A. Establishment of PuroKalusugan (PK)
• Organization of PuroKalusugan in all Barangays and
their Puroks
• Identification of at least three (3) Barangay (with
their respective puroks) per Municipality/City as
priority implementation sites for the initial
implementation of PK
• Provision of enhanced health services and additional
assistance/logistics support from the DOH for the
identified priority implementation sites

• Targeted Intervention: Focused on health promotion, wellness,
and nutrition at the primary level aligned with the services being
given in DOH’s 8 Priority Health Outcomes namely Immunization,
Nutrition, Maternal Health, WASH, TB and HIV AIDS Management
and Control, Road Safety, NCDs esp HPN and DM Control and
Mental Health, Cancer Prevention and Control and Hepatits B
Management
• Implementation sites may focus on at least three (3) programs

Barangay and its Purok
• PuroKalusugan Health Teams: PKTs that will provide direct health
service delivery at least twice a week to priority PK sites
• Core Members: (a) At least a nurse of midwife from the NHWSS,
(b) Other DOH-deployed HRH under the NHWSS, DOH-hired
HRH such as CHD-hired, DPCB-hired,HPB-hired or DOH-hospitals
deployed at LGU,(c) LGU-hired HRH, and (d) BHWs

The PKTs of a municipality/city shall be directly supervised by a


DOH Representative.

• There shall be at least one (1) PK team in each barangay
• The nurse or midwife from DOH-deployed HRH shall lead the PK
Team and shall directly report to the DOH Representative

• PK team composition may be expanded

• Different cadres of health care workers may potentially be
deployed depending on the identified needs and strategies to be
implemented.

contributions to the 8 Priority Health Outcomes.

Implementation and Sustainability of
PuroKalusugan
• Fund Review Allocation: Review budget allocations, identify
potential areas for reallocation.

• Maximize Patient Benefits: e.g. Konsulta packages
• Cost projection: Close coordination between CHDs and LGUs to
identify financial needs and implementation costs for potential
funding from the LGUs, Central Office and other investments on
health

E. Leadership and Governance
• Creation of PuroKalusugan Steering Committee and Technical
Secretariat: To provide overall leadership, implementation and
policy direction. Provision of direct oversight, through the CHDs.
• Establishment of the PuroKalusugan Technical Working Group
• Mobilization of PDOHOs and DOH Reps: Supervise and monitor
implementation and provide technical assistance
• Community Leadership and Engagement: Engaging formal and
informal leaders as Health Champions

F. Monitoring and Evaluation
• The performance monitoring and evaluation of DOH Bureaus,
Services, Offices and Centers for Health Development relating to
the PuroKalusugan initiative shall be reflected in their respective
Office/Individual Performance Committee and Review (O/IPCRs)
• Further, the performance of DOH-hired HRH, whether under the
NHWSS or those directly hired by DOH Offices/CHDs, deployed
for the implementation of the PuroKalusugan or as members of
PKTs, shall be monitored and evaluated based on indicators
aligned with the DOH’s 8 Priority Health Outcomes as reflected
in their respective IPCRs.

• The highest total score value is 8. Areas will be ranked according to the
total score value. The M/CHOs may select at least 3 priority
barangays with their respective puroks as the initial implementation
sites. The sites may be expanded or adjusted depending on the
identified gaps and resources available in the communities until all the
areas identified in the ranking are served. If the resources available
cannot meet the needs of all identified puroks, those with a higher
number of households may be prioritized.
