SCOPE OF WORK
&
SPECIFICATION
Project : PROPOSED CONSTRUCTION OF COGONAN BHS
Location : Brgy. Cogonan, Municipality of Labangan, Zamboanga del Sur
Date : October 23, 2015
GENERAL REQUIREMENTS:
The Contractor is limited to the choice of brand, make, source, manufacturer, installer or specialty trade
contractor specified herein.
When an “equal “or “equivalent “is placed immediately after the specified materials, the contractor/s is
allowed to make substitution. However, the Owner and the Supervising Architect / Engineer reserve the right to
reject the proposed substitution.
SCOPE OF WORK:
The proposed work shall consist of furnishing and providing all labor, tools, equipment, utilities,
supervision and satisfactory performance of work and operations necessary to complete the Proposed
Construction of COGONAN Barangay Health Station (BHS) located at Brgy. Cogonan, Municipality of Labangan,
Zamboanga del Sur.
The major works to be undertaken shall include, but not limited to the following:

V. Carpentry Works
VI. Doors and Windows
VII. Steel Works and Signage
VIII. Roofing Works
IX. Tile Works

I. LAYOUT AND EXCAVATION
The works to be undertaken in this item includes setting out of lines and grades in the areas of the
building construction prior to any excavation works. Services and site works within the contract shall be set
out by suitable hurdles, pegs or markers and levels shall be established and pegged from a common datum.
All setting out shall be carefully checked against the drawings prior to commencement and any
discrepancies clarified before construction commences. Building dimension is 6.80m x 8.00m.
EXCAVATION:
Excavation shall be taken out accurately to the lines, levels, grades and sections shown in the
plan/drawing. Areas to be excavated will be for footings, wall footings/zoccalo, column footings, septic tank
and placenta pit with a total volume of 26.81 cubic meters.

[email protected] CLEARING & GRUBBING:
Clear area which building and related construction and site grading are to be accomplished of
matted roots, brush, snags, rubbish, spoils and other objectionable materials. Spoiled materials shall be
removed to approve disposal areas.
LINE & GRADE:
Grade areas within limits and to elevations indicated on the plan. Stake out accurately lines of
building within the lot and all bearings and other measurements shall be carefully verified and plans
properly coordinated.
All materials for backfill must have the approval of the Supervising Engineer/Architect.
II. EMBANKMENT WORKS
Embankment construction shall consist of constructing embankments including preparation of the
areas upon which they are to be placed; the placing and compacting of approved material within the areas
where unsuitable material has been removed; and placing and compacting of embankment material in
holes, pits and other depressions within the area.
Embankments and backfill shall contain no muck, peat, sod, roots or other deleterious matter.
Where there is evidence of discrepancies on the actual elevations and that shown in the plans, a
preconstruction survey referred to the datum plane used in the approved plan shall be undertaken by the
Contractor under the control of the supervising engineer to serve as basis for the computation of the actual
volume of the embankment materials.
Initial volume of filling materials to fill in shall be provided to elevate the proposed construction of
the building equivalent by 0.30m from natural ground line, or 17 cubic meters of earth, with finish floor
level at 0.40m elevation creating two steps. However, with the actual terrain and level of ground of the
project site being hilly or too low such that rainwater may certainly flow into the building, another volume
of filling materials equivalent to 22 cubic meters must be provided to raise the building further by 0.40m to
such level that is safe from flood.
III. REINFORCED CONCRETE WORKS
REINFORCED CONRETE:
All concrete works shall be done in good workmanship and conform to the standard practice in
government projects unless otherwise specified. Concrete works with a volume of 10.85 cubic meters are
for column footings, columns, floors, steps, ramp, roof beams and lavatory counters. Metal reinforcement
for all concrete works shall be included as indicated in the plan.
PROPORTIONING OF CONCRETE MORTAR:
All concrete work shall be done in accordance with the Standard Specifications for Concrete
Materials as adopted by the government. The mixture shall be preserved.
Class “A” – (1:2:4) mixture for all slabs and other concrete works.
All provisions of the latest National Structural Code for Buildings and the American Concrete
Institute’s Building Code Requirements For Reinforced Concrete shall be made part of this Specification.
STEEL REINFORCEMENT:
Use 16mmØ deformed bars as main reinforcing bars for footings, columns, and 12mmØ for roof
beams. Deformed bars 10mmØ will be used for lateral ties, stirrups, and temperature bars for floor slabs.
Metal reinforcement shall meet the requirements of standard specification for billet steel reinforcement
bars of the American Society for testing materials or its nearest equivalent in metric. Also, GI #16 tie wires
shall be used as ties on columns, stirrups and temperature bars.

