Objectives
By the end of this lecture, you should be able to:
 Define admission of a patient.
Know the types of admission.
Identify the requirements for admission of a patien
t.
Know and carryout the procedure for admission
Theoretical notes:

• Patients are admitted to an inpatient healthcare facility


if their health condition requires continuous healthcare
for more than 24 hours.
• Admission is allowing and facilitating a patient to stay in
in the hospital unit or ward to observation, investigation
and treatment of diseases he or she is suffering from.
• The term “hospitalization” means admitting the patient
to an inpatient healthcare facility. Inpatient healthcare c
an be administered in various facilities such as hospitals
, sanatoriums, hospices or nursing care homes.

• Admission is always an experience to the patient which is full of anxiety and
uncertainties regardless of their age, nationality or educational background
and may include:
• Painful diagnostic or therapeutic procedures.
• Their family
• Their job/business loss
• Missing studies
• Terminal illness eg. Cancer, HIV.
• Loss of privacy.
• Hospital expenses..
• A positive therapeutic relationship between the patient and nurse should b
e maintained from admission and throughout patient's stay in the hospital t
o dispel patient's anxiety by being friendly, understanding, kind and helpful.
Forms of health care

• Outpatient care; General, specialized;


• Institutional care;
• Pharmaceutical service.
Types of patient admission
according to priority:
• Planned admission – the patient has been previously bo
oked for hospitalization, examination or surgery as decid
ed by the patient and physician and it is expected that th
e patient will remain in hospital for the required period.
• The hospitalization period starts after initial examination
s in the outpatient facility.
• Emergency admission – the patient is admitted without
referral from a doctor in the case of a life-threatening co
ndition.
• In this case the patient require urgent attention.
Requirements for patients'
admission.
• Patient's chart.
• Record book for patient's belongings.
• A pen.
• Admission book.
• Observation tray.
• Wheelchair or stretcher.
Procedure for patient admis
sion.
• Obtain patient's personal and sociodemographic data from the patient in order to
create a nurse patient relationship, allay anxiety, identification, legal purpose and
follow up.
• Take patient's height and weight and other vital observations to aid diagnosis and
treatment.
• Patient or N.O.K signs the admission document as a legal document for routine ca
re and treatment and the patient undertakes to abide by the conditions of admissi
on and hospital policies and a declaration that the information given is true and a
ccurate.
• Notify the ward of tje patient's arrival, preliminary diagnosis and name of the doct
or for the ward staff to prepare to receive the patient and proper management.
• Bed or room to be occupied by the patient must also be confirmed.
• Take a non-ambulant patient on a wheelchair or trolley to minimise energy expen
diture and worsening of condition.
• Help patients who have difficulty I walking with their luggage.
On arrival at the ward;
• The nurse should speak slowly and clearly.
• The case sheet is red carefully and the following par
ticulars are filled in the admission book.
Patient's full name.
Age.
Inpatient number/ hospital number.
Sex.
