plant or live apparatus, or
could affect the status of plant or apparatus in service, the
consent of a control engineer has been given and recorded on
the safety document.

9.4.2.2 Issue and receipt


a) A Limited Work Certificate shall be issued by an authorised
person, where applicable, with the consent of a control
engineer.

b) A Limited Work Certificate shall be received by a competent


person.

c) The issue and receipt of a Limited Work Certificate shall be


carried out in accordance with section 9.4.1.1.

9.4.2.3 Transfer
a) Any transfer shall be handled by an authorised person.

b) The authorised person responsible for the transfer of a Limited


Work Certificate shall inform a control engineer of the
transfer, if the Limited Work Certificate has been issued with
the consent of a control engineer.

c) The transfer shall be carried out in accordance with section


9.4.1.2.

9.4.2.4 Clearance and cancellation


a) A Limited Work Certificate shall be cleared by the recipient.

b) The recipient shall warn the working party not to continue


work on or testing of plant or apparatus, and where applicable,
request them to sign out on the Register of Working Party
Members.

c) The recipient shall check that the work site has been cleared of
all tools and equipment and shall specify any exceptions on the
Limited Work Certificate.

d) The recipient shall sign the clearance only after confirming that
all persons working under the Limited Work Certificate have
been withdrawn.

e) The Limited Work Certificate together with any keys issued


shall be returned to the authorised person.

f) A Limited Work Certificate shall be cancelled by an


authorised person, who shall notify a control engineer of the
cancellation if the Limited Work Certificate is issued with the
consent of a control engineer.

9.4.3 Permit to Work or Permit to Work with Restoration of Motive


Power

When a Permit to Work with Restoration of Motive Power is in


force, no other E&M safety document shall be in force on the
same items of plant or high voltage apparatus.

9.4.3.1 Preparation

A senior authorised person who issues a Permit to Work or


Permit to Work with Restoration of Motive Power shall ensure
that:
a) the precautions necessary for achieving safety from the power
system have been taken in accordance with sections 4.1.2 and
8.2 and are recorded on the Permit to Work or Permit to
Work with Restoration of Motive Power;

b) all keys to safety locks, used to secure points of isolation and


circuit main earths, and other keys necessary to achieve safety,
have been placed and locked in the key safe(s);

c) the key safe key(s) are handled in accordance with section 9.2;

d) the number of additional earths issued, where applicable, have


been recorded on the Permit to Work or Permit to Work with
Restoration of Motive Power, and

e) where applicable, a Customer Switchgear Isolation Certificate


with Part 2a has been duly completed or an Inter Control
Circuit Isolation Certificate has been issued.

9.4.3.2 Issue and receipt


a) A Permit to Work or Permit to Work with Restoration of
Motive Power shall be issued by a senior authorised person,
where applicable, with the consent of a control engineer.

b) A Permit to Work or Permit to Work with Restoration of


Motive Power shall be received by a competent person.

c) When a Permit to Work with Restoration of Motive Power is


issued, the senior authorised person shall also issue those keys
that allow the restoration of the motive power as stated on the
safety document to the recipient.

d) If the nature of the work makes it necessary for the work to be


carried out by, or under the direct supervision of, a senior
authorised person, the Permit to Work or Permit to Work
with Restoration of Motive Power recipient shall be the senior
authorised person who is in charge of the work.

e) The issue and receipt of the Permit to Work or Permit to


Work with Restoration of Motive Power shall be carried out
in accordance with section 9.4.1.1.

9.4.3.3 Transfer
a) Any transfer of Permit to Work shall be handled by a senior
authorised person.

b) The senior authorised person responsible for the transfer of a


Permit to Work shall inform a control engineer of the transfer.

c) The transfer shall be carried out in accordance with the section


9.4.1.2.