[email protected] SAMPLE AND TESTS:
Cement, gravel and concrete samples shall be tested by reputable testing laboratories as required
by the structural engineer. Sample and fees shall be borne by the contractor.
In the event of conflict or ambiguity with other parts of the contract documents, the supervising
engineer and/or structural engineer should be consulted.
For schedule of reinforced concrete, refer to the construction notes on structural plans.
MATERIALS: VERIFY STRUCTURAL PLANS
a. Portland Cement: ASTM C 150, TYPE I, only approve brand shall be used;
b. Gravel: G-1 well graded, clean hard, particles of gravel or crushed rocks shall be used.
c. Sand: S-1, washed, clean, greenish in color.
d. Water: clean, free of deleterious materials which would be impair strength of bond.
CONCRETE MIXTURE:
Refer to general specification as to mixture, strength and proportion.
VIBRATOR:
All concrete slab 100 mm thick shall be compacted with high frequency internal mechanical
vibrating equipment supplemented by hand spading and tamping. Vibrators shall be designed to operate
with vibratory element submerged in the concrete, and shall have a frequency of not less than 6,000
impulse per minute when submerged.
IV. MASONRY WORKS:
The Contractor shall hire competent masons with adequate tools and equipment required for the
execution and proper completion of all masonry works as shown and indicated in the plan. A net area of
156.6 square meters shall form part of this item. The work includes plastering and finishing for all CHB walls
and partitions, 231.9 square meters in surface area, and to include also window crown/mouldings.
CONCRETE HOLLOW BLOCK:
Concrete hollow blocks 4” x 8” x 16” shall be erected for perimeter walls and partitions. Joint fillers
and mortar for block laying shall be concrete of the class “B”, with ratio 1:3.
All walls shall be provided with vertical bars 10mmØ spaced at 0.60m and horizontal bars 10mmØ
at every three layers tied and securely anchored at the foundation.
All hollow blocks shall be laid plumb and true to line.
V. CARPENTRY WORKS
Installation of ceiling shall use 3.5mm x 4’ x 8’ fiber-cement boards fastened to metal furring double
0.400mm x ¾” x 2”, with a total ceiling area of 100.1 square meters for both interior and exterior.
Also, included in this item are fabrication and installation of air ventilation at exterior ceiling to
locate at all corners, and fabrication and installation of door jambs.
Lumber shall be of approved quality of the respective kind required for various parts of the work,
well-seasoned, thoroughly dried and free from loose and unsound knots, sap or other imperfection which
may impair its strength, durability or appearance.
Framing lumber for door jambs shall be of rough dimension. All exposed woodwork shall be
smoothly dressed and well-seasoned and sand papered. The kind of lumber to be used shall be or as
indicated in the approved plan.
All scaffolding and staging shall be of coco lumber and using 1/4” thick ordinary plywood as forms.

