Sedation due to Medetomidine &
Dexmedetomidine
● Dexmedetomidine is a selective alpha2-adrenergic agonist, meaning it primarily
stimulates alpha2 receptors in the brainstem, which leads to sedation and
analgesia.
● It reduces central sympathetic outflow, leading to a decrease in heart rate, blood
pressure, and other sympathetic nervous system activity.
● It also has anxiolytic (anxiety-reducing) and analgesic effects.
How does Atipamezole Work?
● It is an Alpha Adrenergic receptor antagonist and competes with
Dexmedetomidine/ Medetomidine for the same receptor causing a reversal of
the symptoms of CNS Depression.
● It reverses analgesic effect, Respiratory depression and hypotension
● It does not entirely reverse the effects of bradycardia that occur due to
Dexmed/Med
INDICATIONS
● Rapid reversal of Sedation and Anesthesia - It counteracts the effects of Medetomidine and
Dexmedetomidine
● Allows for Faster recovery times, reducing the risk of prolonged sedation and anesthesia
related complications.
● Helps reduce side effects like Hypotension and respiratory depression, but does not entirely
reverse the effects of bradycardia.
● For emergency use in case of overdose or excessive sedation.
● Facilitates Outpatient Procedures - Allows for procedures that require short sedation times.
ReWoke allows patients to recover faster and be sent home quicker.
Contraindications and Precautions
➔ ReWoke can cause abrupt reversal of sedation. Therefore dogs that have received the medication
recently should be handled with care. There is a potential for apprehensive or aggressive behaviour of
the dog and hence should be considered when handling the patient.
➔ Sedation Relapse: While is effectively reverses the effects of the sedative, reversal to pretreatment
status may not be quick and patient should be monitored for sedation relapse. This however is majorly
seen in pets that have received dexmed through IV route.
➔ Analgesic reversal: ReWoke reverses analgesic effect of Dexmed/Medetomidine and therefore
additional pain management measure are to be taken
➔ Debilitated animals: Should not be used in animals that show signs of debilitation as may cause
collapse and even death.
➔ Should not be used in pets with pre existing cardiovascular issues.
➔ Breeding/Pregnancy and Lactating safety: Has not been evaluated for use in animals used for
breeding. And hence should no be used.
➔ Age and Weight: Has not been evaluated for use in pets less than four months of age and weighing
below 2kgs
ADVERSE REACTIONS
- Occasional Vomiting may occur
- At times a period of Apprehensiveness and Excitement will be seen in pets treated
with Atipamezole
- Other effects include Diarrhea , Hypersalivation and and Tremors
Dosage
Has to be Given IM irrespective of the route of Dexmed/Medetomidine was administered.