9.4.3.4 Clearance and cancellation


a) A Permit to Work or Permit to Work with Restoration of
Motive Power shall be cleared by the recipient.

b) The recipient shall warn the working party not to continue


work on or testing of the plant or apparatus and where
applicable, request them to sign out on the Register of Working
Party Members.

c) The recipient shall check that the work site has been cleared of
all tools and equipment and shall specify any exceptions on the
Permit to Work or Permit to Work with Restoration of
Motive Power.

d) The recipient shall sign the clearance only after confirming that
all persons working under the Permit to Work or Permit to
Work with Restoration of Motive Power have been
withdrawn.

e) The Permit to Work or Permit to Work with Restoration of


Motive Power together with any key safe key(s), additional
earths and any other items issued with, and listed on the safety
document and any other keys issued shall be returned to a
senior authorised person.

f) A Permit to Work or Permit to Work with Restoration of


Motive Power shall be cancelled by a senior authorised
person, where applicable, with the consent of a control
engineer.

g) The senior authorised person, who cancels the Permit to


Work or Permit to Work with Restoration of Motive Power
with the consent of a control engineer, shall inform the control
engineer of any restrictions imposed or the change(s) in the
operational status of the plant or apparatus concerned.

9.4.4 Sanction for Test

9.4.4.1 Requirements
a) A Sanction for Test shall be issued where:

i) the test voltage on the conductors of high voltage


apparatus exceeding extra low voltage, or
ii) circuit main earths may be removed during the test
irrespective of the magnitude of the test voltage.
b) When a Sanction for Test is in force, no other E&M safety
document and no E&M status document shall be in force
within the same isolated zone of the high voltage apparatus
except for:

i) a Customer Switchgear Isolation Certificate with Part 2b


duly completed, or
ii) a Safety Rules Implementation Certificate.
c) Following a test, the apparatus shall, where reasonably
practicable, be energised from the point of testing in accordance
with approved procedures.

9.4.4.2 Preparation

A Senior Authorised Person who issues a Sanction for Test shall


ensure that:
a) the precautions necessary for achieving safety from the power
system have been taken in accordance with section 4.1.2 and are
recorded on the Sanction for Test;

b) all keys to safety locks, used to secure points of isolation, and


other keys, which are not required for the test, have been placed
and locked in the key safe(s);

c) the key safe key(s) is handled in accordance with section 9.2;

d) the number of additional earths issued, where applicable, have


been recorded on a Sanction for Test, and

e) where applicable, a Customer Switchgear Isolation Certificate


with Part 2b has been duly completed.

9.4.4.3 Issue and receipt


a) A Sanction for Test shall be issued by a senior authorised
person, where applicable, with the consent of a control
engineer.

b) A Sanction for Test shall be received by an authorised person.

c) The issue and receipt of the Sanction for Test shall be carried
out in accordance with section 9.4.1.1.

d) In addition to items issued in section 9.4.1.1, the senior


authorised person shall issue to the recipient those keys that
allow operation of the apparatus within the isolated zone
necessary for the testing.

9.4.4.4 Testing
a) The recipient shall be responsible for coordinating all testing
operations within the isolated zone of the high voltage
apparatus on site. The testing operations may be carried out
without reference to a control engineer.

b) When the recipient is a senior authorised person, the testing


operations of the high voltage apparatus within the isolated
zone shall be carried out by:

i) the recipient accompanied by a competent person;


ii) a competent person under the direct supervision of the
recipient;
iii) an authorised person, who is instructed by the recipient,
accompanied by a competent person to carry out testing
operations at the remote ends, or
iv) a competent person under the direct supervision of an
authorised person who is at the remote ends and instructed
by the recipient.
c) The authorised person receiving instructions from the recipient,
as stated in section 9.4.4.4 b) iii) and 9.4.4.4 b) iv), shall
confirm the completion of the testing operations at the remote
ends to the recipient.

d) When the recipient is not a senior authorised person, the


testing operations of the high voltage apparatus within the
isolated zone shall be carried out by the recipient accompanied
by a competent person or by a competent person under the
direct supervision of the recipient.

e) The application of test voltage shall be carried out by the


recipient accompanied by a competent person or by a
competent person under the direct supervision of the recipient.

f) Work of minor nature essential to the completion of testing may


be carried out under the terms of a Sanction for Test. Such
work shall be carried out in accordance with approved
procedures.

g) Work of minor nature shall be carried out either by the recipient


or a competent person under the direct supervision of the
recipient.

h) The apparatus under test shall be discharged to earth before


application and removal of any test connections.

i) Where the spouts at the remote ends may be subject to test


voltage, the shutters of the spouts shall be locked shut.

j) Danger live notices shall be attached to any apparatus with


exposed live conductors, which may give rise to danger and are
in the vicinity of persons carrying out the testing. The
boundaries of the test area shall, where practicable, be
identified visibly.

k) Where danger may arise during the test, the apparatus, its
associated testing equipment, leads and connections, and the
remote ends of the apparatus with exposed live conductors that
could be made live by the test voltage shall be adequately
guarded by barriers and attached with danger live notices to
avoid danger.

