ABSTRACT
This capstone project, entitled “Barangay Bulalacao Health Center
Database Management System,” aimed to design, develop, and implement a
comprehensive Database Management System (DBMS) for the Barangay
Bulalacao Health Center. The health center relied on manual methods for
managing patient records, consultation logbooks, and generating reports, resulting
in inefficiencies and data inconsistencies.
To address these challenges, this project employed a descriptive research
design and utilized MySQL as the database management system. The DBMS
featured user-friendly interfaces for data entry, retrieval, and reporting. It
incorporated data validation and security measures to ensure data integrity and
confidentiality.
The expected outcome of this project was to improve the health center's
data management capabilities, enhance patient care, and increase operational
efficiency. Specifically, the DBMS enabled the health center to efficiently manage
patient records, consultation logbooks, and generate accurate reports. This, in turn,
facilitated better decision-making, improved healthcare services, and enhanced
patient satisfaction.
Ultimately, this project had the potential to serve as a model for rural
health centers, promoting better healthcare services and management. By
providing a reliable and efficient DBMS, this project aimed to contribute to the
improvement of healthcare delivery in the community.

Chapter 1
Introduction
This chapter provides relevant information about the research study by outlining
the proposed system, its significance, and its benefits. This study, “Barangay Bulalacao
Health Center Database Management System,” aimed to enhance the manual process of
recording and storing data and documents at the Barangay Bulalacao Health Center.
Background of the Study
According to Ms. Saikie L. Rollo, Registered Midwife, and Mrs. Appie, Secretary
of Barangay Bulalacao Health Center, the proponents interviewed them about the
problems their office frequently encountered. Secretary Appie explained that the health
center handled tasks manually, such as calculating ages, managing redundant resident
information, tracking medicine inventory, and storing medical information for each
resident. They used manual methods for keeping and storing data, often placing it in
envelopes and then storing it in cabinets, which led to disorganization and aggravation.
The researchers created a system to help the Bulalacao Health Center easily store,
update, add, and manage resident data. The current manual processes were inefficient,
prone to errors, and lacked security. This study aimed to enhance data storage, automate
processes, improve accuracy, and provide better accessibility for residents.
The Barangay Bulalacao Health Center used a manual process of inputting data,
which involved handwriting and encoding in Microsoft Word. The manual process of
storing papers involved placing them in envelopes and storing them in cabinets or using

flash drives, which led to many errors, such as misplaced papers, data redundancy, and
aggravation—not only for the Barangay Health officials but also for the residents. These
processes also caused delays and dissatisfaction due to the manual recording of data. The
manual retrieval of records was a time-consuming process that resulted in errors and
misplaced documents.
The researchers developed a Database Management System to help Barangay
Health officials easily store, input, and organize data. This study supported services such
as resident registration and document printing. The system also helped the health center
improve the efficiency, accuracy, and accessibility of resident records and medical
histories, benefiting both the health center and the residents. A Health Center Database
Management System was a modern tool for healthcare facilities, enabling efficient
management of patient information, operational data, and various other aspects of the
center’s operations.
The researchers developed a system entitled “Barangay Bulalacao Health Center
Database Management System.” It was a system that helped the employees of Barangay
Bulalacao store data more effectively, organize database files, and provide quick
transactions and document printing for end users. This system benefited not only the
employees of Barangay Bulalacao but also all the residents of the Barangay.
The Database Management System (DBMS) for Barangay Bulalacao Health
Center had the potential to modernize operations, enhance service delivery, and promote
community development through the effective use of technology and streamlined

processes. It was also a significant step toward improving healthcare delivery and
enhancing resident care within the community.
Objectives of the Study
The researchers aimed to develop an effective and efficient database management
system for record keeping and eliminating data redundancy.
1. To design and develop a centralized database system that securely stores and
manages patient records, consultant logbooks, and health services for Barangay
Bulalacao.
2. To provide a system that can easily update, retrieve, add, and record information
for the Barangay Bulalacao Health Center.
3. To automatically generate reports such as patient statistics and the health senior
panel.
Significance of the Study
This study, entitled “Barangay Bulalacao Health Center Database Management System,”
was intended and proved beneficial to the following:
To the Barangay Bulalacao Health Center: It helped improve the processing of
documents, enhanced record-keeping, reduced redundancy, and overall increased
efficiency.
To Health Center Employees: It improved the efficiency and effectiveness of
managing barangay records. This study addressed issues related to data
accessibility, record-keeping accuracy, and transparency. It enhanced service
delivery to the RHU every quarter and promoted better resource allocation.

