WAIVER AND RELEASE OF LIABILITY

I, the undersigned participant, hereby acknowledge that I am voluntarily
participating in the above-mentioned Fun Run activity organized by the
National Youth Commission (NYC), in partnership with Intersections, Inc., and
other event partners (collectively referred to as the “Organizers”).
I understand that participation in this event involves physical activity that may
pose certain risks, including but not limited to injury, illness, dehydration,
accidents, or other unforeseen circumstances. I voluntarily assume all such risks,
known and unknown, even those arising from the negligence of the Organizers
or others.
In consideration of being allowed to participate in this event, I hereby:
Release, waive, and discharge the National Youth Commission, Intersections,
Inc., their officers, employees, representatives, sponsors, and volunteers from any
and all liability, claims, demands, actions, or causes of action arising out of or
related to any loss, damage, or injury (including death) that may be sustained
by me, whether caused by the negligence of the Organizers or otherwise.
Agree to indemnify and hold harmless the Organizers, their officers, employees,
and volunteers from any claims, damages, liabilities, or expenses that may arise
from my participation in the event.
Acknowledge that I am physically fit, have sufficiently trained for participation,
and have not been advised otherwise by a qualified medical professional.
Consent to receive medical treatment deemed necessary during the event and
understand that I will be responsible for any medical or related expenses.
Authorize the Organizers to use photographs, video recordings, and other
media containing my likeness for documentation, publication, and promotional
purposes, without any form of compensation.
Data Privacy Notice and Consent
In compliance with Republic Act No. 10173, otherwise known as the Data
Privacy Act of 2012, I hereby give my consent to the National Youth Commission
(NYC), Intersections, Inc., and their authorized representatives to collect, record,
organize, update, and process my personal information, including but not
limited to my name, contact details, age, and images, solely for purposes
related to my participation in this Fun Run event.
I understand that my personal data will be handled with strict confidentiality
and stored securely, and that I have the right to access, correct, or withdraw my
consent in accordance with the law.
I have carefully read this Waiver and Release of Liability, fully understand its
contents, and sign it voluntarily. I understand that by signing this document, I am
giving up certain legal rights, including the right to sue the Organizers.
