Dentigerous Cyst

Dentigerous cysts are benign odontogenic cysts associated with the crowns of unerupted permanent teeth. They develop between the reduced enamel epithelium and the tooth crown due to pressure from the erupting tooth obstructing venous flow around the tooth follicle. Clinically, they present as painless swellings that can displace adjacent teeth. Imaging shows well-defined radiolucent lesions attached to the neck of an unerupted tooth. Dentigerous cysts most commonly involve the mandibular third molar or maxillary third molar.

Dentigerous cyst

Cyst of the jaw :

a. Odontogenic
epithelium of tooth-forming tissues

Radicular cyst Dentigerous cyst Residual cysts Odontogenic keratocyst

b. Non-odontogenic

epithelium not involved with tooth formation

Dentigerous cyst
Benign odontogenic cysts, associated with crown of permanent unerrupted teeth Present at 2nd to 4th decade Most important features : Ability to expand Displace adjacent teeth or bone Trigger : periapical inflammation of adjacent teeth

Pathogenesis
Pressure erupting tooth on follicle Obstruct venous flow accumulation exudate between follicular epithelium and crown Encloses crown, attaching to the neck

Clinical presentation
Painless Palpable mass Displace adjacent teeth Most common location : Mandibular third molar Maxillary third molar Maxillary canine

Imaging modality
OPG CT MRI

QUIZ
A healthy 25-year-old man came to a dentist with chief complaint of mass in the right jaw, that had been present for several months. This is the image result : a. Please describe this picture b. What is the most likely diagnosis?

Odontogenic Lesions Associated with an Impacted Third Molar Tooth Lesion Pathologic Features Radiologic Features
Dentigerous cyst Fluid accumulation between crown of an unerupted tooth and follicular epithelium Crown of an unerupted tooth projects into well-defined cystic lesion; roots of tooth outside lesion Multilocular lucency in periapical; may be multiple; daughter cysts present; may expand and erode cortical bone and cause root resorption Ranges from expansile, unilocular to multilocular lucency with soft-tissue component; internal septations and soap bubble appearance; may expand into adjacent soft tissue with imperceptible bone wall

Keratocystic Thin, stratified keratinizing odontogenic squamous epithelium with tumor basal palisading Ameloblastoma Peripheral palisading and budding, unicystic

Odontogenic keratocyst

Figure 5a. Lateral oblique radiograph shows an ellipsoid, expansile, multilocular, corticated, lucent lesion occupying the anterior two-thirds of the left ramus with an impacted third molar crown displaced inferiorly within the lesion. The mandibular canal appears to be displaced inferiorly as well

Radiographic appearance
Lucent lesions Round or ovoid Well-defined Around the crowns of unerupted teeth
