Addressing Health Care's
Blindside in Albuquerque’s South
Side: Logic Model Workshop
Why the logic model?
• Orient/educate others about what we are doing
• One-pager for leadership to understand the overall objectives
• Understand the bigger picture and key players
• See how we all fit in and what our contributions are
Why the logic model? (cont.)
• Clearly identify how we reach outcomes
• What does success look like?
• The logic model helps the team to keep focus on the many parts of project
and keeps those desired outcomes in mind at all times--short and long-
term
• Barriers are expected- change in the project team, movement towards
other areas, etc. the model can prevent ‘drift’ or scope change
“…it is inspiring to see the bigger picture in terms of the
impact on the community and health. I usually deal in
the day to day referrals, phone calls and resources and
we do see good impact there, but to view the larger
vision of how the community will be transformed, helps
me and the team move forward with enthusiasm.”
Meta Hirschl, Adelante
