Reading this book can be as straightforward as beginning at here


and working your way to the end. There are, however, several ways
to navigate these pages. Imagine that the entire book is a whole
human being and through the contents page you can find specific
areas and aspects of that person. This differentiation of the whole
person is not meant to imply that a person can be divided and
treated as isolated parts. Quite the contrary: That the person must
be looked at as a whole is the central argument of the book.
Humans are collections of many elements that can be affected in
many ways. Learning about the elements is meant to foster a better
understanding of the whole.
Most sections contain a special Clinical Notes box. We appreciate
that information without practical application can lose meaning and
lose interest, so we hope that these boxes add clarity, as well as
insight into what can be created with the information in the main
body of the text. We also encourage you to make your own clinical
notes that you can slide between the pages as you develop your
own thoughts and ideas about the text.
Finally, we have written this book as a conduit that might connect
us to you the reader both abstractly, through the words on the page,
and also literally, by stimulating conversations at conferences, or

Foreword

Einstein noted that “science without religion is lame and religion


without science is blind” (Knowles, 1999, p. 290). We can twist that
analogy to “psychotherapy without science can be a presumptuous
ritual with questionable benefit.” Theory without sufficient backing of
research to demonstrate its efficiency can be countertherapeutic. Yet
despite these concerns, the 20th century saw the rise and fall of
more than 100 types of different psychotherapies. Almost like mini-
religions and cults with esoteric terminologies, they are often based
entirely on concepts that make sense only within the context of the
particular theory. They become walled-off silos with an infrastructure
of presumed competency and proficiency, defined by the jargon of
their theoretical schools. Many of these psychotherapies have
achieved so much popularity that they are known simply by their
initials. They have built economic franchises with their own
publications, dues for membership, certifications, and conferences
where devotees give esoteric lectures understood only by those with
higher levels of acculturation within those schools. Many are led by a
guru, a person with a branded name. A noted cognitive behavioral
psychologist joked that one of the largest psychotherapy
conferences in the world was not really about the evolution in the
field, but “so that you can see them before they die.”
The pop-psychotherapy marketplace even infiltrated the graduate
schools. One applicant, during her interview for a postdoc position,
informed me that, were she selected, her goal for the year would be
to find her “theoretical home.” In response, I suggested that the
21st century necessitates that we find common denominators
between the different schools of psychotherapy based on science
and discard the extraneous theoretical debris. This comprehensive
book does an excellent job of just that. The common denominators
explored throughout the following chapters illustrate research that is
bio–psycho–social in nature. This integrative perspective subsumes
the relevant contributions of the past and discards the purely theory-
driven cul-de-sacs that are based only on the tight confines of a
particular school.
Without a common language backed by research, clients of
therapists are put at a disadvantage. Our societies have sanctioned
us as experts in mental health, and we attempt to prove our
expertise with framed diplomas decorating our offices and letters
following our names. Even though therapists are generally well-
meaning and compassionate, they are marinated in different
theoretical schools. Clients are often confused to hear completely
varying perspectives about their problems from two different
therapists.
To resolve these problems, the authors of this book have for
many years offered a website entitled The Science of Psychotherapy.
There they have provided numerous articles, interviews, and
seminars on cutting-edge research in the field. Fortunately, they
have followed up those efforts with this book.
In bringing science to psychotherapy, we need to ensure that it is
real science and not a mere veneer. Sure, conference attendance
numbers and book sales go up when the latest brain terms are used.
But we need to be careful to avoid reductionism when talking about
specific brain regions, such as the currently in vogue vagus or the
ever-popular right hemisphere. Yes, the right hemisphere is
important, but only in balance with the left. For example, consistent
research has shown that overactivity of the right hemisphere is
associated with avoidance and withdrawal—accordingly, anxiety and
depression.

