CONTENT
DEFINITION
The Atraumatic Restorative Treatment (ART) is a procedure based on
removing carious tooth tissues using hand instruments alone and restoring the
cavity with an adhesive material.
INDICATIONS
Atraumatic restorative treatment is carried out :
• Only in small cavities( involving dentine)
• In those cavities that are accessible to hand instruments.
CONTRAINDICATIONS

ART should not be used when:
There is presence of swelling (abscess) or fistula near the carious tooth.
The pulp of the tooth is exposed.
Teeth have been painful for a long time and there may be chronic
inflammation of the pulp.
Essential Instruments and Material
• MOUTH MIRROR : The instrument is used to reflect light
towards the field of operation in order to view the cavity
indirectly and to retract the lip, cheek and tongue.
• EXPLORER : The instrument is used to identify the depth of
caries.
• TWEEZER: The instrument is used to carry cotton rolls,
wedges and articulating paper from the tray to the mouth.
• Spoon Excavator: The instrument is used to remove soft
dentinal caries.
• DENTAL HATCHET: Used for widening the entrance of cavity.
• CARVER: Blunt end is used for inserting the mixed glass ionomer into the
cleaned cavity and into pits and fissures. The sharp ends is designed to
remove excess restorative materials and to provide shape to the glass
ionomer.
• PETROLEUM JELLY: The material is used to keep moisture away from the
glass ionomer restoration and to prevent the examination gloves from
sticking to glass ionomer and gets hard.
• COTTON WOOL ROLLS: They are used to absorb saliva so that the tooth to
be treated can be kept dry.
ARRANGEMENT IN THE MOUTH
• A very important aspect for ART is the control of saliva around the tooth to
be treated.
• Location in the mouth and method of placement of cotton roll is:
• Upper Teeth: Retract the lip and cheek with mouth mirror to make space
for the placement of cotton roll. Place the cotton roll in position with a
slight rotating action from the tooth towards gingiva. This will prevent the
cotton roll from coming out easily.
• Lower Teeth: Ask the patient to stick the tongue out. Push the tongue aside
with the mouth mirror. Place a cotton roll on each side of the floor of
mouth. Then ask the patient to retract the tongue back to its normal
position.
PROCEDURE FOR ART
• The tooth is isolated with cotton rolls.
• The tooth surface to be treated is cleaned with cotton pellet
• The entrance of the lesion is slightly widened by hand
instrument to remove overhanging unsupported enamel rods
with enamel hatchet.
• The dental caries is removed by using either the small or
medium sized spoon excavator.
• If necessary, provide pulpal protection by calcium hydroxide.
• The cavity surface along with occlusal margins are cleaned.
• The cavity is acid etch.
• Glass ionomer cement is mixed.
• The mixed glass ionomer is inserted into cavity and slightly overfilled.
• A gloved finger which is smeared with petroleum jelly is pressed on top of
the entire occlusal surface and slight pressure is applied.
• The bite is checked.
• Excess material is removed with a sharp carver.
• The bite is rechecked and all high points are removed.
• The filling is covered with petroleum jelly or varnish may be applied.
• The patient is instructed not to bite with the tooth for at least half an hour.
ADVANTAGES OF ART
Easily available hand instruments are used rather than the expensive
electrically driven dental equipment's as it is a painless procedure the need of
local anesthesia is eliminated.
ART involves only the removal of decalcified tooth tissue which results in
small cavities and conserve sound tooth structure as much as possible.
The combined preventive and curative treatment can be done in one
appointment.
It is cost effective and less time-consuming procedure.
DISADVANTAGES OF ART
• Atraumatic restorative treatment procedure are not long lasting.
• The average life is 2 years depending upon the rate of caries activity of
individual oral cavity.
• Because of lower wear resistance and low strength of existing glass
ionomer materials their use is limited to small or medium sized one
surface cavity only.
• The continuous use of hand instruments over long period of time may
result in fatigue.
• A relatively unstandardized mix of glass ionomer may be produced due to
hand mixing.
