Performance is rated on a scale from "Outstanding" to "Unsatisfactory" for factors like administration, communication, teamwork, decision making, and managing change. Sample phrases are provided for evaluating attitudes, creativity/innovation, and decision making. The form also includes spaces for employee and supervisor signatures.
