CONTROLLED SUBSTANCES:
“The drugs that may have a high potential for
abuse, lack of accepted safety for use and the
use of which may lead to severe psychological
or physical dependence are known as
controlled drugs.”
OR
Substances, products or preparations,
including certain medicines, that are either
known to be, or have the potential to be,
dangerous or harmful to human health,
including being liable to misuse or cause
social harm, are subject to control under the
Misuse of Drugs Acts 1977 to 2016. They are
known as “controlled drugs”.

SCHEDULES OF CONTROLLED
DRUGS:
• Controlled drugs are categorized into 5 schedules
by Misuse of Drugs Regulation 2001:
• Schedule 1(e.g : marijuana)
• Schedule 2(e.g: cocaine)
• Schedule 3(e.g: steroids, benzodiazepenes)
• Schedule 4
• Part A(e.g: most benzodiazepenes and z drugs )
• Part B (e.g: certain anti-epileptics and MAOIs)
• Schedule5(e.g: low strength painkillers)
CATEGORIES OF CONTROLLED
DRUGS:
• Apart from schedules CDs can also
be categorized into three classes — A, B
and C.
• Class A drugs (Schedule – I) are considered
most likely to cause harm and carry the most
severe punishment for unlawful possession
and supply
• Class B means Schedule – II and
• Class C means Schedule III to V drugs.

The responsibilities of hospital administrator
include:
• Policy making regarding
• Ordering
• Prescription
• Dispensing
• Administration and documentation of
controlled dugs
POLICY MAKING FOR ORDERING
CDS:
• Preparation of orders
• Doctor’s order and signature
• PRN or SOS orders
• Telephone orders
• Verbal orders
• Ordering non ward CDs from pharmacy
PRESCRIBING:
1. Prescription for own personal use
2. Refilling
3. Prescribing controlled drugs in outpatient
department:
prescriptions for controlled drugs
maybe dispensed to outpatients from
pharmacy.
DISPENSING:
1. For home use
2. For employees
3. Dispensing for home use during
pharmacy-off hours
ADMINISTRATION:
Administration by nurses:
The nurse should use
proper number of tablets or ampules from
nursing stock and should also record the
number and doses administered.
Information necessary on daily controlled
drugs administration sheet:
The information
required is date, amount given, patient’s full
name, patient’s hospital number, amount
administered, filling out inventory column.
DISPOSAL OF CONTROLLED
DRUGS:
1. Aliquot part of controlled drug solutions
2. Refusal of prepared dose by the patient or
cancellation by doctor
3. Accidental destruction and contamination
of drugs
DELIVERY OF CONTROLLED
SUBSTANCES TO NURSING
STATION:
The delivery must be done by a reliable
person of hospital pharmacy, nursing staff or
the messenger staff.
DOCUMENTATION:
Record keeping of all activities during the
handling of controlled substances must be
done.
An efficient record must be maintained that
can be audited periodically .
ROLE AND RESPONSIBILITIES OF
PHARMACY DEPARTMENT:
1. System of controlled drug loss report
2. Controlled drug delivery system for
nursing station
3. Protocol for drug abuse and diversion
4. Documentation
NURSES’ RESPONSIBILITIES:
• Auditing of narcotic inventory
• Physical count of narcotics
• Monitoring on audit record
• Forwarding of loss report to pharmacy
department
USE OF CONTROLLED DRUGS IN
RESEARCH:
Notification to pharmacy and therapeutic
committee:
The person engaged in research
regarding the controlled drugs is obligated to
notify the institutional pharmacy and
therapeutics committee.
Documentation :
The person doing research is
bound to document each and every step of
his research .
