INTRODUCTION
A nursing organization, like any other organization, viewed as a structure that provides a
framework of duties and responsibilities of nursing personnel. It is a group of nurses working
together at different levels as a team to accomplish a single common objective of nursing
services. It is a process or a system where nurses at different levels carry out many activities
by utilizing various resources to ensure the best possible client care.

Hospital nursing organization is like a pyramid, broad at the base and tapering toward the top.
The organization follows a line of hierarchy. Based on the hierarchy, their position and
relative responsibilities, there are three categories or level of nursing personnel:
administrative, supervisory and operational level.
Administrative level
In a hospital setting, nursing personnel at the top or administrative level are concerned with
formulating and executing policies, goals, objectives and procedures for nursing services.
They plan and co-ordinate various executive and managerial activities. They give instructions
to nurses at supervisory and operational levels. Every teaching hospital with 150 bed strength
should have one nursing superintendent assisted by one deputy nursing superintendent and
two assistant nursing superintendents.
Supervisory level
The nursing personnel at the supervisory level concern with middle management of nursing
services. They are the heads of either department or nursing unit or ward.
Operational levels
Operational level management comprises nurses at the operational category or first line
managers designated as staff nurses or sister grade II. They work in different wards, nursing
units, operation theatres, OPDs and other units, such as blood bank, infection control and
injection room. The job responsibilities vary according to the nature of work.
Community Health Nursing
There is no formal hierarchy of nursing organization in a community setting. Various
categories of nursing personnel working at district, community health centres, family welfare,
subcentre and periphery level.
At district level, District Public Health Nursing Officer (DPHNO) is responsible for
supervising activities related to health and family welfare services.
At PHC and CHC level, Public Health Nurse, female or male supervisor are responsible for
the health and public Health programme issues.
At subcentre level, ANM/health worker female is responsible 3000-5000 rural population for
their health-related preventive and promotive services.
At village level. Under NRHM, ASHA in the ratio of 1:1000 population supervise the
authorised village area and in Urban area U- ASHA covers 1000-2500 population and
visit 200-500 household.

1. Chief nursing officer does plan, coordination, supervision, controlling, reporting to higher
medical officer & delegating the work schedules to other nurses & allocate the
required staff at the right place & time. Explains the job description, supervision and delegating
responsibilities to each staff nurse.
2. Provides Nursing Services for patients according to policies of Hospital and the
nursing services department.
3. Determines the Philosophy, objectives and policies of the nursing department in
accordance with those of the hospital.

in planned manner depending on the suitability and qualification as well as the need of the

Nursing Services Administration
• Responsible for adequate and qualitative nursing care of the patients receiving treatment
in the indoor or the out-door.
• Interview and recruits nursing staff and other staff whose duties are related to nursing.
• Prepares annual statistics and projects manpower needed for patient care.
• Attending the correspondence related to nursing service from outside agencies
and individuals.
• Investigates complaints, prepares reports and takes disciplinary action or recommends
the same.
• Conducts supervisory rounds of wards and departments every day and ensures that all
serious patients are looked after by the supervisory staff.
• Prepares proposals for special equipment’s required for nursing services, giving
specification and participates in purchase committee meeting as a member.

Nursing supervisor is accountable for the nursing care management of a ward or a unit

1. Direct patient care:
• Ensures proper admission, discharge of her patients.
• Plans nursing care makes patient’s assignment as per their nursing needs.
• Assists in the direct care of the patient as and when required.

responsible for planning, organizing and supervising all the community health’s of the
district. She also participates in policy making activities, and evaluates the performance of
community staff.
The Qualifications
• University degree in nursing.
• Experience in community nursing is highly desirable.
• Strong problem-solving skills.
• Expertise in specialties such as mental health and geriatrics are important assets.

Job Summery (Roles)
Public Health Nurse is a midwife. She provides preventive and promotive srvices, monitors
and supervises the work of health supervisors and ANMs at the field.
Duties and responsibilities:
Main functions are:
1. Managerial and administrative activities
2. Supervision and guidance related activities
3. Health counselling and teaching
4. Environmental sanitation related activities
5. Subcentre action plan related
6. Field and other activities
To fulfil these responsibilities their activities are:
• To Helps in organization of maternal and child health programme.
• To promote health and nutrition education activities through the lady health
visitors, auxiliary nurse midwives.
• To help in developing school health programme in the district.
• To ensure regular supply of equipment’s for MCH work.
• To help the statistical officer in the district family welfare bureau in compiling
the periodical progress report of MCH activities.
• To review the periodical progress report on MCH/FP work done by the LHVs/ANMs,
female multipurpose workers, etc .and put up to the district medical officer/district
family welfare officer the points requiring attention and further action.
• To provide for continuing education regarding family care, family planning in the district
through short in-service training sources.
• Provides health care to all individual, families and community in any field that she
is appointed.
• Trains, supervises and guides health workers in the delivery of health care services to
the community.
• Plans health and family welfare programmes among with health worker.
• Carries out supervisory health visits in the area covered by the health worker.
• Coordinate the work of the male and female health worker and the personnel.
• Assists in conducting medical examination of the school children.
Health Assistant/Supervisor