9.4.4.5 Clearance and cancellation at the completion of testing


a) A Sanction for Test shall be cleared by the recipient.

b) The recipient shall ensure that all testing equipment, tools,


gears, plugs and leads that are applied after the issue of the
Sanction for Test, are removed.

c) If an earth and test truck is used in the test, it shall either be


removed or reverted to earth mode with the earthing switch
locked closed after the test.

d) The recipient shall warn the working party not to continue


testing of the apparatus and request them to sign out on the
Register of Working Party Members.

e) Other than long circuit:

i) The recipient shall verify the clearance at all points of


isolation except for the remote ends where no testing
operation has been carried out.
ii) The competent person who has accompanied the recipient
and participated in the testing operations, shall witness the
verification of clearance carried out by the recipient at all
points of isolation except for the remote ends where no
testing operation has been carried out.
f) For long circuit:

i) The recipient shall verify the clearance at all points of


isolation at the local end. The competent person who has
accompanied the recipient and participated in the testing
operations, shall witness the verification of clearance carried
out by the recipient.
ii) The recipient shall verify the clearance at the remote ends if
testing operations have been carried out at the remote ends
except in section 9.4.4.5 f) iii). The competent person who
has accompanied the recipient shall witness the verification
of clearance carried out by the recipient.

iii) The recipient is not required to verify the clearance at the


remote end if the authorised person, under the instructions
of the recipient, has carried out testing operations only on
the circuit main earths at the remote end. The authorised
person who has carried out the testing operations shall
verify the clearance at the remote end and confirm it to the
recipient.
iv) The competent person, who has accompanied the
authorised person at the remote end, shall witness the
verification of clearance carried out by the authorised
person and confirm it to the recipient if the testing
operations have been carried out only on the circuit main
earths.
g) The competent person who witnessed the verification of the
clearance as specified in section 9.4.4.5 e) ii) and 9.4.4.5 f) ii),
shall sign the clearance on the Sanction for Test.

h) The exceptions to the conditions within the isolated zone since


the time of issue of the Sanction for Test shall be specified on
the Sanction for Test.

i) The recipient shall sign the clearance on the Sanction for Test,
after he has verified the clearance at the points of isolation as
specified in section 9.4.4.5 e) and 9.4.4.5 f).

j) The Sanction for Test together with any key safe key(s),
additional earths and any other items issued with, and listed on
the safety document shall be returned to a senior authorised
person.

k) The Sanction for Test shall be cancelled by a senior


authorised person, where applicable, with the consent of a
control engineer.

l) The senior authorised person, who cancels the Sanction for


Test with the consent of a control engineer, shall inform the
control engineer of any restrictions imposed or the change(s) in
the operational status of the apparatus concerned.

9.4.5 Sanction for Test for Interconnection Circuit

9.4.5.1 This safety document shall be issued with the agreement of the
control engineers of the respective power systems for testing of an
inter-system circuit by either party.

9.4.5.2 When issuing the safety document, there shall be no Inter


Control Circuit Isolation Certificate in force on the circuit.

9.4.5.3 The requirements in sections 7.3 and 9.4.4 shall be observed.

9.4.6 High Voltage Live Line Work Certificate

A High Voltage Live Line Work Certificate shall be prepared,


declared, accepted, cleared and cancelled collectively by the
members of the high voltage live line work team.

9.4.6.1 Preparation

All necessary safety precautions shall be taken and recorded on the


High Voltage Live Line Work Certificate.