VI. DOORS AND WINDOWS
DOORS:
In this item includes pre-fabrication and installation of the following, with details and openings
indicated in the plan:
1 set (D1) Panel-type Double Door 1.60m x 2.10m w/door knob, hinges, foot bolts & overhead chain bolts
2 sets (D2) Flush-type Double Door 1.50m x 2.10m with Inset Bronze Glass ¼” x 0.20m x 0.40m, with door
knob lever-type, hinges, foot bolts, and overhead chain bolts
1 set (D3) Panel-type Door 0.80m x 2.10m with door knob and hinges
1 sets (D4) PVC Door 0.70m x 2.10m with door knob and hinges
WINDOWS:
In this item includes supply and installation of the following:
5 sets (W1) Jalousie Glass Window 1.40m x 1.18m, analok-frame, 2-panel, 13-blade ¼” bronze glass
1 set (W2) Jalousie Glass Window 0.70m x 1.18m, analok-frame, 1-panel, 13-blade ¼” bronze glass
2 sets (W3) Jalousie High Window 0.70m x 0.56m, analok-frame, 1-panel, 6-blade ¼” bronze glass
2 sets (W4) Jalousie High Window 1.40m x 0.56m, analok-frame, 2-panel, 6-blade ¼” bronze glass
They shall be done as indicated and shown in the approved plan. All materials shall be in true size
and schedule. A total of 15 job orders shall form part of this work item: 5 doors and 10 windows. Refer to
architectural drawings and details.
VII. STEEL WORKS AND SIGNAGE
Works under this item shall include fabrication and installation of steel trusses made of angle bars
3.5mm x 2 1/2” x 2 1/2” with designs, lengths and sizes as indicated in the detailed drawing/plan, and are
anchored/welded on roof beams. Purlins shall use 1.5mm x 50mm x 100mm Channel Purlins attached to the
trusses at 0.60 meter spacing on centers. Sag rods using plain bars 12mmØ shall be attached to each and
every purlin to prevent sagging and torquing, as indicated in the drawing plans. A total of 104.63 square
meters of roof area shall be considered.
Framing for fascia boards shall be fabricated, using angle bars 3.5mm x 1-1/2” x 1-1/2” for
horizontal members and flat bars 3.5mm x 1” for vertical studs, attached to trusses along roof eaves 0.25m
in depth to which the fascia cover shall be fastened.
Galvanized Iron (GI) pipe 50mmØ shall be used for railings at ramp with sizes and dimensions as
indicated in the drawing plans.
Iron grilles shall be fabricated to fit into all the windows. Framing shall be of flat bars 5mm x 1” and
grilles shall be of plain square bars 10mm x 10mm.
All materials shall be in true size and schedule, refer to structural drawings and details. The
contractor shall provide temporary erection bracing and shoring and make actual measurements in the field
prior to fabrication/installation of c-purlins.
The fabricator/contractor shall submit shop detailed drawings prior to installation for approval of
the supervising engineer and test shall be conducted on welded connections.
Signage to exhibit the name of the facility and the seal of the Department of Health shall be
installed as shown in the plans. Pre-fabricated stainless letterings shall be used to compose the name, 8”
(20cm) in height, for the barangay name “COGONAN”, and 4” (10cm) for “BARANGAY HEALTH STATION”.
The seal shall be made of acrylic plastic 5mm thick, colored-print, 500mm in diameter.
VIII.ROOFING WORKS

MATERIALS AND INSTALLATION:
Works under this item shall include installation of Pre-painted Long-span Roofing Sheets, 0.400mm
in thickness, fastened with tekscrews for steel on metal purlins, covering total roof area of 99.0 linear
meters. Fascia cover shall also be installed along the perimeter of entire roof. It shall be of pre-painted pre-
bent sheets fastened with rivets onto steel fascia framing. Refer to drawings for size and dimension of roof.
IX. TILE WORKS
A. For Comfort Room, unglazed ceramic floor tiles 8” x 8” shall be used on floors set on 25mm
thick mortar setting bed complete with all trimmings. Glazed ceramic wall tiles 8” x 12” shall be used on
walls set with adhesive 6 layers or 1.80m in height. Total tiling area is 15.69 square meters. For setting of
tiles, use tile grout with 1mm spacing in between tiles. Wall and floor edges shall be complete with
trimmings using pvc tile trim 9mm which color matches the tile. Submit tile samples for
Engineer’s/Architect’s approval.
B. For lavatory counters, granite tiles 60cm x 60cm shall be used on all countertops, splashboards
and risers for a total of 4.90 square meters tiling area. This includes also the tiling of 25cm-deep wash sink
for Scrub-up Area. Tiles for splashboard shall be 0.30m high and countertop riser shall not exceed 0.10m.
Edges on splashboard and countertop shall be trimmed with pvc tile trim 9mm which color matches the tile.
C. All interior floors including Porch, Back Exit Landing, and steps shall be furnished with granite
tiles 60cm x 60cm, light in color for interior, covering a total floor area of 56.74 square meters. Submit tile
color sample for Engineer’s/Architect’s approval. Ramp shall be rough finished.
X. SEPTIC TANK AND PLACENTA PIT
This item comprises of excavation equivalent to 11.88 cubic meters for the construction of septic
tank and placenta pit. The size of septic tank shall be 1.80 meters in width, 3.00 meters in length, and 1.80
meters in depth, with two compartments, with design and other details indicated in the plans/drawings. For
the placenta pit, the size shall be 1.20 meters in width, 1.20 meters in length, and 1.50 meters in depth,
with design and other details indicated in the plans/drawings.
Cement-plastering mortar for walls shall be mixed with adequate water-proofing additive.
XI. ELECTRICAL WORKS
SCOPE:
The work included under this item comprises the furnishing of all labor (including supervision and
tools) and materials, unless otherwise specified herein, required to complete and leave ready for use, this
installation of the following items in accordance with this technical specifications and accompanying
drawings.
Furnishing and installation of service complete with conductors, conduits and other accessories
necessary for electric service for power and lightning.
CODES, INSPECTION, PERMITS & FEES:
The installation and equipment shall conform to good engineering practice and in particular,
comply with the requirements laid down in the following documents or its equivalent which are mandatory
or modified only by specific agreement.

