Project Orientation The school head convenes all the teachers and other concerned school personnel and orients   Program of the activity,

  Implementation Phase      
  Distribution of Assent and Consent Teachers distribute the Assent and Consent forms to the identified participants.   Received Assent and Consent

Delivery of Modules Each container has a name of intended learner to be delivered by 25 BHW riding in a tricycle . Delivered the modules
. These modules will be handed to the parents of learners at home with an information that these
will be retrieved on the fifth day of the week – 3:00 in the afternoon

  Actual Dry-Run of Modular Instruction The parents or guardians executes the modular instruction.    

  Pre-test A pre-test is given to measure the level of knowledge of the learner before the modular   Pre-test scores
instruction.
  Day-to-day Modular Instruction The modular instruction is implemented.    
  Posttest A posttest is given to measure the level of knowledge of the learner after the modular instruction.   Posttest scores

Retrieval of the Modules On the fifth day ( Friday ) at 3:00 oclock in the afternoon Retrieved the Modules
modules will be retrieved / collected by the volunteer parents/
barangay officials to be forwarded t the concerned teachers.
 
  Observation and Interview of Teachers Teachers observe the conduct of the modular instruction at home directly or indirectly. They need   Observation notes, interview
with Participants to note anecdotal records of significant events. Interview with the learner and parent/guardian transcripts
may be done on Day 1, Day 3 and last day of the instruction.

  Post-Implementation Phase      
  Consolidation of Findings Teachers consolidate all the feedbacks from the learners, parents/guardians. They also consider   Consolidated findings
their own notes during the observation as well as the results of the tests.

  Presentation of Findings The general findings are discussed by the teachers to all school personnel. This presents the   Summary of findings
realities in the ground which can be basis for interventions.

  Crafting of Intervention Plan for the New The school head leads the formulation of the intervention plan which seeks to increase the   Approved Intervention Plan
Normal readiness of the learners, parents/guardians and school on the modular instruction. The plan will
also mitigate the potential risks during the implementation.

  Presentation of Intervention Plan to the The school head presents the Intervention Plan as the operational plan during the period of   Stakeholders' support to the