9.4.6.2 Declaration and acceptance


a) All members of the high voltage live line work team shall
understand the extent and requirements of the work and shall
ensure all necessary precautions have been taken before work
commences.

b) The high voltage live line work team shall collectively accept
their responsibility for the work specified on the High Voltage
Live Line Work Certificate.

c) One of the members in the high voltage live line work team
shall notify a control engineer of the completion of the
declaration and the acceptance. Work shall only commence after
consent is obtained from the control engineer.

9.4.6.3 Clearance and cancellation

A member of the high voltage live line work team shall notify a
control engineer of the cancellation of the High Voltage Live
Line Work Certificate.

9.4.7 E&M Supplementary Document

9.4.7.1 E&M Safety Document Transfer Record


a) The precautions stated on the safety document shall be
maintained before and after the transfer.

b) The Safety Document Transfer Record shall be attached to the


original safety document.

9.4.7.2 E&M Safety Document Supplementary Sheet

When E&M Safety Document Supplementary Sheet is used, it


shall be attached to the E&M safety document.

9.4.7.3 Register of Working Party Members


a) Working party members shall sign in and sign out on the
Register of Working Party Members when working under one
of the following E&M safety documents:

i) Permit to Work for high voltage apparatus;


ii) Permit to Work with Restoration of Motive Power;
iii) Sanction for Test, or
iv) Limited Work Certificate issued for work in the vicinity of
exposed live high voltage conductor.
b) The registration shall be carried out in accordance with
approved procedures.

9.5 E&M Status Document

9.5.1 Circuit State Certificate

9.5.1.1 The issuer shall notify a control engineer of the issue and
cancellation of the Circuit State Certificate.

9.5.1.2 The issuer shall hand the key safe key(s) with the Circuit State
Certificate to the customer’s authorised representative.

9.5.1.3 The issuer shall retain a copy of the Circuit State Certificate for
reference.

9.5.2 Customer Switchgear Isolation Certificate

9.5.2.1 When a Permit to Work is required, the customer’s authorised


representative shall complete Part 2a of the Customer Switchgear
Isolation Certificate.

9.5.2.2 When a Sanction for Test is required, the customer’s authorised


representative shall complete Part 2b of the Customer Switchgear
Isolation Certificate.

9.5.2.3 The recipient shall receive, where applicable, safety lock key(s) or
key safe key(s) together with the Customer Switchgear Isolation
Certificate.

9.5.2.4 A senior authorised person shall ensure all the associated E&M
safety documents have been cancelled before signing the clearance
of the Customer Switchgear Isolation Certificate.

9.5.3 Inter Control Circuit Isolation Certificate

9.5.3.1 This safety document shall be issued on an inter-system circuit


with the agreement of the control engineers of the respective
power systems.

9.5.3.2 When an Inter Control Circuit Isolation Certificate is in force,


any one party may issue Permit(s) to Work within the isolated
zone stated on the Inter Control Circuit Isolation Certificate
without reference to the other parties.

9.5.3.3 A control engineer shall ensure all the associated Permits to


Work have been cancelled before the cancellation of the Inter
Control Circuit Isolation Certificate.

9.5.4 Safety Rules Implementation Certificate

9.5.4.1 Requirements
Safety Rules Implementation Certificate shall be issued when
plant or high voltage apparatus, which is under construction and
will become part of the power system, reaches a stage whereby it
could be:

a) connected to the power system;

b) considered live by virtue of its proximity to or relation with,


other plant or apparatus, or

c) energised or pressurised from any other power source which


may give rise to danger.

9.5.4.2 Preparation
a) When construction work is required to be carried out on or near
the power system and such work will make the plant or high
voltage apparatus under construction reaching a stage stated in
section 9.5.4.1 a) or b) above, the work shall be carried out
under the terms of a Permit to Work, and in accordance with
the following:

i) Safety Rules Implementation Certificate shall, where


reasonably practicable, be effective before the issue of the
Permit to Work. The Permit to Work shall hence cover
work on the new and existing plant or apparatus.
ii) If it is not reasonably practicable to achieve section 9.4.5.2 a)
i) above, Safety Rules Implementation Certificate shall, be
effective before the cancellation of the Permit to Work,
which covers work on existing plant or apparatus only.
b) The following persons shall be informed regarding the issue of
the Safety Rules Implementation Certificate:

i) Those who are currently working without any E&M safety


document on or in the proximity of the plant or apparatus,
which may be exposed to danger from the power system,
and

ii) Where reasonably practicable, those who previously worked


without any E&M safety document and may return to re-
work on or in the proximity of the plant or apparatus, which
may be exposed to danger from the power system.
c) Safety Rules Implementation Certificate is not necessary
under the following conditions:

i) The entire construction work is covered by Permit(s) to


Work, or
ii) Supply of an isolated portion of the power system is
connected by an 11 kV mobile generator under the on-site
supervision of the senior authorised person in charge of the
connection in accordance with approved procedures.

9.5.4.3 Issue
a) The Safety Rules Implementation Certificate shall be issued
in accordance with approved procedures to notify managers of
relevant parties the effective date and time when the new plant
or apparatus will be subject to these Safety Rules.

b) The responsible manager or his delegate receiving the Safety


Rules Implementation Certificate shall sign on this safety
document to confirm that all relevant persons under their
control have been notified of the date and time of the
implementation of these Safety Rules to the plant or apparatus.

c) All the Safety Rules Implementation Certificates for the plant


or apparatus shall be signed and returned before it becomes
valid.

d) After the Safety Rules Implementation Certificate is effective,


any subsequent work on or testing of the plant or apparatus
shall then be carried out under the terms of E&M safety
document.

10. Training And Authorisation

10.1 Qualification Required for Operational Persons

A person shall only be appointed to work on the power system if


he:

10.1.1 possesses adequate technical knowledge and working experience in


the relevant field;

10.1.2 has satisfactorily completed the pre-requisite approved training;

10.1.3 where necessary, has passed an authorisation assessment in


accordance with approved procedures, and

10.1.4 satisfies any additional requirements applicable to the particular


category of authorisation.

10.2 Training of Operational Persons

10.2.1 Approved operations training courses shall be conducted by


qualified trainers.

10.2.2 New or revised operational safety related rules and procedures shall
be communicated to all relevant operational persons.

10.3 Authorisation of Operational Persons

The initial authorisation, re-certification, change of authorisation,


suspension of authorisation, termination of authorisation and
reinstatement of authorisation of operation persons, shall be in
accordance with approved procedures.

11. Responsibilities Of Persons

11.1 General

11.1.1 It is the duty of all persons who are required to operate, work or test
on, or in close vicinity of plant or apparatus, to comply with these
Safety Rules, applicable approved procedures and statutory
requirements.

11.1.2 A person who is required to work or test on or near plant or


apparatus under the terms of an E&M safety document shall only
start or restart the work or testing with the permission of the
recipient of the E&M safety document.

11.1.3 An operational person shall only carry out the authorised duties
on specific parts of the power system as defined in his
authorisation.

11.1.4 An operational person shall ensure his authorisation is valid


before carrying out the work or testing on or near the power
system.

11.1.5 The issuer of a safety document shall verify the validity of the
authorisation of the recipient before issuing the safety document to
the recipient.

11.2 Competent Persons (CP)

The roles and responsibilities of competent persons shall include


the following:

11.2.1 Understand the nature and limits of the work or testing to be carried
out.

11.2.2 Use safe methods of work, safe means of access and personal
protective equipment provided to ensure safety.

11.2.3 Beware of hazards that may arise during the work or testing and
take necessary precautions to avoid danger and warn other persons
if required.

11.2.4 Roles as a Working Party Leader

11.2.4.1 Recognise at all times whether it is safe to continue the work or


testing.

11.2.4.2 Instruct working party members to cease work or testing and


withdraw immediately in the case where a hazard arises. He shall
report the situation immediately to a responsible person.

11.2.5 Roles as the Recipient of an E&M Safety Document

11.2.5.1 Be a working party leader for work or testing under the terms of
the E&M safety document.

11.2.5.2 Be fully conversant with the contents and requirements of the E&M
safety document.

11.2.5.3 Explain the contents and requirements of the E&M safety


document to working party members under his charge and, where
applicable register them in accordance with approved procedures.