Job Summery (Roles)
Health assistant act as a supervisor for four to six subcentres and its health worker. She is
posted at PHC level and responsible for MCH related activities and maintenance of proper
records and reports in subcentre and send them weekly or monthly at CHC or PHC level. She
also conducted training for dais and ASHAs with the help of ANM.
Responsibilities
Main function of health assistant females are;
1. Supervision and guidance of ANM and others health workers.
2. Coordinating activities of health worker and conducts regular meeting with them.
3. Maintain records and reports of authorized subcentre.
4. Checks equipment, supplies and maintenance of subcentre related activities.
5. Conduct training program for ASHA and dais and assist MO in organizing
training program.
6. Involves in MCH and FP related activities and camo.
7. Involves in Universal immunization program, nutritional program, school health
program, communicable and non-communicable program.
Auxiliary Nurse Midwife (ANMs)/ Health worker (Female):

Job summery
Auxiliary Nurse Midwife is the village level health worker. She is working in 3000-5000
rural population. She performs various activities related to national program, including
MCH, FP, Medical termination of pregnancy, nutrition, immunization, communicable and
non- communicable diseases. She also acts as a facilitator of ASHA workers. Same rural
ANM, urban ANM also working in urban slum area under NURM.
Duties and responsibilities:
Maternal and child health
Registers all pregnant cases, ensures at least four visits by them, first visit within
12 weeks.
Tests urine and blood and identifies high risk pregnancy and provides care for them.
Distribute iron and folic acid tablets to the pregnant mothers.
Refers to BPHC for institutional delivery to prevent complication.
Identifies beneficiaries under Janani Suraksha Yojana.
Provides antenatal and postnatal and new-born care in subcentre and community
level provides treatment in diarrhoea and ARI and refers when required.
Teaching mothers regarding MCH care, FP, nutrition and immunization,
breast feeding.
Maintain maternity records, register of antenatal cases, eligible couple register,
children register up to date.
FP and medical termination of pregnancy
Arrange and help M.O.in conducting MCH and FP clinics at the sub-center.
Distributes contraceptives to the couple and insertion and removal of intra
uterine device as a family planning method.
Identify and refers to women for requiring MTP and discourage unsafe
abortion methods.
Activities related health programs
She involved in various national health related program and try to full fil the national
targeted. Different type of national programme which can be goal oriented or eliminated
program or eradicated program, she involved with its full potentialities. Such as-
Universal Immunization program, Kala azar eradication program, malaria elimination
program.
Nutrition
Assesses children under five for malnutrition and identify low birth weight cases.
Distribution of iron and folic acid to under-five children, adolescent girl and
pregnant women.
Arrange village health nutrition day in community level.
Communicable and non-communicable disease
Notify MO regarding any case of diarrhoea, fever or any type of
communicable diseases.
Provides treatment with medicine and prophylaxis and refers if necessary and follow
up care.
Perform special duties as per guidelines in endemic areas of filaria, dengu, JE or kala-
azar.
Maintain record and register in subcenter level.
Uses information, education and communication methods to impart health
education and make them aware of prevention, healthy lifestyle and early detection
of communicable and non-communicable disease.
Prepare survey list of all identified cases and conducting screening camp.
Records and reports
Maintain all documents, including birth and death register, eligible couple register, pregnant
women register, under 5 register, family planning and others.
Coordinating and team activities
Attend various meeting at PHC and CHC level.
Co ordinate with ASHA and plan for organize health days.
Organize house to house survey.
Participants in health camp and campaigns.
ASHA (Rural)
ROLES & RESPONSIBILITIES
ASHA will be a health activist in the community who will create awareness on health and its
social determinants and mobilize the community towards local health planning and increased
utilization and accountability of the existing health services. She would be a promoter of
good health practices. She will also provide a minimum package of curative care as
appropriate and feasible for that level and make timely referrals. Her roles and
responsibilities would be as follows:
• ASHA will take steps to create awareness and provide information to the community
on determinants of health such as nutrition, basic sanitation & hygienic practices, healthy
living and working conditions, information on existing health services and the need for
timely utilization of health & family welfare services.
• She will counsel women on birth preparedness, importance of safe delivery,
breastfeeding and complementary feeding, immunization, contraception and prevention of
common infections including Reproductive Tract Infection/Sexually Transmitted Infection
(RTIs/STIs) and care of the young child.
• ASHA will mobilize the community and facilitate them in accessing health and health
related services available at the village/sub-center/primary health centers, such as
Immunization, Ante Natal Check-up (ANC), Post Natal Check-up (PNC), ICDS,
sanitation and other services being provided by the government.
• She will work with the Village Health & Sanitation Committee of the Gram
Panchayat to develop a comprehensive village health plan.
• She will arrange escort/accompany pregnant women & children requiring
treatment/ admission to the nearest pre-identified health facility i.e. Primary Health
Centre/ Community Health Centre/ First Referral Unit (PHC/CHC /FRU).
• ASHA will provide primary medical care for minor ailments such as diarrhoea,
fevers, and first aid for minor injuries. She will be a provider of Directly Observed
Treatment Short-course (DOTS) under Revised National Tuberculosis Control
Programme.
• She will also act as a depot holder for essential provisions being made available to
every habitation like Oral Rehydration Therapy (ORS), Iron Folic Acid Tablet (IFA),
chloroquine, Disposable Delivery Kits (DDK), Oral Pills & Condoms, etc. A Drug Kit
will be provided to each ASHA. Contents of the kit will be based on the
recommendations of the expert/technical advisory group set up by the Government of
India.
• Her role as a provider can be enhanced subsequently. States can explore the
possibility of graded training to her for providing newborn care and management of a
range of common ailments particularly childhood illnesses.
• She will inform about the births and deaths in her village and any unusual health
problems/disease outbreaks in the community to the Sub-Centres/Primary Health
Centre.
• She will promote construction of household toilets under Total Sanitation Campaign.
• Fulfilment of all these roles by ASHA is envisaged through continuous training
and upgradation of her skills, spread over two years or more.
Anganwadi worker/AWW
Job title: Anganwadi worker