of immunology, metabolism, and epigenetics developed in the last
20–30 years. We can combine these advances with evidenced-based
practices and outcome management tools to individually tailor the
psychotherapeutic techniques that have been shown to engage
particular systems throughout the body. We have learned a lot over
the years about how we can calm overactivated areas while
increasing activation in areas that need to be boosted. Essentially,
we can teach our clients how to tune up their brains. By normalizing
psychological disorders and their resolutions, we can explain and
delineate road maps for recovery, guiding clients away from the pain
that confuses and overwhelms them.
It has long been shown that from birth to death, the quality of
our relationships profoundly affects not only how we view the world
but also how our brains organize. Psychodynamic theorists were
right all along that attachment styles are carried over into the
therapy room. And so are the perspectives that reflect our
embeddedness within particular socioeconomic and cultural contexts.
It is therefore no wonder that psychotherapy research has for
decades stressed the importance of the “alliance,” “attunement,”
“transference”—whatever terminology was used. This book astutely
reviews that research but goes beyond the wall of the well-
established truisms detailed in a plethora of articles.
The sea change in psychotherapy was prophesied long ago.
Before he embarked on theory without a brain-based perspective,
Sigmund Freud said, “We must recollect that all of our provisional
ideas in psychology will presumably one day be based on an organic
substructure.” The father of American psychology, William James,
speculated that “the act of will activates neural circuits” (Arden,
2015, p. viii). Roughly a century later, we have the science to
support these prophesies. Nobel laureate Roger Sperry argued that,
“‘Mental forces’ could direct electrochemical traffic between neurons
at the cellular level” (Schwartz & Begley, p. 42). And now we know
that our mental activity can influence our immune system and gene
expression, through the work of another Nobel laureate, Eric Kandel,
who said, “Psychotherapy works by producing changes in gene
expression that alter the strength of synaptic connections” (Kandel,
1998, p. 460). In other words, causation is multidirectional at all
levels, even down to gene expression and metabolism.
The science of psychotherapy has now cross-pollinated with the
fields of metabolism, psychoneuroimmunology, and epigenetics.
Similarly, the critical importance of diet, exercise, and sleep for
mental health has been demonstrated in the fields of nutritional
neuroscience, exercise physiology, and sleep medicine. Genes can be
either expressed or suppressed by many factors, such as diet,
exercise, and sleep, and can lead to or away from depression.
Psychosocial factors are of equal importance for mental health,
including lack of social support, as well as traumatic interpersonal
events such as intimate partner abuse, add to the risk of genetic
vulnerability for anxiety and depression.
A few decades ago, the phrase “cells that fire together wire
together” became part of our language. When helping people with
anxiety and/or depression, we can also make relevant the phrase
“cells that fire out of sync lose their link.” With repeated firing, the
brain rewires to develop a habit. This means that prior depressive
episodes increase the potential of becoming depressed again, the
severity of the depression, and the risk for more depression. Prior
suicide attempts increase the risk of not only subsequent suicide
attempts but also more episodes of severe depression.
When we reduce our focus to one factor of mental health, such
as neurons firing, we lose focus on how it interacts with a wide
range of other factors. For example, when we focus on one aspect,
such as attachment, self-esteem, metabolic energy, genes, neurons,
or some other important life-sustaining system, we miss the big
picture: our overall health.
The science of psychotherapy in the 21st century accounts for
how all these systems work together to maintain mental health. We
need both the big picture and the small one to find the feedback
loops between all the systems to potentiate health and well-being.
Reductionism without the contextual big picture cannot explain how
to help people psychotherapeutically, and without some reductionism
we cannot identify the “parts.” Furthermore, without the contextual
consideration of the whole it would be difficult to understand how all
the parts are connected. To account for how the parts contribute to
the whole, we must address how complex systems, such as people,
are organized and change.
Previous attempts to apply holistic perspectives to psychotherapy
were limited by 20th century science. They failed to account for the
interactions among the factors (parts) that work together to support
the whole system. Initially, many “holistic” theories avoided the
exploration of how genes, neurons, and the immune system are
involved in the emergence of the self. Fortunately, 21st century
interdisciplinary science helps explain how all the factors in complex
systems interact to create an individual self: you or me.
A variety of efforts have attempted to explain how the whole is
greater than the sum of its parts. They represent the early
precursors to the multidisciplinary field now referred to as the
“science of complexity” (Mitchell, 2011). Despite the passing of more
than a half century since Austrian biologist Ludwig von Bertalanffy’s
book General System Theory was published, the term “wholeness”
remained associated with broad platitudes.