11.2.5.4 Adhere to, and instruct working party members under his charge
to adhere to, any conditions, instructions or limits specified on the
E&M safety document during the course of the work.

11.2.5.5 Provide on-site supervision of the working party while work or


testing is in progress.

11.2.5.6 Warn all working party members to cease work or testing, if on-
site supervision is not available.

11.2.5.7 Provide direct supervision for work or testing as determined by the


issuer of the E&M safety document. While the work or testing
under his direct supervision is in progress, he shall, at his
discretion, suspend the work or testing of all other E&M safety
documents issued to him depending on the level of supervision
required.

11.2.5.8 Keep in safe custody the E&M safety document, all relevant keys
and equipment issued.

11.2.5.9 Warn all working party members not to continue work or testing
on or near the plant or apparatus concerned before clearing the
E&M safety document.

11.2.5.10 Receive, transfer and clear E&M safety document in accordance


with section 9.4.

11.3 Authorised Persons (AP)

11.3.1 The roles and responsibilities of authorised persons specified in


this section do not apply to authorised persons (HVLLW),
authorised persons (HCLLW), whose roles and responsibilities
are defined in sections 6.2 and 6.3, and approved procedures.

11.3.2 In addition to the roles and responsibilities of competent persons,


authorised persons shall have some or all of the following roles
and responsibilities within the limits stated on their certificate of
authorisation:

11.3.2.1 When preparing the plant or apparatus safe for work:


a) Carry out high voltage switching or earthing operation in
accordance with section 3.1 and 3.2, and

b) Ensure that the limits of safe working areas are clearly marked
and achieved with necessary precautions taken, if necessary.

11.3.2.2 Carry out testing operations within the isolated zone of the
apparatus under the terms of a Sanction for Test in accordance
with section 9.4.4 when acting as the recipient of a Sanction for
Test;

11.3.2.3 Issue, receive, transfer, clear and/or cancel appropriate E&M safety
documents in accordance with section 9.4.

11.4 Senior Authorised Persons (SAP)

In addition to the roles and responsibilities of authorised persons,


senior authorised persons shall have some or all of the following
roles and responsibilities within the limits stated on their certificate
of authorisation:

11.4.1 Demarcate and establish the working area of apparatus and its
access inside the enclosure with exposed live high voltage
conductor for work or testing in accordance with section 4.9.2.1.

11.4.2 Implement these Safety Rules to ensure that all safety precautions
essential for making the plant or apparatus safe for work or testing
have been completed, these include:

11.4.2.1 Confirming that the plant or apparatus is correctly identified and


the safe working area clearly demarcated.

11.4.2.2 Confirming through a control engineer that safety precautions at


all locations are completed.

11.4.3 Prior to issue an E&M safety document or E&M status


document:

11.4.3.1 Determine whether additional earths or additional precautions are


required;

11.4.3.2 Determine whether direct supervision is required;

11.4.3.3 Ensure that the contents of the safety document to be issued are
correct and unambiguous, and

11.4.3.4 Ensure that safety from the power system has been achieved or will
be achieved when the requirements of the safety document are
fully implemented.

11.4.4 Issue, receive, transfer, clear and/or cancel appropriate E&M safety
documents and E&M status documents in accordance with
section 9.4 and 9.5.

11.5 Control Engineers (CE)

Control engineers shall have some or all of the following roles and
responsibilities within the limits stated on their certificate of
authorisation:

11.5.1 Consent to the release of plant or apparatus from service, its


restoration from outage and the connection to the power system.

11.5.2 Instruct authorised persons to carry out high voltage switching.

11.5.3 Check with senior authorised persons to confirm that the safety
precautions which have been taken are adequate for the work or
testing to be carried out.

11.5.4 Consent to the issue and cancellation of E&M safety documents


and E&M status documents, where appropriate.

11.5.5 Consult with control engineer(s) of other power system(s) to


agree, initiate and record those actions necessary to establish and
maintain safety precautions on plant or apparatus, which is inter-
system apparatus.

11.5.6 Where appropriate, maintain a record of high voltage switching,


application and removal of circuit main earths, E&M safety
documents and E&M status documents.
