The College of Education, which has been designated a Center of Excellence in Teacher Training

by the Commission on Higher Education (CHED), is committed to quality pre-service education.


It is on this premise that we require our students to enroll in experiential courses that will make
them more globally competitive and competent teachers in the future. One of these courses is the
Teaching Internship, which helps the college verify the knowledge, skills, and values that our
students have developed during their previous semesters at the university. The said internship
necessitates full-time teaching exposure in basic education schools using a clinical approach under
the mentorship of a cooperating teacher (CT). Similar to field study courses, this course is based
on the different aspects of good teaching to make sure that high standards of personal and
professional development are met.

In this regard, we are seeking your consent to allow your child, ___________________________

Dasmarinas Integrated High School
internship at ________________________________________ (name of the cooperating school).
The teaching internship will begin on February 27, 2023, and will last until the 360 duty hours are
completed. By signing this consent form and attaching one valid ID, you understand the nature of
the teaching internship and so as the responsibility of your child to stay safe and healthy during
the entire duration of the internship.

Your consent on this matter will provide a wide array of opportunities for self-reflection and the
chance to accomplish the expected teaching-learning activities for your child. Thank you for the
continuous support you are giving your child and the College of Education.