Job Summery (Roles)
The AWW is a health worker, chosen from the community and have to go 4 months of
training of health, nutrition, and childcare and an AWW covering 1000 rural population.
Role and responsibilities of AWWs
To elicit community support and participation in running the programme.
To weigh each child every month, record the weight graphically on the
growth card, use referral card for referring cases of mothers/children to
the sub-centres/PHC etc., and maintain child cards for children below 6
years and produce these cards before visiting medical and para-medical
personnel.
To carry out a quick survey of all the families, especially mothers and children in
those families in their respective area of work once in a year.
To organise non-formal pre-school activities in the anganwadi of
children in the age group 3-6 years of age and to help in designing and
making of toys and play equipment of indigenous origin for use in
anganwadi.
To organise supplementary nutrition feeding for children (0-6 years) and expectant
and nursing mothers by planning the menu based on locally available food and local
recipes.
To provide health and nutrition education and counseling on breastfeeding/ Infant &
young feeding practices to mothers. Anganwadi Workers, being close to the local
community, can motivate married women to adopt family planning/birth control
measure.
AWWs shall share the information relating to births that took place
during the month with the Panchayat Secretary/Gram Sabha
Sewak/ANM whoever has been notified as Registrar/Sub Registrar
of Births & Deaths in her village.
To make home visits for educating parents to enable mothers to plan an
effective role in the child's growth and development with special emphasis
on new born child.
To maintain files and records as prescribed.
To assist the PHC staff in the implementation of health component of the
programme viz. immunisation, health check-up, ante natal and post natal
check etc.
To assist ANM in the administration of IFA and Vitamin A by keeping
stock of the two medicines in the Centre without maintaining stock
register as it would add to her administrative work which would effect
her main functions under the Scheme.
To share information collected under ICDS Scheme with the ANM.
However, ANM will not solely rely upon the information obtained
from the records of AWW.
To bring to the notice of the Supervisors/ CDPO any development in the
village which requires their attention and intervention, particularly in
regard to the work of the coordinating arrangements with different
departments.
To maintain liaison with other institutions (Mahila Mandals) and
involve lady school teachers and girls of the primary/middle schools in
the village which have relevance to her functions.
To guide Accredited Social Health Activists (ASHA) engaged under
National Rural Health Mission in the delivery of health care services
and maintenance of records under the ICDS Scheme.
To assist in implementation of Kishori Shakti Yojana (KSY) and
motivate and educate the adolescent girls and their parents and
community in general by organzing social awareness programmes/
campaigns etc.
AWW would also assist in implementation of Nutrition Programme
for Adolescent Girls (NPAG) as per the guidelines of the Scheme and
maintain such record as prescribed under the NPAG.
Anganwadi Worker can function as depot holder for RCH Kit/
contraceptives and disposable delivery kits. However, actual
distribution of delivery kits or administration of drugs, other than OTC
(Over the Counter) drugs would actually be carried out by the ANM or
ASHA as
decided by the Ministry of Health & Family Welfare.
To identify the disability among children during her home visits and
refer the case immediately to the nearest PHC or District Disability
Rehabilitation Centre.
To support in organizing Pulse Polio Immunization (PPI) drives.
To inform the ANM in case of emergency cases like diahorrea,
cholera etc.
Above different cattgories of nursing personnel in hospital and community level there
are others categories of nursing personals according to their duties and
responsibilities. Such as—

CONCLUSION:
Nursing and non-nursing personals in hospitals and field level plays an important role in
patient care and improve the standard of care. All of them provides their own responsibilities
according to their grade and helps to preventive, promotive and curative care in community.
These are maintaining a chain system to continuing departmental work smoothly and
successfully.
