![PLCMC CSC ELECTION 2011
NOMINATION FORM
To encourage broader participation by members of the CS congregation, the CS-Election
Committee (EC) invites nominations for CS Committee members from full members, for
the following positions (see overleaf). Please ensure that another full member signs as
seconder. You may refer to Job Description folder/Website for list of current CSC
positions and job descriptions. All completed forms must be submitted to the Church
Office/dropped into the Nomination Box no later than 28 August 2011. No late
nomination will be accepted.
Pre-requisites for Nomineed
The nominee must be:
• Aged 16 and above. A full member of PLCMC and has attended PLCMC CS for a
reasonable period of time.
• A person of good character, temperate, faithful in all things, and, most importantly, a
sincere Christian (reference 1 Tim 3:1-12)
• Be in agreement with and convicted to fulfill the CS Vision to “Grow & Multiply”.
I’ve read and understood the pre-requisites for nomination of chairpersons and
coordinators of ministries/committees in CSC. I hereby nominate the following person for
the position as indicated on the reverse.
