UNDERTAKING

I, GRACE O. DEMOY, of legal age, Filipino and with residence address at Blk. 60 lot 26
SunFlower, Southfairway Homes, Landayan, City of San Pedro, Laguna, after being duly sworn in
accordance with law, depose and state that;

1. I am the registered owner of SACHIELAE WATER REFILLING STATION situated at Blk.


60 lot 26 SunFlower, Southfairway Homes, Landayan, City of San Pedro, Laguna.

2. I am applying for a Certificate of Occupancy for the aforesaid refilling station with the Office of
the Building Official ("OBO") of the City of San Pedro, Laguna;

3. However, the OBO did not issue the said Certificate because I covered the pathway in front of the
refilling station. attached herein the before and after photo;

4. I will undertake that if I refill/sell my water in front of the house instead inside of the house,
Office of the Building Official ("OBO") will file the appropriate and necessary legal actions;

5. This undertaking is being executed in compliance with the application for Certificate of
Occupancy and for whatever legal purposes it may serve.
