Patient R is a 15-year-old male who was admitted to the hospital with difficulty breathing, edema, and mild ascites. He has a history of recurrent sore throats and fevers since age 5. A recent diagnosis of rheumatic heart disease and pleural effusion on the left side led to his hospitalization. He comes from a low-income family living in an urban area with risk factors for streptococcal infection like crowded conditions. His physical exam shows edema, elevated heart rate and respiratory rate, and a fever.
