1. Scope

This document stipulated the mandatory requirements applicable to the TIEJUN Taiba-1 IPP
CCGTP Project and its Contractors. TIEJUN and its Contractors shall ensure that all
requirements listed here are fully understood, implemented, always complied with, and always
monitored.

3. Responsibility

3.1 Project Director

It is TIEJUN's sole duty to establish a proper and functional HSE Department with proper
resources for the TAIBA-1 IPP Project. The project director has the overall responsibility for HSE
on the TAIBA-1 project.

He is responsible to:

 Appoint an HSE director and HSE Manager.


 Ensure the HSE Department has enough resources to perform their duties regarding
manpower and material perspective.
 Actively involve and lead HSE issues of the project.
 Actively support all audits/inspections.
 Execute disciplinary actions against violators.
 Participate in incident investigations.
 Participate in weekly & monthly HSE meetings.
 Lead and assist in accident/incident investigations to ensure all accidents and incidents
are properly investigated including near-miss incidents, first aid cases, all recordable
cases, chemical spillage incidents, property damage, etc.
 Participate in work-site design reviews to ensure adequate work areas, traffic control,
parking areas, lighting levels, receiving areas, etc. Assures that the location of offices,
shops, maintenance areas, fabrication shops, and medical, and sanitation facilities

reflect safety considerations.

3.2 HSE Director

The HSE Director assumes the lead HSE position on the project and shall be responsible for
monitoring and administering a pro-active project HSE program designed to assist in
recognizing, evaluating, and subsequently eliminating or controlling hazardous acts or
conditions.

3.1. HSE Department Manager

The HSE manager assumes the lead safety position on the project and is responsible for
monitoring and administering a proactive project safety program designed to assist in
recognizing, evaluating, and subsequently eliminating or controlling hazardous acts or
conditions. He works in close coordination with the ACWA HSE Manager and in conjunction with
his project manager and construction manager assisting in the implementation of the safety
program.

The HSE manager shall:


i. Develop a comprehensive safety program and recommend its implementation.
ii. Administering appropriate safe work practices/procedures for the project.
iii. Promoting a high level of safety awareness on the project through new
employee/Contractor/ Subcontractor orientation and contact with project supervision.
iv. Conducting Monthly Safety Self-Assessments, tracking performance, and reporting
trends to site management.
v. Conducting quarterly safety audits.
vi. Maintaining all safety-related records and files associated with the project.
vii. Administer the first aid / medical program and maintain facility and supplies.
viii. Maintain pertinent information (i.e., phone number, locations) of Emergency Response
Services, physicians, and hospitals.
ix. Lead and assist in accident/incident investigations to ensure all accidents and incidents
are properly investigated including near-miss incidents, first aid cases, all recordable
cases, chemical spillage incidents, property damage, etc.
x. Complete and distribute applicable reports.
xi. Evaluating Contractor/Subcontractor safety systems before work activities
xii. Chair the weekly meeting with safety supervisors.
xiii. Initiate Job Bulletins concerning safety issues.
xiv. Continuously evaluating project working conditions and safe work practices, and if
warranted, developing positive recommendations for project management.
xv. Monitoring the behavior of the people and arranging training for employees to change
their attitude and behavior positively towards working safely.
xvi. Participate in work-site design reviews to ensure adequate work areas, traffic control,
parking areas, lighting levels, receiving areas, etc. Assures that the location of offices,
shops, maintenance areas, fabrication shops, and medical, and sanitation facilities
reflect safety considerations.

xvii. Review the work schedules as they are planned to be aware of the number of
Contractors and craft workers working in the various areas; anticipate hazards and
implement supporting safety activities.
xviii. Establish personal protective equipment requirements and recommend purchase
requisition.
xix. Plan the safety publicity program and order posters, visual aids, signs, etc.
xx. Establish and maintain site, Emergency Response Team.

3.2. HSE Engineer/Supervisor

 Assist the HSE Management in the performance of his / her duties.

 Conduct comprehensive HSE inspections of the workplace and monitor day-to-day work
in progress.

 Maintain a working relationship with site supervision/foreman.

 Engage with general workers and promote a positive HSE culture on site.

 Assist the HSE Management in the preparation and delivery of HSE training.

3.3. Security and Fire Supervisor/personnel

 Ensure the Security procedure was fully implemented.

 Ensure the Fire Prevention and Protection Procedure is fully implemented, check the
daily hot work permits, conduct monthly checks on the availability and suitability of fire
extinguishing systems.

 Arrange the training for fire personnel and security guards.

 Take immediate action for the fire and security incident according to the emergency
plan.

 The management for ID and vehicle gate passes and check the daily material&
equipment gate passes.

