Invalidation of Identification Cards
In line with the Department Memorandum No. 2023-0238 dated June 26, 2023
entitled “Photo Session and Issuance of the New Department of Health Identification Card”,
please be informed that
all identification cards (ID) with the old layout/design and IDs issued
to the following projects/programs and those whose contract of service are not under the
Department of
Health are no longer valid effective April 1, 2025.

Administrative Officers and AO Designates with jurisdiction and signatories of the ID
application forms of the aforementioned projects and programs are hereby instructed to
collect and surrender all IDs to the Personnel Administration Division, Administrative
Service (PAD, AS) on or before April 1, 2025. To enter the DOH compound, the
projects/programs and others affected by this issuance may request a gate pass from the
General Services Division, Administrative Service.
Furthermore, employees having the old ID layout/design may coordinate the
replacement of
their ID to Mr. Ryan B. Dordas of PAD, AS
at
local 4205S.