ELECTRICAL MANUFACTURERS’ ASSOCIATION (NEMA)
In addition to the requirements of the Codes and Specifications referred to the above, National,
City, Provincial, Municipal Laws and regulations or district having jurisdiction and the Public Utility Company

[email protected] furnishing the electrical services shall be followed without any conflict with the above rules and regulations
and standards.
MATERIALS:
All materials shall be new and shall conform to the Standard Underwriters Laboratories, Inc. or
Bureau of Labor Standards.
WIRES:
Wires shall be 3.5mm2 thermoplastic wires (TW) approved for use in hazardous and wet locations.
OUTLETS BOXES & FITTINGS:
All boxes shall be PVC (high impact & heat resistant) of standard make at all outlets on concealed
work except for the terminal box and boxes which comprises the main circuit board or panel board which
will be thicker in gauge.
LIGHTING FIXTURES:
Lighting fixtures shall be as indicated in the plans and/or schedule of lightning fixture. Use only
lightning and blast made from Philips as indicated in the plans or its approved equivalent unless otherwise
modified with the consent and approval of the Professional Electrical Engineer.
WIRING METHOD:
All wirings shall be installed in standard uPVC plate embedded in concrete slabs and hollow blocks
structures run concealed in wall partitions and inside spaces between ceilings, unless otherwise specified.
GUARANTEE:
The electrical contractor shall guarantee his work for a period of one (1) year from the date of final
acceptance by the owner, except for particular items specifically mentioned elsewhere in these
specifications. The electrical contractor, shall without additional compensation for the specified, replace any
work, materials furnished and installed by him under contract develop defects due to poor workmanship
and materials except from ordinary wear and tear.
XII. PLUMBING WORKS
SCOPE:
The Contractor shall furnish materials necessary for the complete installation of the plumbing
system, including all piping connections to the septic tank/placenta pit and/or street sewer and water
supply to all fixtures and to the water storage tank and to local water supply mains or to drilled well water
supply source.
ROUGHING-IN:
“Roughing-in” for pipes and fixtures shall be carried along with the constructions of the building
preferably exposed and not objectionable in the opinion of the architect/engineer.
PIPING:
Sanitary piping shall be PVC Pipes S-1000 unless otherwise specified or it’s approved equivalent.
All pipes shall be embedded in concrete and masonry walls. Pressure leakage test are to be made
before water supply pipes are covered or embedded in concrete.
PVC Sanitary pipes in trenches or earth shall be laid true to line on a stable and suitable prepared
foundation, each section of pipe being properly bedded. In soft ground liable to settlement, provide a gravel
base twelve (12) inches deep and tampered. Backfilling shall be carefully placed and tamped under and
around the pipe, in such a manner that the pipe lines or connections are not disturbed.
TRAPS:

[email protected] Every plumbing fixture shall be separately trapped by a vented water sealed trap, placed as closely
as the conditions allow, but in no case at a greater distance than twenty (20) inches except that in the case
of an upper or only fixture on a soil pipe, extended full size through thereof, said fixture having the center of
the stack a vent will not be required.
LOCKS AND VALVES, EXPOSED PIPES:
Each fixture with supply pipe branch shall have a valve between the main and the fixtures so as to
allow the fixtures to cut-off for repairs.
SEPTIC TANK AND PLACENTA PIT:
Included in this item are roughing in of plumbing fittings and appurtenances of the septic tank and
placenta pit. All materials shall be of good quality and should be in accordance with the standards and
specification as indicated in the plan or as required by the supervising engineer/registered master plumber.
XIII.PAINTING WORKS
The painting work shall be for all masonry finishes (exterior and interior), ceiling finishes, painting
of wooden materials and ferrous materials as specified in the plan. Painting covers a total area of 387.40
square meters for all three different types of surfaces.
All paint shall be of good quality. All surfaces to be painted shall be clean and well sand papered to
smooth and all holes shall be covered with putty. Apply two (2) coatings for topcoat to even the application.
Paint colors should be consulted to the supervising engineer prior to application.
CLEAN UP:
All excess of excavation, aggregates and other debris shall be leveled with the ground and
disposed-off properly. All concrete spilling shall be properly scrapped-off and broom-cleaned.
All other items shown in the plan that may not be mentioned in this SCOPE OF WORK and
SPECIFICATION shall still form part of this AGREEMENT.