3.4. Doctor

As head of the Medical Facility, he must perform his duties with the highest degree of
competence and professionalism. He must be able to deal efficiently and diligently with
employees of varied nationalities, cultures, and backgrounds. Listed are his duties and
responsibilities. It is understood that the physician may be asked to perform other duties and
responsibilities as determined by the Project Director or the HSE Manager.

 Conducts medical and physical examinations of employees and writes the


necessary recommendations.

 Attends to outpatients performs the necessary examinations and prescribes the


corresponding treatment.

 Dispenses medications to patients.

 Attends First Aid Cases and or Surgical/Emergencies and evaluates and prescribes
the necessary treatment or medications.

 Makes the necessary hospital referral for cases that to his judgment cannot be
treated at the clinic. Before any hospital referral shall be made, the Physician
shall coordinate with the Project HSE Manager or his designated representative.
The Physician shall fill out a Hospital Referral Form of which a copy shall be
provided to the Project HSE Manager.

 He shall maintain a patient’s logbook indicating the patients seen during the day.
A Daily Illness Record Log shall be provided. The Physician must maintain an
individual patient’s file for record purposes which shall serve as an aid for further
follow-up treatment. Relevant history shall be written in the patient files, i.e.,
history of allergies to food and medications and present medications he is taking,
etc.

 He shall conduct periodic inspections of the job site and its premises regarding
the health and hygiene standards of the site.

 He shall conduct periodic health inspections at the camp’s mess facilities.

 He shall conduct periodic health education programs and attend a medical safety
meeting in coordination with the Project HSE Manager if the need arises when
there is an increase in the number of specific injury cases or illnesses.

 He shall keep and maintain a statistical record of the number of cases attended to
or treated in a certain month.

 He shall conduct a periodic emergency medical evacuation drill to evaluate the


preparedness of contractor medical staff and foreman/supervisors in dealing with
an injured employee.

 He shall perform liaison work to the Health Ministry for health/medical-related


matters.

 He shall make the necessary or relevant recommendations required for the safe
performance of work tasks on Site.

 The Physician shall have the authority to issue a 24-hour bed rest/no work status
when necessary. The patient must report back to the clinic the following day for
further evaluation.

3.5. Nurse

The Nurse is an active participant in the promotion and implementation of the health care and
injury treatment program on the Project. He shall competently perform the following duties
and responsibilities and must be willing to perform other duties and responsibilities asked for
by the Project HSE Manager.

a) He shall assist the physician in medical examinations and administer prescribed treatment
to a patient and or the injured.
b) He shall assist the physician in the performance of clinic-related activities.
c) He shall maintain a record of all cases attended to.
d) He shall keep an inventory of medicines dispensed and all equipment and supplies of the
Medical Facility.
e) He shall ensure that instruments are autoclaved/sterilized in a proper manner. Instruments
shall be disinfected or soaked in a prescribed soaking solution before use.
f) He shall see to it that needles/blades shall be one time used and shall be disposed of
accordingly in a designated sharp-only receptacle.
g) The nurse shall see to it that sterile gloves shall be used in all invasive or open cases when
blood or body fluids shall meet any part of the body.
h) All patients kept in the observation room shall be closely monitored.
i) The Nurse shall accompany all patients (who do not need the assistance of the Doctor)
referred to the Hospital. He shall inform the Physician of the condition of the patient while
at the Hospital.
j) The Nurse shall not apply stitches or sutures to a wound.
k) In the absence of the Doctor, the Nurse shall address cases limited only to the dispensing
of analgesics/antipyretics, and anti-diarrhea medications. All other cases shall then be
referred to the hospital for further treatment and management in the absence of the
physician.

3.6. Environmental & Social Engineer (ES)

The ES shall be suitable, qualified, and experienced in environmental issues relating to


construction activity. The ES will report to the EE on environmental issues in the ESMP and
assist the EE in the maintenance and continual improvement of the ESMP. The responsibilities
of the ES include but are not limited to the following:
a) Understanding environmental aspects and procedures relevant to activities under his/her
supervision.
b) Maintain incident register and complaints register.
c) Maintain the training register and attendance form.
d) Maintain records of the environmental inspections and audits; and
e) Monitor compliance of Project activities to the ESMP requirements, report non-compliances
to the EE and provide recommendations for remedial actions.

3.7. Sub-contractor HSE manager/ supervisor

The Sub-contractor safety representative is responsible for implementing the Contractor’s


safety program on the project. He is responsible for monitoring and administering a pro-active
project safety program in accordance with the project safety program designed to provide
assistance in recognizing, evaluating, and subsequently eliminating or controlling hazardous
acts or conditions. He works in conjunction with his project/site manager, Sub-contractor
construction manager and site safety representative assisting in the implementation of the
HSE programs.