I am so pleased that Richard and Matt rely on the perspective of
the interdisciplinary science of complexity, which is close to my
heart, and the Santa Fe Institute, which functions as a center of
activity in this field in the town where I live. The science of
complexity explores how multiple systems work together to produce
phenomena far greater than can be produced by any of them alone.
This integrative perspective promotes common ground as we make
sense of how the system self organizes and the systems from which
the mind emerges, all made possible through energy.
The study of complex systems considers the collective, the
system-wide behaviors, to be of fundamental importance. As an
alternative to reductionism, complexity theory attempts to explain
systems in terms of their constituent parts and the interactions
between them. This approach investigates how the relationships
between a system’s parts give rise to its collective behaviors, which
interact to form relationships with its environment. This book brings
together the multidisciplinary sciences that account for how systems
work together to produce more than each discipline can individually.
No stone is left uncovered as it addresses the emerging research in
metabolism, genomics, neuroscience, psychology, and
psychoneuroimmunology.
Using the lens of the science of complexity, we can make sense
of how the metabolic, immune, genomic, and nervous systems are
interdependent parts of the “self.” Harmonizing the patterns of
activity among these life-sustaining systems contributes to our
mental health. The systems that contribute to self-organization are
hierarchical, in that they are embedded in one another. Our
hierarchical systems range from genome, to organelles in cells, to
cells, to organs, to body-wide systems. They all work together,
interdependently, through feedback loops. The highest in the
hierarchy emerges from the organization of the mental operating
networks, which generate states of mind, which collectively we call
the mind.
The concept of self-organization is central to the science of
complex systems, so it is quite relevant to psychotherapy. Self-
organization describes how order arises from local interactions
between parts that were initially disordered. The process can be
spontaneous, as occurs when sufficient energy is channeled by
changes in self-care. The components of complex systems interact in
such a way that the self-care activities cannot be summed up to help
us understand how the entire system self-organizes. Given that we
are constantly adapting to our respective environments, our self-
organizing systems are constantly reorganizing through feedback
loops.
As complex systems, we do not simply revert to a steady state as
conceptualized by homeostasis. Yet for centuries, health was based
solely on this concept, and during the last century psychotherapy
followed along. However, homeostatic mechanisms provide only the
primary means of stability, but complex systems such as humans are
dynamic because they employ positive feedback processes. Indeed,
given that we are living creatures and not machines, homeostatic
mechanisms cannot offer the sole means of maintaining health and
keeping us thriving. The degree to which homeostasis can regulate
our health during changing demands is limited. Whereas
homeostatic stability is regulated by a fixed set point and associated
negative feedback loops, we need to explain how our physiological
systems help adapt and change during changing conditions.
To address the limitations of homeostasis as the principal
mechanism that maintains our health, a model referred to as
allostasis, meaning in Greek “stability through change,” emerged in
the research on stress (Sterling, 2004). I am delighted to see this
concept addressed prominently in this book (Chapter 4). Allostasis
offers positive feedback adjustments of physiological mediators to
deal with episodic and brief challenges to promote health. Our
physiological resources mobilize when our brain anticipates needs,
and then regulates the rest of our body to meet those needs.
Both homeostatic and allostatic parameters for each life-
sustaining system widen as we move up the hierarchy. Yet at lower
levels, homeostatic parameters are more prevalent. For example, the
mitochondria maintain a much smaller operating parameter in terms
of how energy is produced than does the immune system. The
immune system contains mitochondria, without which it would not
function. The brain, which operates at a higher hierarchical position,
contains both immune cells and mitochondria in all brain cells. If the
metabolic and immune cells chronically exceed their parameters,
mood and neurocognitive disorders result.
Psychological disorders, ill health, and ultimately death result
from disorganization of the interplay between the homeostatic and
allostatic feedback loops within and between life-sustaining systems.
The disorganization within a system cannot be isolated; it spreads to
other systems. In response to trauma, disorganization can spread
relatively quickly and can result in abrupt destabilizing shifts in self-
organization. As neurons disconnect from neuronal networks, the
immune system is overactivated or the metabolism becomes
sluggish. The risk of widespread disorganization increases. When
organ systems disorganize, the result is serious illness or, in the
extreme, death. On the other hand, the greater the self-organizing
connectivity, the greater the potential for health. The connectivity is
driven by the basic biological energy underlying metabolism, and
without a stable supply of energy that connectivity is lost. Our job as