To the residents of Barangay Bulalacao: It helped ensure that their information
was secure, preventing redundancy or the misplacement of documents.
To the researchers: This study provided insight into how digitizing the manual
processes at Barangay Bulalacao Health Center could improve the community.
The researchers developed their writing, analytical, and research skills while
creating a thesis that could contribute to improving the community as a whole.
To future researchers: This study benefited others who wished to conduct similar
research. The results served as background information for conducting their own
studies.
Scope and Delimitation of the study
Scope
This study focused on creating a Database Management System for Barangay
Bulalacao Health Center. This system was useful and beneficial to the Barangay Health
Officials and Bulalacao residents by providing users with the ability to access, verify,
update, and track resident profiles/records and medical histories. This study focused only
on inputting data related to prenatal care, children and adolescents, family planning,
infants, and the inventory of medicines, for better storage, report printing, and data
retrieval or restoration. If this study was approved by the panel, the researchers would
have also offered this system to other Barangay Health Centers.

Delimitation
This system is designed only for Barangay Bulalacao premises. The system is
offline, and cannot access online, and the residents must register personally at the
Barangay Bulalacao Health Center. This system needs an electricity, it will work only
with the support of electricity. This system would can be using computer desktop and
cannot applicable to mobile phones.
Definition of Terms
The following terms are defined operationally as used in this study:
Barangay Health Workers (BHW): Refers to a person who works at the
barangay health center and assists the midwife. These individuals were selected as target
respondents for the study.
Bulalacao Health Center: Refers to a place where the residents of Barangay
Bulalacao go when they need assistance. It provides services such as immunization for
infants, family planning for women, prenatal care for pregnant women, and also stores
data related to age, medicines, and other health information.
Children and Adolescents: Refers to data on age, specifically identifying whether
individuals belong to the infant, child, or adolescent categories. This information is stored
in the system by the midwife or administrator.

Data: Refers to information that is collected, stored, and processed to extract
insights.
Database: Refers to a collection of data or storage designed to allow efficient
retrieval and manipulation. A database can be thought of as an electronic filing system
that allows users to store, update, and retrieve data as needed.
Database Management System (DBMS): Refers to a system responsible for
adding, retrieving, and updating data, with significant usability in the proposed study.
Family Planning: Refers to a method used by women to prevent pregnancy,
control the number of children, and space out pregnancies. The information provided by
women at the health center is stored in the system by the midwife or administrator.
Gravidity: Refers to the number of times a woman has been pregnant.
Infants: Refers to children aged 0-12 months who require monthly immunization
to maintain a healthy and strong body. The information about infants is stored in the
system by the midwife or administrator.
Manual Process: Refers to a series of tasks performed by Barangay Health
Workers without the use of automation or machines.
Midwife: Refers to a trained healthcare professional who provides prenatal, labor,
birth, and postpartum care to women. The midwife also assists Barangay Health Workers
with determining which medicines residents need and can use the provided system.

Modernized Technology: Refers to current technology that can replace manual
processes, making tasks quicker and less aggravating, especially when managing data and
information.
Offline: Refers to a state where a device, system, or application is not connected
to the internet, meaning it is not online.
Parity: Refers to the number of times a woman has given birth to a fetus.
Record: Refers to a collection of data or information stored in a structured format,
often in a database, file, or document. Records can be physical (e.g., paper documents) or
digital (e.g., electronic files).
Researchers: Refers to individuals or a group who systematically investigate and
analyze data, concepts, and phenomena to advance knowledge and solve or improve
specific problems.
Residents: Refers to individuals who have concerns and are assisted and
interviewed by Barangay Health Workers.
System: Refers to a set of principles or procedures by which something is done—
an organized scheme or method.
User: Refers to an individual who interacts with a system to achieve a specific
goal or task.
