Digital Subscriber Line (DSL)


DSL technology leverages the existing copper infrastructure, but allows digital signals to
be transmitted instead of analog signals. It enables the full bandwidth of twisted copper
telephone cables to be utilized. With splitter-based services, the DSL signal is extracted
from the phone line as it enters your premises and is wired separately to a DSL modem.
This involves additional hardware and installation by the service provider at the customer 5
site. The shielded option does not require installation, but the telephone company's
equipment and some of your equipment may need to be upgraded.
With Asymmetric Digital Subscriber Line (ADSL), most of the duplex bandwidth is
devoted to the downstream direction, with only a small proportion of bandwidth available
for upstream. Much Internet traffic through the client's connection, such as web browsing, 10
downloads, and video streaming, requires high downstream bandwidth, while user requests
and responses are less significant and therefore require less upstream bandwidth.
Additionally, a small proportion of the downstream bandwidth can be allocated for voice
instead of data, allowing you to make phone calls without needing a separate line.
DSL-based services are a very low-cost option compared to other solutions offering similar 15
bandwidth, making them available to customers at extremely competitive prices.

The one system in each room consists of 5 networked PCs in total, one in the Practice
Manager's office and the other in Reception alongside the file server. (Each PC has its own
laser printer.) There is also a dot-matrix printer in Reception for prescriptions, as these are
printed on special paper. All users have access to Microsoft Office.

Doctors use the system primarily to access and hold the records of all patients in the 5
practice. These files contain personal details and the medical history of the patients. The
doctor can call up the appointment book prior to consultation. By clicking on the patient's
name, they have immediate access to that patient's records. At the end of each consultation,
the doctor enters brief case notes including the diagnosis and treatment. This database can
also be used to produce statistics for research and reports. 10

Doctors can also access a drug database on CD-ROM, which provides prescribing
information on thousands of drugs, including their suitability for different categories of
patients. This is updated every month. Another database is a conditions dictionary, which
provides information on a wide range of problems.

Reception staff use specialized software developed from a database tailored to enter all
appointment dates and times for each doctor. The program generates daily lists of 15
appointments and can be accessed by the doctors. Reception uses the patient database to
identify children and elderly people who are due for vaccinations. They then use mail
merging to create letters requesting appointments to be made.

The Practice Manager uses a package based on a spreadsheet to calculate salaries for each 20
employee of the health center. She enters all income and expenditure to produce practice
accounts. She also uses a database to produce a monthly rota of which doctors are on call
in the evenings and on weekends. This rota is available over the network to all users.
