AFFIDAVIT OF DISCREPANCY
I, GABRIEL KARLO EDMALIN, of legal age, single, and with
address at B16 Lot 6 Banahaw St., Heneral Dos Pasong Kawayan II,
Gen. Trias, Cavite, under oath, state:

1. THAT my name is GABRIEL KARLO EDMALIN as stated and


registered in my birth certificate issued by office of the Civil
Registry of ______________________. A copy of my birth
certificate is hereto attached as ANNEX “A”;

2. THAT the name GABRIEL KARLO EDMALIN also appears in


my passport which was issued based on the data appearing in
my birth certificate. A copy of the page of my passport where
my name and picture appears is hereto attached as ANNEX
“B”;

3. THAT the name GABRIEL KARLO CANEDA, have been use in


my school records;

4. THAT the name GABRIEL KARLO EDMALIN and GABRIEL


KARLO CANEDA refer to one and the same person, the herein
Affiant;

5. THAT I am executing this Affidavit to attest to the truth of the


foregoing facts and to use the same for whatever legal
purpose it may serve.

stated and registered in my birth certificate. A copy of my birth
certificate is hereto attached as ANNEX “A”;

7. THAT the name MARIMELL ANNE GUIMOC VERZOSA also


appears in my national identification card which was issued
based on the data appearing in my birth certificate. A copy of
the national ID where my name and picture appears is hereto
attached as ANNEX “B”;

8. THAT the name MARINEL ANNE GUIMOC VERZOSA, have


been use in my school records;

9. THAT the name MARIMELL ANNE GUIMOC


VERZOSA and MARINEL ANNE GUIMOC VERZOSA refer to
